Mowat-Wilson Syndrome

AIM: Mowat-Wilson Syndrome is multiple congenital anomaly which 250 different cases published until now. We wanted to present the features of this syndrome by our case, which is followed from the newborn period.


MATERIAL AND METHODS: The patient was transported from the newborn clinic with the prediagnosis of  Hirschsprung’s disease (HD). All clinical features were examined and operation was planned.


FINDINGS: The history of X group medication and polyhydramniosis was attained from the mother. The patient was operated for abdominal distension and Hirschsprung’s disease and sigmoid colostomy was performed. Mowat-Wilson syndrome was suspected in the patient due to typical facial appearance (high forehead lines, prominent forehead, deep-seated and large eyeballs, hypertelorism, saddle nose, wide eyebrows, ear anomalies, open mouth, protruding and triangular chin, M-shaped upper lip, prominent columella, wide neck), urogenital anomalies (hypospadias, embedded and torsion of the penis, VUR), eye disorders, congenital heart defects (ASD, PDA), the corpus callosum abnormalities, epilepsy, intellectual impairment (growth and mental retardation, speech impairment and Hirschsprung's disease.

The diagnosis was confirmed by genetic studies and surgeries for Hirschsprung's disease and cardiac abnormalities has been completed. He is under follow up for other interventions.


RESULT: Mowat-Wilson syndrome which has a Hirschsprung's disease component, .is an important entity. Urgent intervention should be planned for HD in a patient with typical facial features. Other features of the syndrome should be sought when the final genetic diagnosis is made.

AMAÇ: Mowat-Wilson Sendromu, günümüze kadar 250 değişik özelliklerde olgular yayınlanmış çoklu bir doğumsal anomalidir. Yenidoğan döneminden başlayarak izlenen olgumuz sendromun özelliklerini anlatmak amacı ile sunulmuştur.


GEREÇ VE YÖNTEM: Hastanemiz yenidoğan ünitesinden Hirschsprung hastalığı ön tanısı ile kliniğimize nakil edilen hastanın tüm klinik özellikleri incelenmiş ve anomalilerin sağaltımı için ameliyatlar planlanmıştır. 


BULGULAR: Öyküsünde, annenin hamilelik döneminin 5. Haftasına kadar X grubu ilaç kullanımı olduğu ve polihidramnioz bulunduğu öğrenilmiştir. Karında distansiyon bulunan hasta ameliyata alınmış ve ameliyatta Hirschsprung hastalığı saptanarak sigmoid kolostomi yapılmıştır. Hastada tipik yüz görünümü (yüksek alın çizgisi, çıkıntılı alın, derin yerleşimli ve geniş gözbebekleri, hipertelorizm, semer burun, geniş kaşlar, kulak anomalileri, açık ağız, çıkıntılı ve üçgen şeklinde çene, M şeklinde üst dudak, belirgin columella, geniş ense), ürogenital anomaliler (hipospadias, gömülü ve torsiyone penis, VUR), göz bozuklukları, doğumsal kalp anomalileri (ASD, PDA), corpus callosum anomalisi, epilepsi, entelektüel bozukluk (gelişme ve zeka geriliği, konuşma bozukluğu), eklem bozuklukları ve Hirschsprung hastalığı ile Mowat-Wilson Sendromu düşünülmüştür. Genetik incelemeler ile tanısı kesinleşen hastanın Hirschsprung hastalığı ve kalp anomalilerine yönelik ameliyatları tamamlanmıştır. Diğer girişimleri için hastamızın izlemi devam etmektedir. 


SONUÇ: Mowat-Wilson Sendromunun bir parçasının da Hirschsprung hastalığı olması yönünden önemlidir. Tipik yüz görünümü ile dikkat çeken böyle bir hastada Hirschsprung hastalığının bulunacağı düşünülerek acil girişimler planlanmalıdır. Genetik tanı kesinleştiği zaman sendromun diğer özellikleri de aranmalıdır.
