ARE BIODEGRADABLE STENTS THE ULTIMATE SOLUTION FOR CAUSTIC ESOPHAGEAL INJURY?

INTRODUCTION

Complications in children caused by caustic drinking are actually public health problems but also an indispensable part of pediatric surgery in Turkey. Balloon or bougie dilatation is frequently used in esophageal strictures. Stent application experience on cases not responding to dilatation is limited.  

CASE STUDY

3 years old female patient applied with oil solvent drinking complaint was diagnosed with flexible endoscopy and phase 3 caustic esophageal damage. Gastrostomy was opened. She was taken under balloon dilatation program due to advancing stricture. Medical treatment was started for gastroesophageal reflux detected during follow up. It was decided to place esophageal stent due to heavy stricture findings not allowing endoscopy penetration after two years and 14 dilatations. Biodegradable (ELLA-Biodegradable Esophageal Stent BD, Czech Republic) stent was placed on   esophageal 12 and 18 cm stricture area through endoscopy. Recession was observed in the findings in the early period but it was seen in the endoscopy applied 3 months later that proximal of stent and stricture in distal. Total 5-balloon dilatation was applied on both ends. One mitomycin c was applied. Due to non-healing, esophageal transposition was made 4 years after caustic drinking.

DISCUSSION

Usage of esophageal stent in adult patients particularly following cancer surgery is gradually increasing. Most common disease in children that would cause stent indication in children is strictures developing due to caustic drinking. These naturally advance different than strictures seen in adult patients. In addition to dysphagia causes by stents non-absorbed due to long response of tissue against burn, they cause problem during defecation as they are embedded to the tissue. However, it may be thought at least degradable stents are theoretically a more effective treatment. But as seen in the case presented, stricture areas that were not present at the beginning developed on lower and upper ends of stent in the long term due to continuing tissue response. These have independently caused dilatation indications and extension of stricture line. Although there is only one case, there is no proof showing that biodegradable stents are beneficial in long-term treatment of caustic esophageal damage.

KOSTİK ÖZOFAGUS HASARI SAĞALTIMINDA BİYO-ÇÖZÜNÜR STENT BİR ÇÖZÜM MÜDÜR?

GİRİŞ

Çocuklarda kostik madde içimi ve buna bağlı gelişen komplikasyonlar, esasen bir halk sağlığı sorunu olmakla birlikte, Türkiye çocuk cerrahisi pratiğinin ayrılmaz bir parçası haline gelmiştir. Özofagus striktürlerinde sıklıkla balon veya buji ile dilatasyon uygulanır. Dilatasyona yanıtsız olgularda stent uygulanmasıyla ilgili deneyim kısıtlıdır.

OLGU SUNUMU

Yağ çözücü içimi yakınmasıyla başvuran 3 yaşında kız hastada bükülebilir endoskopi ile evre 3 kostik Özofagus hasarı saptandı. Gastrostomi açıldı. Gelişen striktür nedeniyle balon dilatasyon programına alındı. Takipte saptanan gastroözofageal reflüye yönelik medikal tedavi başlandı. İki yıl ve 14 dilatasyonun ardından, endoskopun geçişine izin vermeyen ağır striktür bulguları nedeniyle özofageal stent yerleştirilmesine karar verildi. Özofagus 12 ile 18. cm’ler arasındaki striktür alanına, endoskopi yardımlı, biyo-çözünür (ELLA-Biodegradable Esophageal Stent BD, Czech Republic) stent yerleştirildi.  Erken dönemde bulgularda gerileme görülmekle beraber, 3 ay sonra yapılan endoskopide stentin proksimali ve distalinde striktürün tekrarladığı görüldü. Her iki uca toplamda 5 kez balon dilatasyonu uygulandı. Bir kez mitomisin c uygulaması yapıldı. İyilşeme sağlanamaması nedeniyle, kostik madde içiminden 4 yıl sonra özofagokolonik transpozisyon yapıldı.

TARTIŞMA

Erişkin hastalarda özellikle kanser cerrahisi sonrasında, özofageal stent kullanımı giderek artan bir ivme göstermektedir. Çocuklarda stent endikasyonu oluşturabilecek en sık görülen hastalık kostik madde alımına bağlı gelişen striktürlerdir. Bunlar, doğaları gereği erişkin hastalarda gelişen striktürlerden farklı seyir gösterirler. Yanığa karşı doku yanıtının çok uzun süre devam etmesi nedeniyle emilemeyen stentlerin, yol açtıkları yutma güçlüğüne ek olarak, özellikle çıkartılmaları aşamasında dokuya gömülmeleri nedeniyle sorun yaratabildiği bilinir. Buna karşın, en azından teorik olarak, çözünebilir stentlerin daha etkin bir tedavi yöntemi olduğu düşünülebilir. Ancak burada sunulan olguda görüldüğü üzere, devam eden doku yanıtı nedeniyle, uzun dönemde, stentin alt ve üst ucunda, başlangıçta olmayan striktür alanları gelişmiştir. Bunlar kendi başlarına dilatasyon endikasyonu oluşturduğu gibi, striktür hattının uzamasına da yol açmıştır. Tek olgu olmasına karşın, biyo-çözünür stentlerin kostik özofagus hasarının uzun dönem sağaltımında faydalı olacağına dair bir kanıt elde edilememiştir.
