A NOVEL METHOD IN IRREDUCIBLE PEDIATRIC IDIOPATHIC ILEOCOLIC INTUSSUSCEPTION: EXTERNAL MANUAL REDUCTION MANEUVER UNDER FLUOROSCOPY

PURPOSE:
Intussusception is one of the most common causes of pediatric emergency. Fluoroscopy-guided hydrostatic/air reduction is a common non-operative management strategy for the treatment. A number of different approaches have been described to try to improve intussusception reduction on enema that include sedation, anesthesia, use of glucagon and delayed repeat enema.The aim of this study is to present the efficacy of external manual reduction under fluoroscopy in pediatric irreducible ileocolic intussusception.

MATERIALS AND METHODS:

After 2012, the external manual reduction maneuver under fluoroscopy was carried out in 10 children with irreducible idiopathic ileocolic intussusception.  The mean age was 12 months. (range 7-21 months). Children with the diagnosis of intussusception had properly fluid-resuscitation and antibiotics.  Then hydrostatic barium enema technique was used in reduction of intussusception.  The barium colon was started from a height of 3 ft, but it was slowly raised to higher level if necessary to see the sudden rush of Barium in to the terminal ileum.  If this not happened after waiting 30 min, it was assumed that the reduction was incomplete.  In these cases, we started massaging with our right hand, in an anti-clockwise direction, distal to the intussusception until meeting its apex. At this point, we gradually increased pressure, from the apex to the base, according to the degree of resistance provided by the intussusception and abdominal wall muscle tone.  We intermittently checked with fluoroscopy whether achieved reduction.
There was no morbidity in our cases.

CONCLUSION:
External manual reduction maneuver under fluoroscopy is a safe and effective procedure in irreducible intussusception during hydrostatic barium enema technique.


İDİOPATİK İLEOKOLİK İNVAJİNASYON TEDAVİSİNDE YENİ BİR YÖNTEM: FLOROSKOPİ ALTINDA EKSTERNAL MANUEL REDUKSİYON

AMAÇ:

İnvajinasyon, çocuk aciller arasında en sık başvuru sebeplerinden biridir. Floroskopi eşliğinde hidropnömotik reduksiyon ise en sık uygulanan cerrahi dışı tedavi yöntemidir. İnvajinasyon tedavinde uygulanan redüksiyon işleminin başarısının artırılması için sedasyon, anestezi,glukagon kullanımı ve tekrarlayan enema yöntemleri geliştirilmiştir.Bu çalışmanın amacı ise floroskopi eşliğinde eksternal manuel reduksiyonun redükte edilemeyen idiyopatik invajinasyonlardaki değerini vurgulamaktır.

MATERYAL ve METOD:

2012 yılı sonrası invajinasyonlarda eksternal manuel redüksiyon, redükte edilemeyen 10 ileokolik invajinasyonlu hastada uygulandı. Hastaların yaş aralığı 7-21 ay arasında ortalama yaşları ise 12 aydı. İnvajnasyon tanısını alan çocuklara sıvı replasmanı ve uygun antibiyoterapi başlandı. Sonrasında hidrostatik baryumlu enema tekniği redüksiyon için tercih edilmiştir. Yaklaşık 90 cm yükseklikten gönderilen baryumlu mayi redüksiyonun gerçekleşmemesi durumunda yüksekliğin yavaş olarak artırılması ile 30 dakika denemeye devam edilmiştir.  

Süre sonunda redüksiyonun gerçekleşmediği durumlarda işlem başarısız olarak değerlendirildi. Bu vakalarda sağ el ile masaj işlemine saat yönünün tersi yönünde invajinasyonun distalinden apekse doğru başlandı.Karın duvarı kas tonusu ve invajinasyonun uyguladığı direncin derecesine göre tersi yönünde basınç yavaş olarak artırıldı.Aralıklı olarak floroskopi ile redüksiyonun olup olmadığı kontrol edildi.

SONUÇ:

Hidrostatik baryum redüksiyon ile redükte edilemeyen hastalarda eksternal manuel reduksiyon manevrası, floroskopi altında uygulanan güvenli ve etkili bir yöntemdir.
