Ultrasound guided hydrostatic reduction of intussusception with saline: safe and effective.

Aim: The study was undertaken to assess the efficacy of ultrasound guided saline enema in reducing intussusception and to determine the role of age and duration of symptoms on this event.

Methods: The case records of patients treated for intussusception at our institutions over the past 10 years were retrospectively analyzed. A total of 419 patients were treated for intussusception, and 375 of them were included into the study. Excluded  cases  included the  patients who required surgery as an initial treatment due to the symptoms and signs of acute abdominal disease.

Results: Hydrostatic reduction was succesful in 313 out of 375 cases (83.46 %).  No complication associated with the procedure was observed. There were 29 episodes of recurrences in 23 patients, and recurrence rates were not found different between patients who responded hydrostatic reduction and those required surgery. The younger age [median (range) 11.00 mo (3.00-108.0) vs 20.00 mo (1.00-180.0); p<0.05] and long duration of symptoms [mean ± SD; 1.95 ± 1.51 days vs 1.44 ± 1.02 days; p<0.001) were significantly associated with the failed hydrostatic reduction.

Conclusion:  Ultrasound guided hydrostatic reduction is an easy, safe and effective method for the treatment of intussusception in the absence of acute abdominal findings.

İnvaginasyonun ultrasonografi destekli hidrostatik redüksiyonu: güvenilir ve etkili bir yöntem

Amaç: Çalışma intussusceptiona azaltmada ultrason eşliğinde serum fizyolojik lavman etkinliğini değerlendirmek ve bu olay yaş ve semptomların süresi rolünü belirlemek amacıyla yapıldı.
Yöntem: Son 10 yılda  invajinasyon nedeniyle tedavi edilen hastaların  kayıtları geriye dönük olarak incelendi. İnvajinasyon tedavi edilen 419 hastanın 375 ‘i çalışmaya dahil edildi

Sonuçlar: Hidrostatik redüksiyon  375 olgunun  313’inde  (% 83.46) başarılı oldu. Prosedür ile ilgili herhangi bir komplikasyon gözlenmemiştir. Orada 23 hastada  29 kez nüks gözlendi.  Redüksiyon uygulananlarla cerrahiye gidenler arasında nüks oranında herhangi bir fark gözlenmedi. Daha genç yaş [ortanca (aralık) 11.00 mo (3,00-108,0) vs 20.00 mo (1,00-180,0); p <0.05] ve [ortalama ± SD belirtilerin uzun süresi; 1.95 ± 1.51 gün vs 1.44 ± 1.02 gün; p <0.001) belirtilerde ki sürenin uzunluğu redüksiyon başarısını anlamlı olarak düşürdüğü bulunmuştur.

Değerlendirme:Ultrason eşliğinde hidrostatik redüksiyon akut karın bulguları yokluğunda rahatça uygulanabilen, güvenli ve etkili bir yöntemdir.
