Coexisting of Extrarenal Wilms’ tumor and Ganglioneuroblastoma in a child; and literature review

Aim: Synchronized occurrence of multiple primary cancers in children is relatively rare. There are eight case reports of concurrent Wilms' tumor and peripheral neuroblastic tumors but an association with extrarenal Wilms' tumor has not been reported yet. A case with extrarenal Wilms' tumor localized in the left inguinal canal and a ganglioneuroblastoma of left adrenal origin is discussed in terms of clinical features and treatment options by searching the literature in this study.

Case Presentation: High ligation together with mass excision was used for a 9-month-old girl with inguinal canal swelling starting three months ago and increasing for the last 15 days. The pathology report was extrarenal Wilms' tumor. Postoperative radiological examination revealed a 2.5*1.5 cm mass at the left adrenal gland. A mass corresponding to the features described with tomography and located in the middle of the adrenal gland, separate from the kidney, was found during the surgical exploration and the mass was excised together with left total surrenalectomy. The left ovary and surrounding tissues were normal. Pathology examination of the adrenal mass revealed stage I ganglioneuroblastoma. The patient was put on a chemotherapy regime for stage 1 Wilms' tumor according to NWTS.

Conclusion: Two tumors of embryonal origin in the same patient may be coincidental but we believe that the clinical features of our patient and of the other patients we found with a literature search may provide guidance in clarifying the relationship between the two tumors and the diagnosis and treatment of future patients.

Bir Çocukta Ekstrarenal Wilms’ tümörü ile Ganglionöroblasom birlikteliği ve literatür taraması

Amaç: Çocuklarda multiple primer kanserlerin senkronize ortaya çıkması oldukça nadirdir. Literatürde Wilms’ Tümörü ve periferal nöroblastik tümörlerin birlikte görüldüğü sekiz olgu bildirimi vardır, ancak ekstrarenal Wilms’ tümörü ile birliktelik henüz bildirilmemiştir. Bu çalışmada sol inguinal kanal yerleşimli ekstrarenal Wilms’ tümörü ve sol sürrenal kökenli ganglionöroblastom birlikteliği olan bir olgu literatür taraması yapılarak klinik özellikler ve tedavi seçenekleri açısından tartışılmıştır.

Olgu Sunumu: Üç ay önce kasık kanalında şişlik şikayeti başlayan ve son 15 gündür şikayetleri artan 9 aylık kız hastaya kitle eksizyonu ile birlikte yüksek ligasyon uygulandı. Kitlenin patolojik incelenmesi ekstrarenal Wilms’ tümörü olarak rapor edildi. Postoperatif dönemde yapılan radyolojik incelemesinde sol sürrenalde 2,5*1,5 cm’lik bir kitle saptandı. Yapılan eksplorasyonda, böbrekten ayrı, sürrenalin ortasında yer alan tomografide tanımlanan özelliklere uyan bir kitle saptandı ve hastaya sol total sürrenelektomi ile birlikte kitle eksizyonu uygulandı. Sol over ve çevresi normal olarak gözlendi. Sürrenal kitlenin patolojik inceleme sonucu evre I anglionöroblastom olarak değerlendirildi. Hasta NWTS’ye göre evre I Wilms’ tümörü protokolü ile kemoterapi tedavisine alındı.

Sonuç: Aynı hastada embriyonel kökenli iki tümörün görülmesi tesadüfi olabilir, ancak hem hastamıza hem de yaptığımız literatür taramasındaki diğer hastalara ait klinik özelliklerin, iki tümör arasındaki ilişkinin aydınlatılmasında ve gelecekte ki hastaların tanı ve tedavisinde yol gösterici olabileceğini düşünüyoruz.
