Giant Urethral Stone in a Childhood Patient with Extrophia Vesica

Introduction: Among the all urinary tract stones, urethral stones are very rarely seen. Generally, most of the stones seen in the urethra are formed in the upper urinary tract and then passed through the urethra. In rare conditions, like as urethral stenosis, diverticule, and foreign body in urethra, the stones can be formed primarly in the urethra. Although urethral stones are rarely seen in children, they can have a variety of clinical presentation including disuria, hematuria, lower urinary tract obsturction and scrotal masses. The purpose of this case report is to present a giant urethral stone detected in a 7-year old boy who had a scrotal mass and a previous extropiha vesica repair surgery.

Case Report: A 7-year old boy admitted to our outpatient clinic with the compliant of a hard scrotal mass which was detected during an abceess drainage performed in a different center for the treatment of scrotal mass and disuria. A giant urethral stone was detected in plain abdominal radiography, scrotal ultrasonography and lower abdomen-pelvic computed tomography. While cystoscopy, the stone which was formed in the posteior urethra and detected in the diverticle without any sign of obstruction was get out with a scrotal incision of the diverticle. The wall of the diverticle was excised and the urethra was repaired.

Comment: It must be kept in mind that, in children who had comorbidities, the anatomical structure of the urethra may be broken leading to a diverticle and this may lead to a rarely seen lower urinary tract stones in this age group.

Çocuk Yaş Grubunda Ekstrofia Vezikalı Hastada Dev Üretra Taşı

Giriş:  Üriner sistem taşları içerisinde üretra taşları ender olarak görülmektedir. Genellikle üretradaki taşların büyük bir kısmı üst üriner sistemde oluşan sonradan üretraya geçen taşlardır. Üretra darlığı, divertikülü ve üretrada yabancı cisim varlığı gibi durumlarda nadir de olsa taş üretrada birincil olarak oluşabilmektedir. Literatüre bakıldığında, üretrada yer alan 63 gr’ lık dev taşın çocuk çağında görülen en büyük üretra taşı olması nedeni ile sunulması amaçlandı.

Olgu sunumu: Yenidoğan döneminde ekstrofiya vezikalis nedeni ile takip edilen yedi yaşında erkek çocuk skrotal kitle ve dizüri yakınması sonucu başvurduğu dış merkezden skrotal kitle ön tanısıyla sevk edildi. Çekilen düz karın radyografisi, skrotal ultrasonografi ve alt abdomen-pelvik bilgisayarlı tomografide dev üretral taş izlendi. Yapılan sistoskopi sırasında üretral obstruksiyona neden olmayan posterior üretrada oluşan divertikülde saptanan, 5x5x4 cm’lik ve 63 gr ağırlığındaki taş skrotal kesi ile divertikül açılarak çıkarıldı.

Yorum:  Altta yatan hastalığı olan çocuklarda üretranın anatomik yapısı bozularak duvarından divertikül gelişebileceği ve bu yaş grubunda ender de olsa alt üriner sistem taşları olabileceği akılda tutulmalıdır.
