Paratubal cyst associated with flegmonous appendicitis: a case report and review of literature

Paratubal cysts are remnants of paramesonephric or mesonephric ducts and are mostly benign.These masses most commonly are seen in the third and fourth decades of life. A 14-year-old girl had signs of acute surgical abdomen. All laboratory parameters were normal, except an increase in white blood cell count (16300/µL). In addition to a normally perfused left ovary with multiple little cysts, a Pelvic Doppler ultrasonography demonstrated a cystic mass adjacent to the right ovary (60x50x40 mm) with no perfusion. The abdominal MRI revealed a unilocular cystic mass with no septation or mural nodule (64x54x43 mm) consistent with a paratubal cyst. With the suspicion of torsion of the ovarian pedicle or fallopian tube accompanied by a paratubal cyst, the patient underwent an emergent exploration. Laparotomy revealed that both fallopian tubes and ovaries were normal. A mass  (6x5 cm) between the left fallopian tube and left ovary was found and enucleated totally preserving the adjacent ovary and fallopian tube. Synchronous abdominal exploration also revealed an inflamed appendix. Standard appendectomy was added to the procedure. Histopathology revealed a flegmonous appendicitis and a paratubal cyst with a smooth inner surface with a thin wall. The patient had an uneventful postoperative course. Paratubal cysts are rare clinical entities and are believed to originate from mesothelium or be remnant of Mullerian duct and Wolffian duct. Most reported cases of PTCs have occured in pediatric patients. Larger lesions may become symptomatic causing pressure or pain. Treatment options include enucleation, cystectomy, fenestration of cyst, aspiration of cyst fluid. There is no pediatric case of PTC associated with a flegmonous appendicitis in the English language literature. This is the second report of a combination of PTC and flegmonous appendicitis and first pediatric case. PTC and flegmonous appendicitis can be seen together as distinct entities in children.

Paratubal kist ve flegmonöz appendisit birlikteliği: olgu sunumu ve literatür incelemesi

Paratubal kistler (PTK) paramezonefrik ya da mezonefrik kanal artıkları olup çoğu benigndir. Bu kitleler yaşamın sıklıkla 3. ve 4. dekadında görülürler. 14 yaşında kız olgu akut abdomen tablosu ile kliniğe kabul edildi. Hafif bir lökositoz (16300/µL) dışında tüm laboratuar parametreleri normaldi. Çoğul küçük kistler içeren normal perfüze olan sol overe ek olarak Pelvik Doppler ultrasonografisinde perfüzyonu olmayan sağ over komşuluğunda kistik kitle (60x50x40 mm) saptandı. Abdomen MRG’de PTK ile uyumlu mural nodül ya da septasyonun olmadığı uniloküler kist (64x54x43 mm) saptandı. PTK’nin eşlik ettiği over ya da tubal torsiyon şüphesiyle olguya acil laparotomi uygulandı. Laparotomide her iki overin normal olduğu saptandı. Sol over ve tuba uterina arasında saptanan kitle (6x5 cm) komşu over ve tuba korunarak total olarak enüklüye edildi. Eş zamanlı yapılan abdominal eksplorasyonda inflame appendiks saptandı ve standart appendektomi cerrahi işleme eklendi. Histopatalojik incelemede flegmonöz appendisit ve iç yüzeyi düzgün ve ince duvarlı PTK saptandı. Ameliyat sonrası gidişat sorunsuz seyretti. PTK’ler mezotelyumdan köken aldığı, Müllerian ya da Wolffian kanal artğı olduğuna inanılan nadir klinik durumlardır. Bildirilen çoğu PTK’ler pediatrik olgularda saptanmıştır. Daha büyük kistler bası ya da ağrı yaratarak semptomatik hale gelebilirler. Tedavi seçenekleri arasında enükleasyon, kistektomi, kist fenestrasyonu ya da aspirasyonu sayılabilir. Literatürde flegmonöz appendisit ile birlikte saptanmış pediatrik PTK olgusu bulunmamaktadır. Sunulan olgu PTK ile flegmonöz appendisit birlikteliği olan ikinci ve ilk pediatrik olgudur. Bu olgudan da anlaşılacağı üzere PTK ve flegmonöz appendisit çocuklarda birlikte görülebilir.
