Presentation of a case who applied with a mass on thoracic wall consistent with malignancy

Introduction

Granulomatous mass in soft tissue  may be induced by infection or tumour but also rarely can be induced by extrapulmonery tuberculosis (TB). A child with mass in the anterior chest wall is presented.


Case report

2 year-old girl with mass in the thorax wall, which noticed six months before is admitted. In the physical examination in the intersection of the sternum and approximately 2 cm size, rigid and fixed, uniformly bounded mass was detected.  The chest radiograph was unremarkable.  In the ultrasonography,in the left costochondral junction 28x13 mm,  a lesion belongs to the chondral components and causing contour irregularities was detected. In magnetic resonance imaging in the intermuscular area, a mass that’s central had necrotic-cystic character. In the Computed tomography which performed for evaluation of bone structure,  the rear contour of the mass shows wide intimacy with 3. costa and irregularity in costa contour and erosive changes was observed. Tuberculin Skin Test and plasma quantiferon resulted negative.  In the pre-diagnosis, malignant lesions such as histiocytosis X and sarcoma which goes with bone involvement was considered. Complete excision of the mass was decided and with rib if necessary. During the operation, a mass with 2 cm in diameter  located in the pectoral muscle, in the conjoined part of 3. rib and  sternum, attached to 3. rib and which  made a minimal consequent destruction. A white-yellow pasty content came within the mass .Culture and cytology samples were taken and were fully excised. Pathological evaluation revealed a benign  granulomatous mass showing caseification.


Results
The case report complaint presented here, with physical examination findings and in the light of advanced evaluation methods, operated with a preliminary diagnosis of malignant bone tumor and performing a radical surgery is foreseen  according to the degree of bone involvement.  However, intraoperative findings, rib resection was not performed. Indeed, the pathological diagnosis has supported nonspecific granulomatous inflammation.  Although granulomatous mass which shows caseification primary suggested TB , in this patient etiology has not been elucidated.

Toraks duvarında malignite ile uyumlu kitle ile başvuran bir olgunu sunumu

Giriş

Yumuşak dokuda granülomatöz kitle, enfeksiyon veya tümör kaynaklı olabileceği gibi, nadiren ekstrapulmnoner tüberkülozu (TBC) da gösterebilir. Bu sunumda toraks ön duvarında kitleyle başvuran bir çocuk sunuldu.

Olgu sunumu

2 yaşında kız hasta toraks duvarında 6 ay once farkedilen kitleyle başvurdu.Fizikbakıdatoraksduvarı sol 3.kostaile sternum kesişiminde, yaklaşık 2 cm. çapında, sert ve fikse, düzgünsınırılı kitle saptandı. Akciğer grafisinde özellik saptanmadı. Ultrasonografide sol kostokondral bileşkede 28x13 mm, kondral komponente ait olan ve kontur düzensizliğine neden olan lezyon saptandı.Manyetik rezonans görüntülemede intermusküler alanda, santrali nekrotik-kistik karakterli kitle görüldü. Kemik yapının değerlendirmesi için çekilen bilgisayarlı tomografide kitle arka kontürünün3. kostaya geniş komşuluk gösterdiği ve kostakontüründedüzensizlik, eroziv değişimler olduğu görüldü. Tüberkülin Deri Testi  ve plazma quantiferon negatif sonuçlandı.Ön tanıda kemik tutulumu ile giden histiyositozis x ve sarkom gibi malign lezyonlar düşünüldü. Kitlenin gereğinde kotla beraber tam eksizyonuna karar verildi. Ameliyatta pektoral kas içinde yerleşimli, sol 3. kot ile sternum birleşiminde 3. kota yapışık, kemikte minimal destrüksüyon yapmış 2 cm çapındaki kitleye ulaşıldı. Kitle içinden beyaz-sarı renkte macun kıvamında içerik geldi. Kültür ve sitoloji örnekleri alındı ve tam eksize edildi. Patolojik değerlendirmede kazeifikasyon gösteren selim granülomatöz kitle saptandı.

Sonuç

Burada sunulan olgu başvuru yakınması, fizik muayene bulguları ve ileri değerlendirme yöntemleri ışığında kemik kaynaklı malign tümör ön tanısı ile ameliyata alınmış ve tutulumun derecesine göre radikal cerrahi yapılması öngörülmüştür. Bununla birlikte ameliyat bulguları eşliğinde kot rezeksiyonu yapılmamıştır. Nitekim patolojik tanı nonspesifik granülomatöz inflamasyonu desteklemiştir. Kazeifikasyon gösteren granülomatöz kitle öncelikle TBC düşündürse de bu hastada primer etyoloji aydınlatılamamıştır.
