Our fundoplication experiences in children

Introduction: Nissen fundoplication is the most commonly performed antireflux surgery. Toupet and Dor fundoplications are the alternative procedures for the patients with weak esophageal peristaltis documented preoperatively. The aim of this study is to present our experience with fundoplication.

Patients and Method: A retrospective study from September 2013 to June 2015 was carried out in 23 children (6 male and 17 female) who underwent fundoplication.

Results: The mean age of the patients was 6.3±5.1 years (range, 5 months-17 years). The mean follow-up was 11.5±6 (range, 1-22) months. Fundoplication is performed for achalasia in 4 patients and gastroesophageal reflux disease in rest of them. 16 of the patients had associated anomalies (2 esophageal atresia, 12 neurological impairment, 3 hiatal hernia). The mean operative time was 146.7±36.8 (range, 75-190) minutes. Fundoplication was performed open in 7 (30.4%) patients and laparoscopically in 16 (69.6%) patients with one conversion from laparoscopic to open fundoplication for metabolic acidosis due to carbon dioxide insufflation. Nissen fundoplication, Toupet fundoplication and Heller myotomy combined with Dor fundoplication were performed in 18, 1 and 4 patients, respectively. Mucosal perforation was occurred in 1 patient and repaired laparoscopically. Gastrostomy was performed in 10 patients with neurological impairment. Mean postoperative hospitalization period was 14.9±8.9 (range, 6-34) days. We recorded 2 redo procedures (8.3%), one for dysphagia refractory to esophageal dilatation and one for recurrent hiatal hernia. Transient dysphagia was occurred in 2 patients (8.3%), which resolved after an average period of 3 months.

Conclusion: Antireflux procedures , especially laparoscopic types, yield satisfactory results. The choice of one procedure over others depends on the surgeon’s experience in performing it and the associated esophageal disorders.

Giriş: Nissen fundoplikasyon en sık kullanılan antireflü cerrahi yöntemi olmakla birlikte, özofagus peristaltizminin zayıf olduğu hastalarda Toupet ve Dor fundoplikasyonları da alternatif teknikler olarak tercih edilmektedir. Bu çalışmanın amacı, fundoplikasyon ile ilgili deneyimlerimizi sunmaktır.

Hastalar ve Yöntem: Eylül 2013- Haziran 2015 tarihleri arasında fundoplikasyon yapılan 23 (6 erkek, 17 kız) hasta geriye dönük olarak incelendi.

Bulgular: Hastaların ortalama yaşı 6.3±5.1 yaş (5 ay-17 yaş) idi. Ortalama takip süresi 11.5±6 (1-22 ) aydı. 4 hastaya akalazya, 19 hastaya ise gastroözofageal reflü hastalığı nedeni ile fundoplikasyon yapıldı. Hastaların 16’sında ek patoloji ( 2 özofagus atrezisi, 12 nörolojik bozukluk, 3 hiatal herni) mevcuttu. Ortalama operasyon süresi 146.7±36.8 (75-190) dakikaydı. 7 (%30.4) hastada açık, 16 (%69.6) hastada laparoskopik cerrahi tercih edildi. Karbondioksit insuflasyonuna bağlı gelişen metabolik asidoz nedeni ile 1 hastada laparoskopiden açık cerrahiye geçildi. 18 hastaya Nissen fundoplikasyon, 1 hastaya Toupet fundoplikasyon ve 4 hastaya Heller myotomi ile birlikte Dor fundoplikasyon yapıldı. 1 hastada meydana gelen mukozal perforasyon laparoskopik olarak onarıldı. Nörolojik hastalığı olan 10 hastaya gastrostomi yapıldı. Ortalama hastanede kalış süresi 14.9±8.9 (6-34) gündü. Bir hastaya özofagus dilatasyonuna yanıtsız disfaji ve 1 hastaya da nüks hiatal herni nedeni ile redo antireflü cerrahi yapıldı. 2(%8.3) hastada gelişen disfaji postoperatif 3 ay içinde kendiliğinden düzeldi.

Sonuç:  Fundoplikasyon antireflü cerrahide tatmin edici sonuçlar sunmakta ve son yıllarda genellikle laparoskopik olarak yapılmaktadır. Fundoplikasyon tipinin seçimi, cerrahın tercihine ve eşlik eden özofageal hastalığa bağlıdır.
