VATS apical pleurectomy and chemical pleurodesis for spontaneous pneumothorax in children

Introduction :Spontaneous pneumothorax (SP)  is a rare life-threatening condition that requires rapid diagnosis and treatment . Most cases are secondary to various underlying lung pathology but a primary SP may occur due to rupture of bullae.  We herein describe  4  children who presented with recurrent SP and treated with VATS.

Cases:Two of them were boys and two of them were girls. Median age was 15(14-17).  Three patients underwent bilateral VATS apical pleurectomy, bullectomy and chemical pleurodezis. In the remaining patient left upper lobectomy and unilateral pleurodesis was performed. In all patients, chemical pleurodesis was performed with sterile talk. The mean removal time of the first shest tube was 5.7 days (3-9) and mean hospital stay was eight days (4-16). No intraoperative or postoperative complications were seen. Histopathological examination revealed eosinophilic bulla formation and lobectomy material was full of emphysematous lung tissue. The patients are doing well during  16 months follow up.

Conclusion:VATS and chemical pleurodesis is a minimal invasive, effective and safe alternative in management of spontaneous pneumothorax in children. 

Çocuklarda Spontan Pnömotoraksta VATS(Video Assisted Thoracoscopic Surgery) ile apikal plörektomi ve kimyasal plörodezis

Giriş: Spontan Pnömotoraks(SP), hızlı tanı konması ve tedavi edilmesi gereken, ender fakat hayatı tehdit eden bir durumdur. Seconder olgular  altta yatan akciğer patolojilerine bağlı olarak ortaya çıkabilirken, primer olgular akciğer dokusundaki büllerin açılması sonucu gelişir. Bu bildiride tekrarlayan SP olan  ve VATS ile tedavi edilen dört çocuk hastanın  sunulması amaçlandı.

Hastalar ve Yöntem: Spontan pnömotoraks nedeni ile acil servise başvuran çocuklardan ikisi kız, ikisi erkekti. Ortanca yaş on beşti. Üç olguda bilateral VATS, apikal plörektomi, bül eksizyonu ve kimyasal plöradezis yapıldı. Diğer olguda sol üst lobektomi ve tek taraflı plörodezis uygulandı.Tüm olgularda kimyasal plörodezis steril talk insuflasyonu ile yapıldı. Ortalama göğüs tüpünün çekilme süresi 5.7(3-9) gün, ortalama hastanede kalış süresi ise 8(4-16) gündü. Ameliyat öncesi ya da sonrası herhangi bir komplikasyon izlenmedi. Histopatolojik incelemede büllerde eozinofilik bül formasyonu , lobektomi materyalinde ise amfizematöz akciğer dokusu izlendi. Olguların 16 aylık izleminde herhangi bir problem yaşanmadı.

Sonuç: VATS ve kimyasal plörodezis, SP’ın tedavisinde minimal invaziv, etkili ve güvenli bir yöntemdir.
