Pancreatitis of Children

Aim: To evaluate the patients with pancreatitis, followed and treated in our clinic. 

Patients and Method: The files of patients, who followed and treated by our clinic with the diagnosis of pancreatitis between the years 2000-2015 were reviewed retrospectively. Age, sex, complaints, Amylase values, etiology, treatment of choice, progress and complications were evaluated.

Findings: The mean age of 32 patients(15 F, 17 M)  with pancreatitis was  9.2 years (2-17 years). All patients were admitted with the complaint of abdominal pain, whereas 22 of the patients were complaining of vomiting and two had jaundice. Mean Amylase levels were 342 IU/mL (145-2365 IU/mL). The etiologies of pancreatitis were biliary stone disease (n:8), idiopathic (n:8), metabolic disease (Type 1 hyperlipidemia, familial hyperlipidemia; n:2), FMF (n:2), cystic fibrosis (n:2), viral (n:2), choledochal cyst (n:2), iatrogenic (L-Asparaginase; n:1), anatomical variation (n:1), surgery following corrosive pylor burn (Billroth I; n:1), side-to-side pancreoticojejunostomy in another center (n:1), otoimmun disease (n:1) and trauma (n:1). Conservative treatment was of choice for the 20 of the patients whereas surgery was done on patients with biliary stone disease-hereditary spherocytosis (n:8), chronic pancreatitis (n:3) and choledochal cyst (n:1). Surgery was planned on another patient with choledochal cyst. In the conservative treatment group, 11 had once, five had twice, three had three times and one patient had 5 times of pancreatitis attacks. Pancreas pseudocyst was developed in two patients following pancreatitis, one had surgery and spontaneous regression was observed on the other. None of the patients died in the mean 4.2 years (4 m-15 y) of follow-up period. 


Conclusion: Pancreatitis could arise depending on different etiological factors. Higher Amylase ratio is an important marker. The knowledge of etiology directly effects the choice of the treatment. Etiology-oriented sophisticated surgical intervention could be required for most of the patients.

Amaç: Kliniğimize takip ve tedavileri yapılan pankreatit  tanılı olguların değerlendirilmesi.

Olgular ve Yöntem: 2000-2015 yılları arasında kliniğimizde pankreatit tanısıyla takip ve tedavileri yapılan hastaların dosyaları geriye dönük olarak incelendi. Olguların yaş,cinsiyet, başvuru yakınması, amilaz değerleri, etyoloji, tedavi yöntemi, seyir ve komplikasyonlar incelendi.

Bulgular: Pankreatit tanısıyla takip ve tedavi edilen 32 olgunun (15 K-17 E) yaş ortalaması 9.2 yıl(2-17 yıl) idi. Olguların tümü karın ağrısı yakınmasıyla başvururken ek olarak 22 olguda kusma 2 olguda ise sarılık mevcuttu. Başvuruda ortalama amilaz değerleri 342 U/ml (145-2365) idi. Pankreatit etyolojisi, safra yolları taş hastalığı (n:8), idiyopatik (n:8) , metabolik hastalık(tip 1 hiperlipidemi, ailevi hiperlipidemi) (n:2), FMF (n:2), , kistik fibrozis (n:2), viral (n:2), koledeok kisti  (n:2), ilaca bağlı ( L-Asparaginaz)(n:1), anatomik varyatif değişiklik (n:1), koroziv pilor yanığı nedeniyle yapılan cerrahi sonrası (Billroth I) (n:1) , dış merkezde yapılan side to side pankreatikojejunostomi sonrası (n:1), , otoimmun hastalık (n:1) ve travma (n:1) idi. Olgulardan 20'sinde konservatif tedavi uygulanırken, safra yolları taş hastalığı-herediter sferositoz nedeniyle (n:8), Kronik pankreatit nedeniyle (n:3) ve koledok kisti nedeniyle bir olguda cerrahi girişim yapıldı. Koledok  kisti olan bir olguda cerrahi girişim planlandı. Konservatif olarak tedavi edilen 11 olguda 1 defa, 5 olguda 2 defa, 3 olguda 3 defa ve 1 olguda 5 defa pankreatit atakları izlendi. İki olguda pankreatit sonrası pankreas psödokisti gelişti, 1 olguda cerrahi girişim uygulandı, bir olguda ise takipte spontan geriledi.  Ortalama 4,2 (4 ay-15 yıl) yıllık takipte kaybedilen olgu yoktur.


Sonuç: Pankreatit farklı etyolojik faktörlere bağlı ortaya çıkabilir. Tanıda amilaz yüksekliği önemli bir belirteçtir. Etyolojinin bilinmesi tedavinin seyrini doğrudan etkiler. Birçok olguda etyolojiye yönelik komplike cerrahi girişim gereki
