A novel technique for isolated esophageal atresia surgery: colonic transposition without cervical esophagostomy.

Introduction and Aim:

Colonic replacement is one of the most commonly performed procedures in long gap esophageal atresia (EA) surgery. An initial cervical esophagostomy is the classical procedure before replecement surgery. Esophagostomy may itself cause compliactions before and after the definitive surgery. This study presents a clinical experience on esophageal colonic replacement surgery without cervical esophagostomy.

Patients and Methods:

Long gap EA patients who underwent colonic replacement surgery without esophagostomy between the years 2010-2014 were retrospectively evaluated.

Results:

There were three male patients operated on as described. Their birth weights were 3500 gr, 2230 gr and 1900 gr, in order of admission to the clinic. They all underwent Stamm gastrostomy as the initial operation and a delayed primary repair was planned. Definitive surgery was done at 5, 5.5 and 3 months of age in respective order. They weighed 8, 6.8 and 4.4 kg on the day of surgery. A right thoracotomy incision was done. The proximal pouch was dissected free. An exploration by the help of a metallic stent inserted through gastrostomy revealed no distal esophageal pouch. A laparatomy was then done and the distal esophageal segment was confirmed to be a very short stump below the diaphragm in all three. A left colonic segment in the first patient and transverse colonic segments in the other two were prepared. The colonic segments were brought up to chest via transhiatal route in an isoperistaltic manner. The proximal esophagocolonic anastomosis was done within the thorax. The distal anastomosis was done behind the stomach.  The early postoperative course was uneventful. A proximal anastomotic stricture needed dilatation in one patient. The mean follow up period is 28 (9-55) months. All three are fully fed orally by age-appropriate diet.

Conclusion:

No cervical esophagostomy was done before definitive surgery in the presented technique. That allowed protection of proximal esophageal pouch intact and without the need of working in the cervical region the proximal anastomosis was kept within the thorax. By means of that, any compressive force that might act on esophagus at the level of thoracic inlet with the potential to impair vascularity could be avoided. Moreover, the patients had the chance of using a much longer segment of their native esophagus. Long term clinical follow-up showed a quite satisfactory result in terms of swallowing and feeding.

Giriş ve Amaç:

Özofagus atrezisi’ne (ÖA) primer anastomozun mümkün olmadığı hallerde en sık uygulanan yöntemlerden birisi kolon transpozisyonudur. Klasik olarak kolon transpoziyonu planlanan olgulara öncelikle servikal özofagostomi uygulanır. Ameliyat daha sonraki bir tarihte gerçekleştirilir.  Özofagostomi, transpoziyon ameliyatı öncesi ve sonrasında kendine özel bazı komplikasyonlara yol açabilir. Bu çalışmada servikal özofagostomi uygulanmadan gerçekleştirilen kolon transpoziyonu ameliyatına ilişkin deneyimin sunuldu. 

Hastalar ve Yöntem:

2010-2014 yılları arasında uzun aralıklı ÖA nedeniyle özofagostomisiz kolon transpoziyonu yapılan hastalar geriye dönük tarandı.  

Bulgular:

Tanımlanan şekilde ameliyat edilen hepsi erkek üç hasta vardı. Başvuru sırasıyla, doğum tartıları 3500 gr, 2230 gr ve 1900 gr idi. Birincil ameliyatta üçüne de Stamm gastrostomi açıldı ve geciktirilmiş primer onarım planlanarak izleme alındı. Definitif ameliyat sırasıyla, 5; 5,5 ve 3 aylıkken gerçekleştirildi. Ameliyat günü tartıları 8; 6,8 ve 4,5 kg idi. Sağ torakotomi kesisi ile girildi. Proksimal poş serbestlendi. Gastrostomiden ilerletilen buji eşliğinde yapılan ekplorasyonda distalde güdük olmadığı saptandı. Batın açılarak her üç hastada da distal güdüğün diyafram altında çok küçük bir segment olduğu teyit edildi. İlk hastada sol kolon ve diğer iki hastada transvers kolon hazırlanarak, transhiatal yoldan izoperistaltik olarak toraksa çekildi. Proksimal özofagokolonik anastomoz toraksın içinde kalacak şekilde gerçekleştirildi. Distal anastomoz mide arkasına yapıldı. Erken takipleri sorunsuz seyretti. Bir hastada proksimal anastomozda darlık nedeniyle dilatasyon yapıldı. Ortalama takip süresi 28 (9-55) aydır. Her üç hasta da yaşa uygun diyetle tam oral beslenmektedir.

Sonuç:

Bu çalışmada sunulan cerrahi yöntemde hastalara definitif ameliyat öncesi servikal özofagostomi yapılmamıştır. Bunun neticesinde proksimal özofagus poşu bütün olarak korunabilmiş ve servikal bölgede çalışılmaya gerek kalmadan, proksimal anastomoz toraks içinde bırakılmıştır. Bu sayede torasik girimde özofagusun kanlanmasını etkileyebilecek her hangi bir bası unsuru kalmamış ve hastaların kendi özofaguslarını daha uzun bir segment olarak kullanması mümkün olmuştur. Uzun dönem klinik değerlendirmede yutma ve beslenme anlamında son derece başarılı sonuç elde edilmiştir.
