Inguinal herni repair with conventional single port by using intracorporeal knotting technique

Purpose: As the interest in minimal invasive surgery has turned to single site access surgery, single incision laparoscopic surgery is becoming popular. It is possible to perform single port laparoscopic inguinal hernia repair using a single conventional port and conventional working instruments without extracorporeal intervention. We herein present our experience with inguinal herni repair conducted with conventional single port-single instrument incisionless endoscopic surgery.

Material and Methods: During November 2013-February 2015, 64 patients underwent inguinal herni repair by conventional single port-single instrument. A 10 mm scope with a parallel eye piece and an integrated 6 mm working channel is inserted through an 11 mm “conventional umbilical port”. Conventional needle holder was introduced through the integrated working channel. Internal inguinal ring were closed by intracorporeal circular purse suturing technique with 2/0 polyester suture (needle; 16 mm, ½ curve) (Ti-Cron, Covidien, USA).

Results: A total 68 inguinal hernia (36 right, 26 left and 3 bilateral) were treated in 64 patients (46 boys, 18 girls). The mean ages of the patients were 59.7 ± 49.5 months (2 months – 17 years). In 2 preoperatively diagnosed right hernia patients were determined as bilateral inguinal hernia during operation. One patient underwent surgery due to incarceration and Amyand hernia was found per-operatively in one patient. Two patients had a recurrent inguinal hernia who had undergone open hernia repair before. Mean operating time was 22.5 ± 8.2 (9-45 min) minutes. Discharge time of the patients was 9.1 ± 5.5 hour (4-24 hour). Postoperative follow-up was 8.4 ± 4.1 months (5-12 months). No preoperative and postoperative complications were encountered. There was no recurrence in follow-up time.

Conclusion: Conventional single port-single instrument incisionless endoscopic inguinal herni repair by use intracorporeal knotting is a safe procedure with excellent cosmetic results. 

İntrakorporeal düğüm tekniği ile konvansiyonel tek port inguinal herni onarımı

Amaç: Endoskopik cerrahide tek port ile uygulanan yöntemlere rağbet artmaktadır. Konvansiyonel tek port kullanarak inguinal bölgeden ek girişim yardımı olmadan ve çalışma aletleri için ek port gerektirmeksizin inguinal herni onarımı uygulanması mümkündür. Bu çalışmada tek port ile laparoskopik  inguinal herni onarımı yöntemini sunmak istiyoruz.  

Gereç ve Yöntem: Kasım 2013- Şubat 2015 tarihleri arasında inguinal herni tanısıyla konvansiyonel tek port yöntemi ile tedavi edilen 64 hastanın kayıtları incelendi. Umbilikustan açık yöntemle 11 mm’lik konvansiyonel port yerleştirilerek 10 mm 0 derece 6 mm çalışma kanallı yan görüşlü teleskop kullanıldı. Konvansiyonel endoskopik bir portegü yan görüşlü teleskopun çalışma kanalından girildi.  İntrakorporeal kese ağzı dikişi yöntemiyle 2/0 polyester dikiş (iğne; 16 mm, ½ eğimli) (Ti-Cron, Covidien, USA) kullanılarak inguinal iç ring kapatıldı.

Bulgular: Ameliyat edilen 64 hastada (46 erkek, 18 kız) 68 inguinal herni (36 sağ, 26 sol, 3 bilateral) onarımı uygulandı. Hastaların yaş ortalaması 59,7±49,5 ay (2 ay-17 yaş) idi. Preoperatif sağ inguinal herni tanısı olan 2 hastada ameliyatta bilateral herni saptandı. Bir hasta inkaresere inguinal herni tanısıyla ameliyat edilirken, bir başka hastada intraoperatif Amyand  herni saptandı. Hastalardan ikisi daha önce açık herni onarımı uygulanmış olan nüks olgulardı. Ortalama ameliyat süresi 22,5 ±8,2 (9-45 dk) dakika idi. Taburculuk süresi 9,1±5,6 saat (4-24 saat) idi. Postoperatif takip süresi 8,4±4,1 aydı (5-12 ay). Preoperatif ve postoperatif komplikasyon saptanmadı. Takip süresinde nüks saptanmadı.

Sonuç: İntrakorporeal düğüm tekniğiyle tek port yöntemiyle inguinal herni onarımı mükemmel kozmetik sonuçları ile güvenli bir yöntemdir.
