The approch of Turkish pediatric surgeons to acute appendicitis

Aim: Although appendicitis is the most common surgical pathology of the abdomen, the diagnosis,  treatment, and follow-up shows a wide variations. We aimed to evaluate the approach Turkish pediatric surgeons to acute appendicitis.
Materials and Methods: W e sent a questionnaire consisting of 13 questions by e-mail to the address of  400 pediatric surgeons of  Turkish Association of Pediatric Surgeons members . We asked to answer the questions.
Results: The 102 (25,5%) of 400 pediatric surgeons replied the questionnaire. the 45 (44.55%)  of survey respondents pediatric surgeons have worked at  university hospital, 35 (34.65%) at Health ministry of education and research hospital, 14 (13.86%) in the State Hospital, 7 (6% 93)  in the private hospitals. The 73 (73%)of pediatric surgeons diagnosed acute appendicitis with physical examination and ultrasonography, 15 (15%)  with physical examination and laboratory findings, 10 (10%)  with  abdominal  tomography, 2 (2%) with  MRI. The 21(21%) of the pediatric surgeons think that appendectomy is an emergent operation. The 60 (60%) of the pediatric surgeons think that appendectomy should be performed in the early period. The 19 (19%) of pediatric surgeons think that an appendectomy is not an emergent operation. We asked to the pediatric surgeons approached to acute appendicitis after the night 23 o’clock. The 53 (53%) of pediatric surgeons were leaving the patients until the next day and  he operated the patient as a  first case, 20 (20%) of pediatric surgeons when they were at the hospital, they immediately performed appendectomy.17 (17% )  left the patients until next day and performed appendectomy after elective cases, 10 (10%) even though at  home, he said no matter what time the patient received surgery. When we asked to the pediatric surgeons about nonoperative and surgery,  71 (71%) surgeons said that nonoperative treatment has no place in treatment of acute appendicitis, 29 (29%) said that it can be applied in the some of  selected cases. 42 (42%) of the  pediatric surgeons have  performed open appendectomy, 36 (36%) have performed  open and three ports laparoscopic appendectomy, 18 (18%) have performed 3-ports laparoscopic, 4 (4%) have performed one  port laparoscopic appendectomy. The 36 (35.64%)  pediatric surgeons have been used drains in perforated appendicitis, 36 (35.64%) of them  have been sometimes used, 29 (28.71%) of them have  not used.
Conclusions: The majority of Turkish pediatric surgeons have used imaging methods for diagnosis, have thought that appendectomy is not an emergent operation but  an operation should be done early,  used drain, have preferred both  open and laparoscopic appendectomy with close proportions.

Türk çocuk cerrahlarının akut apandisite yaklaşımı

Amaç: Apandisit karnın en yaygın cerrahi patolojisi olmasına karşın,  tanı, takip ve tedavisi büyük farklılıklar göstermektedir. Türk çocuk cerrahlarının akut apandisite yaklaşımlarını değerlendirmeyi amaçladık.

Gereç ve Yöntem: 2014 yılında Türkiye Çocuk Cerrahisi derneğine üye 400 çocuk cerrahının adresine elektronik postayla  13 sorudan oluşan anket formu gönderilerek, soruların cevaplanması istendi.

Bulgular: 400 çocuk cerrahından 102 (%25,5)’si ankete cevap verdi. Ankete cevap veren çocuk cerrahlarının 45(% 44,55)’i üniversite hastanesinde, 35 (%34,65)’i sağlık bakanlığı eğitim ve araştırma hastanesinde, 14 (%13,86)’ ü devlet hastanesinde, 7(%6,93)’si özel hastanede çalışıyordu. Çocuk cerrahlarının 73 (%73)’ü fizik muayene ve ultrasonografiyle, 15 (%15)’i fizik muayene ve laboratuvar bulgularıyla, 10 (%10)’u batın tomografiyle, 2 (%2)’si MRG ile akut apandisit tanısı koymaktadır. Çocuk cerrahlarının 21(%21) akut apandisit ameliyatının acil, 60 (%60)’ı erken yapılması gereken bir ameliyat, 19(%19) acil bir ameliyat olmadığını düşünüyor. Gece 23 ten sonra gelen akut apandistli hastaya yaklaşımı sorduğumuzda, çocuk cerrahlarının 53(%53)’ü ertesi güne bırakıp ilk vaka olarak aldığını, 20(%20)’ si hastanede olduğu taktirde hangi saatte olursa olsun hastayı ameliyata aldığını, 17 (%17)’si ertesi güne bırakıp elektif vakalardan sonra aldığını, 10(%10)’u evde olsa bile hangi saat olursa hastayı ameliyata aldığını belirtti. Akut apandisitli hastayı ameliyat etmeden sadece antibiyotikle tedavi edip etmediğini sorduğumuzda çocuk cerrahlarının 71(%71) tedavide yeri olmadığını, 29 (29%) seçilmiş bazı olgularda uyguladığını belirtti. Ameliyatı nasıl yaptıklarını sorduğumuzda çocuk cerrahlarının 42(%42)’ si açık apendektomi, 36(%36)’sı açık ve 3 port la laparoskopik, 18 (18%)’ si 3 portla laparoskopik, 4 (4%) ‘ü tek portla laparoskopik appendektomi yaptıklarını belirttiler. Perfore apandisitte, çocuk cerrahlarının 36 (% 35,64) ‘sı dren kullanıyordu, 36 (%35,64)’sı bazen kullanıyordu, 29 (%28,71)’ u kullanmıyordu.

Sonuç: Türk çocuk cerrahlarının  büyük çoğunluğu tanıda görüntüleme yöntemlerini kullanmakta , appendektominin acil değil, erken yapılması gereken bir ameliyat olduğunu düşünmekte, büyük bir kısmı dren kullanmakta, appendektomide açık ve laparoskopik yöntemleri birbirlerine yakın oranlarda tercih etmektedir.
