HAND-ASSIST LAPAROSCOPIC SPLENECTOMY: CASE REPORT

~~INTRODUCTION:
Before laparoscopic splenectomy the open splenectomy performed by means of subcostal incision had disadvantages such as staying in the hospital for a long time, big incision, and pulmonary atelectasia. Later on, when laparoscopic splenectomy is started to use, it becomes widespread and the disadvantages are reduced. However while the use of laparoscopic splenectomy in normal size spleen is advantageous, for relatively bigger spleen it can cause problems such as vessel injuries since it is hard to use the equipments. Moreover the open splenectomy is preferred because of this reason. The ouster in spleen by means of hand-assist laparoscopic splenectomy helps the surgeons to protect the organs such as stomach and intestine. There is no need for hand assist splenectomy in the routine surgery however it can be used for the big spleens and by inexperienced surgeons.
This study aims to present the case of hand assist laparoscopic splenectomy.
CASE REPORT:  It was decided by paediatric haematology that the six years old patient who was followed by paediatric haematology due to the sickle cell anaemia would have splenectomy in order to reduce the symptoms. In the examination it was seen that the patient had distention and palpable spleen. The spleen sizes of 16 cm were determined in the USG. It was decided to apply laparoscopic splenectomy to the patient. Under the general anaesthesia the patient was positioned conveniently in the supine position and the left side and the head would be up. 10 mm and 30 degrees camera port was entered in under umblicus and 5 mm 3 working ports was entered left quadrant and right upper quadrant with suitable angle. Moreover 6-7 cm handport incision was performed middle line on the belly which turns the belly from the left. After the abdomen was inflated with 12 mm- Hg CO2 the spleen mobilization was performed manually like in the open splenectomy. Splenocolic and gastrosplenic ligaments were liberalizes by means of ligasures. Splenic hilus was dissected by means of hand without harming the pancreatic tail. The artery and veni were clipped with medium endoclips. Later on splenodiafragma and splenorenal ligmens were separated by means of dissection. The spleen was ready to be taken out. Hand-port was taken out from incision. The patient stayed in the hospital for three days and then s/he was discharged without any problem.

CONCLUSİON:
Hand assist helps to the surgeons in the surgery of big spleen during the laparoscopic splenectomy. It shortens the time of the surgery. As it is compared to the open surgery, it is required to use smaller incision and get better cosmetic results, less pain. There is no need to stay in the hospital for a while and it is easy to adopt the daily life after the operation. Moreover the surgeons less experienced in laparoscopic splenectomy in the regular size spleen can be used hand assist in the beginner stage. Therefore we think that it is an advantageous method for the bigger spleen and normal spleen in the beginner level.

HAND-ASSİST LAPAROSKOPİK SPLENEKTOMİ: OLGU SUNUMU

~~GİRİŞ:
Laparoskopik splenktomiden önce subkostal insizyon ile yapılan açık splenektomi uzun süre hastanede kalış, büyük insizyon, akciğer atelektezileri gibi dezavantajları vardı. Daha sonra laparoskopik splenektominin başlaması ile birlikte bu dezavantajlar azalarak tüm dünyada yaygın olarak kullanılmaya başlamıştır. Ancak normal büyüklükte dalaklarda laparoskopik splenektomi avantajlı iken büyük dalaklarda enstrümanların kullanımın zorluğu, kapsül, ve damar yaralanmaları gibi problemler oluşabilmektedir. Hatta bu nedenlerle açık splenektomiye dönüş olabilmektedir. Hand-assist laparoskopik splenektomi ile büyük dalaklarda ekartasyon, mide ve barsak gibi organların korunması ile cerraha kolaylık sağlamaktadır. Hand assist splenektomi rutinde kullanımı gerekli değil ancak büyük dalaklarda ve deneyimi az olan cerrahların laparoskopik splenektomiye geçiş döneminde güvenle uygulanabilir.
Bu çalışmanın amacı hand-assist laparoskopik splenektomi yapılan olgumuzu sunmayı amaçladık.
OLGU SUNUMU:  Altı yaşında orak hücreli anemi nedeniyle pediatrik hematoloji tarafından takip edilen hastaya pediatrik hematoloji tarafından semptomları azaltmak için splenektomi yapılmasına karar verildi. Hastanın yapılan muayenesinde distansiyonu ve palpaple dalağı mevcuttu. Yapılan USG sinde 16 cm’lik dalak boyutları tespit edildi. Hastaya laparoskopik splenektomi kararı verildi. Genel anestezi altında supin pozisyonunda, hastanın sol tarafı ve başı daha yüksekte kalacak şekilde uygun pozisyon verildi. Göbeğin altından 30 derece 10 mm’lik kamera portu ve 5 mm lik 3 adet çalışma portları sol kadrandan ve sağ üst kadrandan uygun açı ile girildi. Ayrıca göbeği soldan dönecek şekilde göbek üstü orta hat 6-7 cm lik handport insizyon yapıldı. 12 mm- Hg CO2 ile batın şişirildikten sonra aynı açık splenektomi deki gibi dalak mobilizasyonu el yardımı ile sağlandı. Splenokolik ve gastorosplenik ligamantlar ligasure ile serbestleştirildi. Splenik hilus pankreas kuyruğuna zarar verilmeden el yardımı ile diseke edildi. Medium endoclips ile arteri ve veni klipslendi. Daha sonra splenodiafragma ve splenorenal ligamanlar diseke edilerek ayrıldı. Dalak tamamiyle serbestleştirilerek çıkarılması için hazır hale getirildi. Hand-port insizyondan dışarı alındı. 3 gün hastanede yatırılan hasta problemsiz olarak taburcu edildi.

SONUÇ:
Laparoskopik splenektomi sırasında el yardımı büyük dalakların cerrahisinde cerraha kolaylık sağlamaktadır. Cerrahi süresini kısaltmaktadır. Açık ameliyata göre daha küçük insizyon yapılarak hastada daha iyi kozmetik sonuçlar, daha az ağrı, daha kısa süre hastanede kalış ve sosyal yaşama dönüş gibi avantajlar sağlamaktadır. Ayrıca normal büyüklükteki dalaklardada laparoskopik splenektomi deneyimi az olan cerrahlarda el yardımını başlangıç aşamasında kullanabilirler. Bu yüzden büyük dalaklarda ve normal dalaklardada başlangıç aşamasında avantajlı bir yöntem olduğunu düşünmekteyiz.
