A CASE OF CUPPING MADE HYPERTROPHIC PYLORIC STENOSIS WITH SEVERE JAUNDICE

Introduction/Aim: Unconjugated hyperbilirubinemia occurs in 2-5% of cases with hypertrophic pyloric stenosis and sometimes bilirubin levels can reach up to 10-20 mg/dL. In this study, a patient was presented who had been treated with “cupping”, a traditional method of alternative medicine.

Case Report: 55-day old male patient with 27 mg/dL bilirubin levels was hospitalized in Clinic of Pediatrics with initial diagnosis of Criggler-Najjar Syndrome and sepsis. It was observed that patient treated with cupping on epigastric region and the back projection by opened tiny wounds. The patient was given phototherapy meanwhile spot urine, urine organic acid screening, metabolic screening, direct coombs and liver serology tests did not result in any disease to explain. Because of vomiting history for a week and pyloric muscle thickness of 4 mm, the channel length of 20 mm measured in the US, the patient was consulted to the Department of Pediatric Surgery. Patient was hospitalized with prediagnosis of hypertrophic pyloric stenosis as blood gas pH 7.69. Severe fluid-electrolyte replacement and simultaneously phototherapy was performed. On the fifth day of treatment, total bilirubin level fell below 17 mg/dL. The patient was operated when blood gas pH 7.42 on the seventh day. The patient who underwent open surgery (Ramstedt Pyloromyotomy) was discharged uneventfully on the fourth postoperative day with 1.25 mg/dL bilirubin level. Patient had no feeding problem and his bilirubin level was normal in the control of the first week and first month.

Conclusion: Severe jaundice that bilirubin values rise up to 27 mg/dL with hypertrophic pyloric stenosis is uncommon. It is thought provoking, that an alternative medicine (cupping) is preferred rather than modern medicine and the possibilities, by a college graduated parent with high level of knowledge and culture.

HACAMAT YAPILAN AĞIR SARILIKLI BİR OLGUDA HİPERTROFİK PİLOR STENOZU

Giriş/Amaç: Hipertrofik pilor stenozunda, indirekt hiperbilirubinemi %2-5 olguda görülür ve bazen bilirubin düzeyleri 10-20 mg/dl değerlerine kadar ulaşabilir. Bu çalışmada, ağır sarılık nedeniyle geleneksel bir alternatif tıp yöntemi olan “hacamat” tedavisi yaptırılmış hipertrofik pilor stenozlu bir hasta sunuldu.

Olgu Sunumu: 27 mg/dl bilirubin değerine sahip 55 günlük erkek hasta, Crigler-Najjar Sendromu ve sepsis öntanıları ile Çocuk Hastalıkları Kliniği’nde interne edildi. Hastaya, epigastrik bölgeden ve sırttaki izdüşüm bölgesinden keskin bir aletle minik yaralar açılarak “hacamat” yapılmış olduğu gözlendi. Fototerapi başlanan hastanın spot idrar, idrarda organik asit taraması, metabolik tarama, direkt coombs ve karaciğer serolojisi testlerinden hiçbiri hastalığını açıklayacak şekilde sonuçlanmadı. Öyküsünde bir haftadır kusma yakınması olması ve US’de pilor kas kalınlığının 4 mm, kanal uzunluğunun 20 mm ölçülmesi nedeniyle hasta Çocuk Cerrahisi kliniğine konsülte edildi. Kan gazında pH:7.69 olan hasta hipertrofik pilor stenozu öntanısı ile kliniğimizde interne edilerek, yoğun sıvı elektrolit replasmanı yanısıra eş zamanlı fototerapi yapıldı. Tedavisinin beşinci gününde total bilirubin düzeyleri 17’nin altına düştü, yedinci gününde kan gazı pH’sı 7.42 olduğunda opere edildi. Açık cerrahi yöntemle “Ramstedt Piloromiyotomi” yapılan hasta, ameliyat sonrası 4. gün bilirubin değeri 1.25 mg/dl olarak şifa ile taburcu edildi. Hastanın 1. hafta ve 1. ay kontrolünde beslenme sorununun olmadığı, bilirubin değerlerinin normal seviyelerde olduğu görüldü.

Sonuç: Hipertrofik pilor stenozunda bilirubin değerlerinin 27 mg/dl’ye kadar yükseldiği ağır sarılık tablosu pek nadirdir. Günümüzde modern tıp ve olanakları yerine, geleneksel bir alternatif tıp yöntemi olan “hacamat” tekniğinin, üniversite mezunu, yüksek bilgi ve kültür düzeyine sahip bir anne baba tarafından öncelikli olarak halen tercih ediliyor olması düşündürücüdür.
