EFFECTIVENESS OF ESWL IN PEDIATRIC RENAL STONES:WITH OR WITHOUT JJ STENT?

PURPOSE

We aimed to evaluate results of patients who had ESWL due to renal stones.

MATERIAL AND METHODS

Patients those admitted to Department of Pediatric Surgery due to renal stones between June 2012-June 2014 were evaluated retrospectively. They were divided into two groups due to replacement of jj stent. Gruop 1 had jj stent and group 2 had not.The demographic data; age, sex, stone size, location, session, and complications were recorded in each group. Elmed Complit ESWL system was used with 11-13kv, 60 frequency, 1000-1200 shots in 2-4 years old patients and 11-14 kv, 70 frequency, 1000-1500 shots over 4 years old patients.

RESULTS

In group 1 total of 15 session ESWL was performed to 5 female and 5 male children with mean age of 3.6 years and stone diameter of 9.2 mm. The location of renal stone was upper pole in 6 patients, 3 in lower pole, and 1 in ureteropelvic junction. jj-stent was placed in all patients preoperatively. 3 had hematuria, 1 had disuria postoperatively. 100 % of patients were stone free. In group 2 total of 18 session ESWL was performed to 8 female and 2 male children with mean age of 5.6 and stone diameter of 10 mm. The location of renal stone was upper pole in 1 patients, 7 in lower pole, and 2 in pelvis renalis.1 had hematuria, 2 had disuria and 1 of them had stone in external urethral meatus postoperatively. 80 % of patients were stone free,there was no fragmantation in 2 patients,and also one of them did not continue treatement.

CONCLUSIONS

In both group we did not recognise any complications in ureters due to stone fragmentation. Preoperative jj stent replacement is unnecessary in these patients. ESWL is a safety alternative treatement method in patients that have renal stones with low complication ratios.

Renal Taşlı Çocuklarda ESWL Etkinliği: jj katater kullanımı gerekli mi?

Amaç: Çocuklarda renal taş tedavisinde ESWL başarılı, komplikasyon oranı yüksek olmayan alternatif bir tedavi yöntemidir. İşlem öncesi jj katater yerleştirilmesinin gerekli olup olmadığı araştırıldı.

Yöntem: Haziran-2012 ile Haziran 2014 arası renal taş tanısıyla ESWL uygulanan olgular geriye dönük olarak incelendi. İşlem öncesi jj katater takılanlar ve takılmayanlar olarak iki gruba ayrıldı. Olgularda yaş, cinsiyet, taş büyüklüğü, taraf,  seans sayısı ve komplikasyonlar değerlendirildi. Elmed Complit cihaz ile 2-4 yaş arası olgulara 11-13kv, 60 frekans ile 1000-1200 atış, 4 yaşın üzerindeki olgulara 11-14 kv, 70 frekans ile 1000-1500 atış yapıldı.

Bulgular: Preoperatif jj stent takılan 5 kız 5 erkek,  ortalama yaşı 3.6 yıl, taş çapı 9.2 mm olan 10 olguya 15 seans ESWL uygulandı. Olguların 6’sında taş üst polde, 3’ünde alt polde ve 1 olguda üreteropelvik bileşkedeydi. Postoperatif 3 olguda hematüri 1 olguda ise disüri görüldü. Tamamında taşsızlık sağlandı. jj stent takılmayan 8 kız 2 erkek, ortalama yaşı 5.6 yıl, taş çapı 10 mm olan 10 olguya 18 seans ESWL uygulandı. Olguların 7’sinde taş alt polde, 1’inde üst polde ve 2 olguda pelvis renalisteydi. Postoperatif 2 olguda disüri, 1 olguda hematüri ve 1 olguda eksternal üretral meada taş görüldü. Taşsızlık oranı %80 idi.

Sonuç:Her iki grupta da taş kırılması nedeniyle üreterlerde obstrüksiyon görülmedi. Taş çapının 10 mm ve altında olduğu olgularda preoperatif jj katater kullanımı gerekli değildir.
