Isolated Penile Torsion in Children

Aim: The aim of this study is to report the incidence of isolated penile torsion among healthy children population and our approach to this anomaly.

Patients and methods: Newborn babies were examined by the same pediatric surgeon before circumcision. Healthy newborns who were born at our hospital were enrolled to the study until the number of 1000 was reached. Babies who had penile torsions were classified according to the angle of torsion. Newborn circumcision was carried on for the ones with less than 450. Surgical correction after six months was advised to the babies with higher degrees of rotation. The patients were controlled on the 7th day after surgery. The parents were called by phone, questioned about the final appearance and called for a control exam.

Results: 1000 completely healthy newborn babies were examined. 200 isolated penile torsions were found. The mean angle was 30.450 (median 200). 157 of them had torsions less than 450, and the remaining had more than 450. All of the 157 babies were circumcised. Surgical correction was performed on 19 patients at the mean age of 12±2 months. Thirteen were corrected with the degloving and reattachment technique. On the remaining six patients, dorsal dartos flap was added to correct the torsion. The control examination on the 7th day was normal. After a mean of 15.6±9.8 months, the parents were still happy and satisfied with the cosmetic appearance on phone contact. 10 patients came for control examination. All had normal appearance, except two, who had residual penile torsions. 

Conclusion: The incidence of isolated penile torsion is 20% in healthy newborns.  Correction is not necessary in cases with less than 450. Penile degloving and reattachment is enough to correct the anomaly in most cases. 

Çocuklarda izole penil torsion

Amaç: Bu çalışmanın amacı sağlıklı çocuk populasyonunda izole penil torsiyon sıklığını araştırmak ve bu anomaliye bizim yaklaşımımızı bildirmektir.

Hastalar ve metod: Sünnet öncesi yenidoğan bebekler aynı çocuk cerahı tarafından muayene edildiler. Çalışmaya hastanemizde doğan, sağlıklı, görünür anomalisi olmayan bebekler alındı ve 1000 sayısına ulaşıncaya kadar çalışma devam etti. Penil torsiyonu olanlar torsiyonun derecesine göre sınıflandırıldılar. 45 derecenin altında olanlara rütin sünnet yapılırken, daha yüksek derecede torsiyonu olanlara 6 aylıktan sonra cerrahi düzeltme önerildi. Cerrahi yapılanlar ameliyat sonrası yedinci gün erken kontrol edildi. Son durumları için ebeveynler telefonla arandı, son durumları hakkında bilgi alındı ve gelebilenler kontrole çağırıldı.

Sonuçlar: 1000 tamamen sağlıklı yenidoğan bebek muayene edildi. 200 izole penil torsiyon saptandı. Rotasyonlar ortalama 30.45 dereceydi (ortanca 20 derece). 157 tanesinde 45 derecenin altında, geri kalanlarda 45 derecenin üzerinde torsiyon saptandı. 157 bebeğin hepsine sünnet yapıldı. Ortalama yaşı 12±2 ay olan 19 hastaya cerrahi düzeltme yapıldı. 13 tanesi degloving ve reattachement tekniği ile düzeltildi. Geri kalan altı tanesine dorsal dartos flebi eklemek gerekti. Ameliyat sonrası bakımda herhangi bir problem yaşanmadı. Ameliyattan sonra yedinci gün yapılan  kontrol muayenelerinde komplikasyon yoktu ve ebeveynler sonuçlarla tatmin olmuş durumdaydılar. Ortalama 15.6±9.8 ay sonra ebeveynlere telefonla bağlantı kuruldu ve hepsinin kozmetik görünümden mutlu ve memnun oldukları öğrenildi. Telefon bağlantısından sonra 10 hasta kontrole geldi. Rezidü penil torsiyonu olan ikisi dışında hepsinin görünümü normaldi.

Yorum: Sağlıklı yenidoğanlarda izole penil torsiyonun sıklığı %20 dir. 45 derecenin altında olanlar düzeltme yapmak gerekli değildir. Penil degloving ve reattachement tekniği çoğu vakada anomalinin düzeltilmesi için yeterlidir.
