The comparison of prophylactic topical estrogen and betamethasone after manual separation in patients with treatment-resistant and recurrent labial fusion

Aim: The aim of this study was to compare the use of prophylactic topical estrogen and 0.05% betamethasone after manual separation in terms of side effects and recurrence in treatment-resistant patients with recurrent labial fusion.

Material and Method: Patients who had only used topical estrogen, had undergone only manual separation, and had undergone manual separation together with topical estrogen use for labial fusion at external centers were included in the evaluation. All patients underwent manual separation following 5% lidocaine ointment application at the outpatients department. After the procedure, the first group of patients was administered topical estrogen and the second group was administered 0.05% betamethasone for 3 weeks. The patients in both groups were evaluated in terms of possible side effects and recurrence of the disease.

Conclusions: A total of 146 patients presented to our outpatients department due to labial fusion between December 2011 and April 2014. Of these patients, 41 (28%) were not included in the study as it was their initial presentation. The first group consisted of 55 patients (52.3%) and the second group of 40 patients (47.7%). The mean age was 19.47±16.61 (2-74) months for the first group and 16.25±12.28 (4-48) months for the second group. Previous topical estrogen therapy had been prescribed for 23 patients (41.8%) in the first group, and 16 (40%) in the second group. Manual separation had been used for 17 patients (30.9%) in the first group and 11 patients (27.5%) in the second group. Manual separation followed by topical separation was used in 15 patients (27.2%) in the first group and 13 patients (32.5%) in the second group. Side effects of breast enlargement (n = 2), hyperpigmentation (n = 1), and vaginal bleeding (n=1) were found in 7.2% of the patients in the first group, and only local irritation was found in one patient (2.5%) in the second group. The recurrence rate was 23.6% (n=13) in the first group and 7.3% (n=3) in the second group.

Discussion: The use of low-dose betamethasone prophylactically in patients with symptomatic labial fusion who have undergone manual separation is superior to topical estrogen use in terms of potential side effects and disease recurrence.

Tedaviye dirençli ve tekrarlayan labial füzyonlu hastalarda manuel seperasyon sonrası, profilaktik topikal östrojen ve betametazon kullanımının karşılaştırılması

Amaç: Bu çalışmanın amacı daha önce tedavi edilmiş dirençli ve tekrarlayan labial füzyonlu hastalarda, manuel seperasyon sonrası profilaktik topikal östrojen ve %0,05 betametazon kullanımının yan etki ve tekrarlama açısından karşılaştırılmasıdır. 

Materyal ve Metod: Labial füzyon nedeniyle daha önce başka merkezlerde yalnızca topikal östrojen, yalnızca manuel seperasyon ile manuel seperasyon ve sonrası topikal östrojen kullanan hastalar değerlendirmeye alındı. Tüm hastalara poliklinikte %5 lidocaine pomad uygulamasını takiben manuel seperasyon uygulandı. İşlemden sonra birinci grup hastalara 3 hafta süreyle topikal östrojen, ikinci grup hastalara ise %0,05 betametazon başlandı. Her iki grupdaki hastalar olası yan etkiler ve hastalığın tekrarı açısından değerlendirildi.

Sonuçlar: Polikliniğimize Aralık 2011-Nisan 2014 tarihleri arasında labial füzyon nedeniyle 146 hasta müracaat etti. Bu hastaların 41’i (%28) ilk başvuru olduğu için çalışmaya dahil edilmedi. Birinci grup 55 hasta (%52,3) ikinci grup ise 40 hastadan (%47,7) oluşmaktaydı. Birinci grubun yaş ortalaması 19,47±16,61 (2-74 ay), ikinci grubun ise 16,25±12,28 (4-48 ay) idi. Daha önceden topikal östrojen tedavisi birinci grupta 23 (%41,8), ikinci grupta 16 (%40), manuel seperasyon birinci grupta 17 (%30,9), ikinci grupta 11 (%27,5), manuel seperasyon ve sonrasında topikal östrojen ise birinci grupta 15 (%27,2), ikinci grupta ise 13 (%32,5) hastaya uygulanmıştı. Birinci grupta hastaların %7,2’sinde (n=4), meme büyümesi  (n=2), hiperpigmentasyon (n=1) ve vaginal kanama  (n=1) saptanırken, ikinci grupta yalnızca bir hastada (%2,5) lokal irritasyonla karşılaşıldı. Birinci grupta tekrarlama oranı %23,6 (n=13) iken ikinci grupta bu oran %7,3 (n=3) olarak bulundu. 

Tartışma: Manuel seperasyon uygulanan semptomatik labial füzyonlu hastalarda, profilaktik olarak kullanılacak düşük doz betametazon olası yan etkiler ve hastalığın tekrarı açısından topikal östrojen kullanımına göre daha üstündür.
