EXCISION OF MESENTERIC CYST BY MEANS OF SINGLE PORT LAPAROSCOPY

INTRODUCTION: Mesenteric cysts are rare benign masses. They are mostly observed in males. They can be asymptomatic for a while. 75% of the mature ones are until 5 years old. 60% mesenteric cysts originate from small intestine. It is hard to differentiate the omentum cysts from the pancreas cysts by means of physical examination and from other radiological cysts like duplication cysts. The treatment is open surgery or laparoscopic excision.
This study aims to present the case in which extracorporeal omentum cystes excision was performed by means of single port.
CASE REPORT
6 years old male patient consulted with the complaint of abdominal pain. As a result of USG and BT analysis, a multiseptation cystic mass which was 62X32 mm was determined in the abdomen. It was decided that there would be surgery with the pre-diagnosis of lymphangioma, omentum cyst and duplication cyst. In the surgery the belly was cut longitudinally and the intraabdominal organs in the belly were seen and 10 mm thoracic trocar was entered. The gloves were connected to trocar by means of 2/0 silk in order to prevent losing air. 5 mm trocars were connected from the two fingers of the gloves in order to prevent losing air. Later on CO2 gas 12 mm pressure was provided from the trocar in the finger of the gloves into the abdomen with 2 mm/L current. After the abdomen was full with the gas, it was entered to the abdomen by means of 5 mm telescope and dissector. In the exploration carried out, mesenteric cystic mass was determined. The cyst was minimized by aspiring by means of an injector. The cyst was hold by means of a locked hand tool and taken out from the incision enlarged 2-3 cm. The excision of mesenteric cyst was carried out extracorporeally and the intestines were sent to the abdomen. After staying in the hospital 3 days the patient was discharged. It should be followed without problem in the 5th month after the surgery.
RESULT: The excision of mesenteric cyst by means of single port is an easy and credible approach. It is convenient to determine the cysts in the abdomen which cannot be differentiated radiologically. The single port by means of gloves is cost efficient. It is thought that single port is an advantageous method since it has good cosmetic results, it is required to stay in the hospital for a short time and it is easy to adopt the daily life after surgery.

 

SİNGLE PORT LAPAROSKOPİ YARDIMI İLE MEZENTER KİSTİ EKSİZYONU

GİRİŞ: Mezenter kistler nadir görülen bening kitlelerdir. Daha çok erkeklerde görülür. Uzun süre asemptomatik kalabilir.  Olguların % 75 i 5 yaşına kadar semptom vermektedir. Mezenter kistlerin % 60’ı ince barsak kaynaklıdır. Omentum kistlerinin hem fizik muayene ile hemde radyolojik olarak pankreas kistlerinden, dublikasyon kistleri gibi diğer radyolojik kistlerden ayırt edilmesi güçtür. Tedavisinde açık cerrahi veya laparoskopik olarak eksizyon  yapılmaktadır.
Bu çalışmada single port yardımı ile ekstrakorporal omentum kisti eksizyonu yapılan  olgumuzu sunmayı amaçladık.
OLGU SUNUMU
6 yaşında erkek hasta karın ağrısı şikayeti ile başvurdu. Yapılan USG ve BT incelemede batında 62X32 mm multiseptasyonlu kistik kitle tespit edildi. Lenfanjiom, omentum kisti ve dublikasyon kisti ön tanıları ile ameliyata karar verildi. Ameliyatta göbek longitudinal olarak kesilerek göbeğin içinden batın içi organlar görüldü ve içerisinden 10 mm’lik toraks trokarı girildi. Eldiven trokara bağlanarak hava kaçırmaması için 2/0 ipek ile trokara bağlandı. Eldivenin iki parmağından 5 mm’lik trokarlar girilerek hava kaçırmaması için bağlandı. Daha sonra eldiven parmağının içindeki trokardan CO2 gazı 12 mm basınç, 2 mm/L akım ile batın içine verildi. Batın yeterince gaz ile doldurulduktan sonra 5 mm lik teleskop ve disektör trokarın içerisinden birlikte batına girildi. Yapılan eksplorasyonda mezenter kaynaklı kistik kitle tespit edildi. Kist dışarıdan girilen bir enjektör ile aspire edilerek küçültüldü. Kist kilitli el aleti ile tutuldu ve 2-3 cm büyütülen insizyondan dışarı alındı. Ekstrakorporal olarak mezenter kisti eksizyonu yapılarak barsaklar tekrar batına gönderildi. 3 gün hastanede yatırılan hasta taburcu edildi. Ameliyat sonrası 5. ayında problemsiz takip edilmedir.
SONUÇ: Single port yardımlı mezenter kist eksizyonu kolay ve güvenilir bir yaklaşımdır. Batında radyolojik olarak ayırt edilemeyen kistik kitlelerin ayırıcı tanısında yararlıdır. Eldiven yardımıyla single port düşük maliyeti, iyi kozmetik sonuçları, kısa süre hastanede yatış ve kısa sürede sosyal yaşama dönüş ile single portun avantajlı bir yöntem olduğunu düşünmekteyiz.
