Aspiration of a an interesting foreign body: Myiasis

Aim: Herein we presented  the diagnosis and treatment of a child in whom myiasis aspiration with larvae proliferation at respiratory epithelium was detected. We were unable to find such a case presented in English literature.
Case:  An eight-month- old girl admitted to emergency department with sudden onset of cough and wheezeing . In medical history, a myiasis was seen by the mother in her hand.  There were nasal discharge and coarse cracles in physical examination. Hemoglobin was 11.1gr/dL , leucocytes were 12.900/mm3 on complete blood count. Total IgE was 0.6 IU/mL. Chest X-ray was normal. Bronchoscopy was performed with probable diagnosis of foreign body aspiration of myiasis. There were mucoid secretion and alive and active larvae at left lower bronchi. The bronchi was washed with normal saline and aspirated. Sample from aspiration material was obtained for microbiological evaluation in which larvae were seen. Albendazol was started. Every third day, bronchoscopy was performed and the bronchi was washed with saline and aspirated. The purpose of repeating bronchoscopy was clearence and reduction of larvae count. In the third operation, the amount of larvae was significantly reduced. Microbiological investigation showed the larvae which were belonging to myiasis species. In the fourth operation, there was only secretion and the larvae were all cleared from the trachea and bronchi. The patient was scheduled to routine follow-up for any other respiratory complication.
Conclusions: The aspiration of myiasis and proliferation of larvae at the brochi and trachea is a very rare type of foreign body aspiration. The purpose of treatment through repeated bronchoscopy, saline wash and aspiration was the clearence of larvae without any damage to respiratory epithelium and this management was successful in this subject.
 

İlginç bir olgu: Tatarcık aspirasyonu

Amaç: Küçük karasinek (tatarcık) aspirasyonu sonrası solunum yollarında larva gelişimi ile seyreden, litaretürde bir örneğini tespit edemediğimiz olgunun tanı ve tedavi sürecini sunduk.
Olgu: 8 aylık kız hastanın annesi, çocukta ani başlayan öksürük sonrası elinde küçük karasinek görmüş. Öksürük ve hırıltılı solunum şikayeti ile hastanemiz aciline başvurdu. Fizik muayenesinde nazal akıntı ve dinlemek ile kaba raller tespit edildi. Tam kan sayımında hemoglobin:11.1gr/dL, lökosit:12.9 K/mm3, total IgE:0.6 IU/mL tespit edildi. Akciğer grafisi normaldi. Öksürük şikayeti devam eden hastaya yabancı cisim – sinek aspirasyonu ön tanısı ile yapılan bronkoskopide sol alt bronşta mukoid sekresyon, trakeada çok sayıda hareketli larva tespit edildi. Serum fizyolojik ile bol yıkama ve aspirasyon yapıldı. Aspirasyon materyalinden örnek alındı.  Materyaller mikrobiyoloji tarafından değerlendirildiğinde canlı larva tespit edildi. Yapılan incelemede miyaz(tatarcık) larvası olduğu görüldü. Albendazol başlandı. Olguya 3 gün ara ile kontrol bronkoskopi; toplam 4 kez bronkoskopi yapıldı. Her işlemde serum fizyolojik ile yıkama, aspirasyonlar yapılarak larva sayını azaltmak ve bronşların temizlenmesi amaçlandı. 3. bronkoskopide canlı larvaların oldukça azalmış olduğu görüldü. 4. bronkoskopide trakea ve bronşlarda sekresyon olduğu, canlı larvanın temizlenmiş olduğu görüldü. Olgu takibe alındı.
Sonuç: Miyaz (tatarcık) aspirasyonu sonrası trakea ve bronşlarda larva görülmesi çok nadir görülen bir olaydır. Olgumuzun tedavisinde kısa aralıklarla bronkoskopi yapılarak bronş mukozasına zarar vermeden serum fizyolojik ile yıkama ve aspirasyon ile larva sayısının azaltılarak temizlenmesi etkili olmuştur.
