A Very Rare Cause of Lower Gastro Intestinal (GI) Bleeding

Introduction: Meckel’s diverticulum, intestinal duplication, polips, hemorrhoids and fissures are frequent causes of lower GI bleedings in infants and children. Meckel’s diverticulum as the most frequent may consist ectopic mucosa like gastric, pancreatic, colonic, jejunal, duodenal tissue up to 34-95% apart from ileum . Sometimes gastric mucosa localized in ileum or jejunum without any diverticular component or duplication may cause massive bleeding.
Case report: A twelve years old boy with a 2 months history of massive rectal bleeding without any abdominal pain and blood transfusion for two times to correct 7,1 gr/dl hemoglobin level  was admitted to the emergency unite of our hospital. Physical examination was normal apart from minimal anal fissure. Antigliadine anticore for Celiac disease, tissue transglutaminases, antigens for adeno virus, rota virus, entamoeba cyst was studied to highlight bleeding etiology. All were negative.  Scintigraphic scan with Tc99m for Meckel’s diverticulum was positive. The patient was referred to our clinic for surgery. There was not any diverticular structure in laparoscopic exploration. Laparotomy was performed. A twenty cm length of intestinal wall thickening mimicking intestinal duplication 65 cm proximal to the ileocecal valve was detected. Resection of this segment was performed and the ends were anastomosed in two layers. Preoperatively the resected segment was longitudinally cut over the antimesenteric wall and no duplication was seen. The lumen was covered with gastric mucosa having multiple hemorrhagic foci. The histopathologic evaluation was ectopic gastric mucosa. Postoperative follow-up was uneventful.
Discussion: Meckel’s diverticulum is the most common cause of painless rectal bleeding in children. Duplication with ectopic gastric mucosa is the second. A very rare entity with gastric mucosa localized in ileum without duplication formation may be the cause of massive painless rectal bleeding.  Resection of the effected segment is curative in such cases.

Nadir Bir Alt Gastro İntestinal Sistem Kanama Nedeni

Giriş: İnfantlarda ve daha büyük çocuklarda alt gastrointestinal kanama sebepleri meckel divertikülü, intestinal duplikasyon, polipler, hemoroid ve anal fissürlerdir. En sık kanama sebebi olan Meckel divertikülünün iç yüzünde ileal mukoza yanında %34- 95 oranında mide, pankreas, kolon, jejunum veya duodenum mukozasında ait ektopik mukoza yer alır.  Bazen herhangi bir duplikasyon ya da divertikül olmaksızın ileal ya da jejunal yerleşimli gastrik mukoza ağrısız abondan kanama sebebi olabilir.
 Olgu sunumu: 12 yaşında erkek olgu 2 ay önce karın ağrısı olmaksızın abondan rektal kanama ve hemoglobin değerinin 7.1 gram/dl olması üzerine 2 defa eritrosit süspansiyonu transfüzyonu yapılma öyküsü ile acil servise başvurdu. Fizik muayenede minimal anal fissür dışında patoloji saptanmadı. Kanama nedenleri açısından çölyak hastalıgı için antigliadin antikor, doku transglutaminazları, parazitolojik olarak adenovirus rota virus entemobea kist antijenleri bakıldı. Negatif saptandı. Meckel divertikülüne yönelik yapılan sintigrafide Tc99m tutulumu mevcuttu.  Olgu operasyon amacıyla cerrahi servisine nakil alındı. Laparoskopik eksplorasyonda divertiküler herhangi bir yapı saptanmadı. Laparotomide ileoçekal valvden 65 cm proksimalde 20 cm lik bir ileal ansda duplikasyon izlemini veren yapı saptanıp rezeksiyon anastomoz yapıldı. Peroperatif incelemede çıkarılan barsak ansı antimezenterik duvardan longitüdinal açıldı. Duplikasyon saptanmadı ancak bu ansda tüm mukozayı çepçevre kaplayan mide mukozası saptandı. Patoloji sonucu ektopik mide mukozası olarak yorumlandı.  Postoperatif izlem sorunsuz geçti. 
Sonuç: Çocukluklarda ağrısız rektal kanamanın en sık sebebi meckel diveritkülüdür. Bunun yanında ektopik gastrik mukozalı duplikasyonlar ikinci sırada yer alır. Çok nadir görülen başka bir form olan duplikasyon olmaksızın ileal yerleşimli gastrik mukoza abondan kanama sebebi olabilir. Tedavide rezeksiyon anastomoz ilk seçenektir.
