Chromosomal anomalies in newborns with gastrointestinal atresias: place of the genetic investigation

Background and Aim: Gastrointestinal atresias may be associated with several chromosomal abnormalities. Especially, association with Down syndrome in duodenal atresia and esophageal atresia cases is known for a long time. In this study, we aimed to determine accompanying chromosomal abnormalities in cases operated for gastrointestinal atresia at our department.

Material and Method: Clinical records of the gastrointestinal atresia cases operated at our institution between January 2004 and January 2014, reviewed retrospectively. Types of gastrointestinal atresia, morphologic sings and associated anomalies and results of the genetic evaluation were analyzed.

Results: During the 10-year period, 351 cases were operated for gastrointestinal atresia which that esophageal atresia, duodenal atresia/web, jejuno-ileal atresia, colonic atresia, and anal atresia. Morphological markers of genetic abnormalities were detected in 12 cases. Chromosomal abnormalities were Trisomy 21 in 10 cases and Trisomy 18 in 2 cases. In cases with Trisomy 21, 6 cases had duodenal atresia (6/47), 3 had EA + TEF (3/115), and 1 had anal atresia (1/143). In cases with Trisomy 18, 1 had anal atresia (1/143), 1 had EA + TEF (1/115).

Conclusion: In our study, trisomy 21 was frequent with duodenal atresia which is consistent with the literature. Trisomy 21 can also associates esophageal and/or anal atresia. In cases with gastrointestinal atresia associated with morphologic stigmata suggesting chromosomal anomalies, genetic evaluation yields more predictive and prognostic information.

Gastrointestinal atrezili yenidoğanlarda kromozomal anomaliler; genetik incelemenin yeri

Giriş ve Amaç: Gastrointestinal atreziler çeşitli kromozom anomalileri ile beraber görülebilirler. Özellikle 21 Trizomi’nin duedonal atrezisi, özefageal atrezi veya anal atrezi ile beraberliği; ve Turner Sendromu’nun ise ileal atrezilere eşlik edebileceği bilinmektedir. Bu çalışmada, kliniğimizde opere edilen gastrointestinal atrezili olgulara eşlik eden kromozomal anomalilerin ortaya konulması amaçlanılmıştır.

Materyal ve Metod: Ocak 2004 ve Ocak 2014 yılları arasında, kliniğimizde opere edilen intestinal atrezi olgularının klinik kayıtları geriye dönük olarak değerlendirildi. Olguların atrezi tipleri, eşlik eden morfolojik işaretler ve genetik inceleme sonuçları değerlendirildi.

Bulgular: 10 yıllık süre içerisinde 351 olgu özefagus atrezisi, duedonum atrezisi/web, jejunoileal atrezi, kolonik atrezi, anal atrezi ön tanıları ile opere edildi. Olgulardan morfolojik stigmatları olan 12 olguda yapılan genetik çalışma sonucunda 10 olguda Trizomi 21, 2 olguda Trizomi 18 saptandı. Trizomi 21 saptanan 10 olgudan 6’sı duedonum atrezisi (6/47), 3’ü ÖA+TÖF (3/115), ve 1’i anal atrezi (1/143), Trizomi 18 olgularından 1’i anal atrezi (1/143), 1’i ÖA+TÖF (1/115) sebebiyle opere edilmişti.

Tartışma: Çalışmamızda 21 Trizomi, literatür ile uyumlu olarak en sıklıkla duedonal atrezilerde görülmekle beraber, özofagus ve anal atrezi gibi diğer gastrointestinal atrezilerde de görülebilmektedir. Gastrointestinal atrezili olgularda kromozomal anomali yönünden şüpheli morfolojik işaretler varlığında hastaların genetik analizlerinin yapılması daha belirleyici ve prognostik bilgiler verir.
