PREFERRING TO USE SETON ON ANAL FISTULA IN PEDIATRIC SURGERY PRACTICE

Aim:

Fistula in ano requires surgery is a healthy problem in anorectal diseases of childhood. It is more frequent in male. Treatment options for fistula are commonly fistulotomy and fistulectomy. Although excellent healing there was recurrence rate. It is known that cutting seton have better control of continance and healing than other procedures. The aim of study was evaluate to use cutting seton for anal fistula.

Method:

 Latest one year, the hospital records of two (n=2) male patient suffers from perianal fistula were examined. Aged of patients were 14 to16 years old. Crohn’s disease was not detected on patients. Both of them were studied with magnetic resonance.

Results:

All of the patient magnetic resonance imagining has been reported as intersphincteric fistula and single fistulatract . vCutting seton was performed on patients . Pathological findings were resulted as fistula tract. Both of the patients were discharged on third day after surgery. During the postoperative follow-up periods local infection, hemorrhage wasn’t observed. The time of removal seton was avarege two months. Both of the patients were incontinance at the end of procedure. Wound healing was sucsessful.

Conclusion:

Perianal fistula seton implementation is a positive initiative that results in terms of continence and recovery in children, but it was observed that the recovery in the process take a long time. We think that is an appropriate treatment option for children with ketone with these results

ÇOCUK CERRAHİSİ PRATİĞİNDE PERİANAL FİSTÜLDE SETON UYGULAMSI

Amaç:

Perianal fistül, çocukluk çağı operasyon gerektiren anorektal hastalıklarda için en sık görülen sağlıklı bir sorundur. Erkeklerde sık görülür. Fistül için tedavi yaygın seçenekleri fistülotomi ve fistülektomidir. Tam iyleşmeye rağmen nüks görülmektedir. Diğer cerrahi prosedürlere göre Seton ile yönetim daha iyi kontinans ve iyileşmeye sahip olduğu bilinmektedir. Bu çalışmada çocuklarda Seton ile perianal fistül tedavisini değerlendirmek amaçlandı.

 

Yöntem:

Perianal fistülü olan iki (n = 2) erkek hastanın hastane kayıtları incelendi. Hastaların yaşı 14 ve16 yaşında idi. Crohn hastalığı olan hastalarda tespit edilmemiştir. Hastalara Manyetik Rezonans (MR) inceleme yapıldı.

 

Bulgular:

Hastaların MR’ı intersfinkterik tek fistül olarak rapor edildi. Hastalara seton yötemi ile perianal fistül onarımı yapıldı. Patolojik bulgular fistül yolu olarak sonuçlandı.Her iki hastada ameliyat sonrası üçüncü günde taburcu edildi. Ameliyat sonrası takip süreleri boyunca lokal enfeksiyon ve kanama gözlenmedi. Ortalama iki ayda tam iyleşme sağlandı. Hastalarda iyleşme sonrasında inkontinans saptanmadı.

Sonuç:

Perianal fistülde seton uygulanması kontinans ve iyleşme açısından çocuklarda olumlu sonuç veren bir girişim olduğu ancak iyleşme sürecinin uzun zaman aldığı gözlemlendi. Bu sonuçlar eşliğinde çocuklarda seton ile tedavi uygun bir seçenek olduğunu düşünüyoruz.
