THE RELATIONSHIP BETWEEN LEPTIN AND INFLAMMATORY RESPONSE IN CHILDREN WITH ACUTE APPENDICITIS

~~Introduction and Aim: The most common laboratory tests for the diagnosis of acute appendicitis (AA) are white blood cell count (WBCC) and measurement of serum C reactive protein (CRP) levels. To decrease the ratio of negative appendectomy and perforations, additional diagnostic tests are being developed. In this study it was aimed to determine both the relationship between the inflammation at AA and the leptin levels and the role of leptin at anorexia, which is the most common symptom.

Patients and Method: This study was carried out from January 2006 to February 2007. The serum leptin levels, WBCC and CRP levels of 40 patients (Group I) who were dignosed with AA macroscopically during the operation and of 20 children (Group II) who did not have an inflammatory disease in their background and at present were investigated. 27 of the patients in Group I were boys and 13 of them were girls. The average age of the patients in Group I was 7.3 with an age range of 2–10 years. 15 of the children in Group II were boys and 5 of the them were girls. The average age of the children in Group II was 5.6 with an age range of    2–10 years. Serum leptin levels, WBCC and CRP levels of these children were measured preoperatively, perioperatively and at 48 h after the start of the operation. For the examination of the data Kolmogrov-Smirnov, Student-t test and Mann-Whitney U test were used.

Results: It was found out that 26 of the patients (%65) were diagnosed with perforated AA and 14 of the patients (%35) were diagnosed with non-perforated AA. There was no significant difference in the WBCC between the patients with the perforated appendicitis and the non-perforated AA. The CRP levels of the patients were higher than the control group. The CRP levels were higher in patients with perforated AA than those patients with non-perforated AA. The difference between preoperative and perioperative leptin levels of the groups was not statistically significant. But, leptin levels at 48 h after the start of the operation were lower than the Group II and the difference was statistically significant. Leptin levels of the patients with non-perforated appendicitis were higher than those of the patients with perforated appendicitis and the difference was statistically significant. There was positive correlation between the body mass index and the leptin levels in both groups. 
Conclusions: As a result of the study, it can be concluded that the WBCC and measurement of serum CRP are useful in the diagnosis of AA. While the WBCC is not able to determine the perforation, CRP is an effective parameter in determining the perforation. On the other hand, it can be concluded that since leptin, which has an effect on anorexia, increased during the acute inflammatory period of the disease, it has an important role during inflammatory response. 

AKUT APANDİSİTLİ ÇOCUKLARDA İNFLAMATUVAR YANIT LEPTİN İLİŞKİSİ

~~Giriş ve Amaç: Akut apandisit tanısı için en sık kullanılan laboratuvar testleri, kan lökosit sayımı (BK) ve serum C-reaktif protein (CRP) düzeyi ölçümüdür. Negatif apandektomi ve perforasyon oranlarını düşürmek için yardımcı tanı yöntemleri geliştirilmektedir. Bu çalışmada akut apandisitteki inflamasyon ile leptin seviyeleri arasındaki ilişki ve en sık görülen semptom olan iştahsızlığın mekanizmasında leptinin rolünün belirlenmesi amaçlanmıştır.

Hastalar ve Yöntem: Ocak 2006-Şubat 2007 tarihleri arasında akut apandisit düşünülen ve operasyon sırasında makroskobik olarak akut apandisit tanısı konulan 40 hasta (Grup I) ile özgeçmişinde ve çalışmaya alındığı sırada inflamatuvar hastalığı olmayan 20 çocuğun     (Grup II) serum leptin seviyeleri, BK sayıları ve CRP düzeyleri ölçüldü. Grup I ‘deki hastaların yirmi yedisi erkek, onüçü kız, yaş ortalamaları 7.3 yaş (2–10) idi. Grup II’deki çocukların onbeşi erkek, beşi kız, yaş ortalamaları 5.6 yaş (2–10)  idi. Ölçümler preoperatif, peroperatif ve postoperatif 2. gün yapıldı. Verilerin değerlendirilmesinde Kolmogrov-Smirnov, Student-t testi ve Mann-Whitney U testi kullanıldı.

Bulgular:  Hastaların 26’sında (% 65) perfore apandisit, 14’ünde (% 35) perfore olmamış akut apandisit tespit edildi. Perfore apandisiti olan ve olmayan hastaların BK değerleri arasında anlamlı fark bulunamadı. Hastaların CRP değerleri kontrol grubuna göre yüksek saptandı. Perfore apandisiti olan hastaların CRP değerleri, perfore olmayanlara göre yüksekti. İki grup arasındaki preoperatif ve peroperatif leptin değerleri istatistiksel olarak anlamlı değildi. Hastaların postoperatif 2.gün leptin değerleri kontrol grubuna göre daha düşüktü ve sonuçlar istatistiksel olarak anlamlı bulundu. Perfore apandisiti olmayan hastaların leptin değerleri, perfore olanlara göre daha yüksekti ve sonuçlar istatistiksel olarak anlamlı idi. Vücut kitle indeksi ile leptin arasında hem hasta hem de kontrol grubunda pozitif yönde korelasyon saptandı.
 
Sonuç: Akut apandisit tanısında BK ve CRP yararlı iken, perforasyona gidişi göstermede BK etkisiz, CRP etkili bir parametre olarak bulundu. İştahsızlık üzerinde etkili olduğu bilinen ve hastalığın akut döneminde arttığı görülen leptinin inflamatuvar cevap sırasında önemli rol oynadığı düşünüldü.
