An alternative to inguinal hernia repair: The experiences on Percutaneous Internal Ring Suturing (PIRS)

Background:  The objective of this study is to present the outcome of our experiences withPIRS over a period of four years along with technical details and clinical results of inguinal hernias treated at an institution where laparoscopic repair is optionally done.

Methods: The PIRS technique was usedin 365 children (172 males, 193 females). All data were collected from October 2011 to November 2015. All complications and hernia recurrences were documented.

Results: In total, 462 laparoscopic herniorraphies using thePIRS technique were performed. The mean age was 6.67±3.04 years (2 mo-17 y). The mean operative time was 13.96±7.83 minutes. A direct inguinal hernia and one femoral hernia were discovered in two different male cases. Both of them were operated using PIRS. In these two cases, the internal ring was found closed. A recurrence was identified in five cases (1.08%).  All recurrences appeared within the first six months postoperatively and underwent a repair using the open approach. The mean follow-up period was three years.  Only one transtitional hydrocele was observed, and it resolved spontaneously in six months. No iatrogenic cryptorchidism or atrophy was found in the patients. In 8 cases operated with polypropilene suture, a stitch absess was observed, no abcess was found in cases operated with polyester multifilament suture.

Conclusion:
PIRS is safe, reproducable, and offers excellent cosmetic results that are comparable with an open approach. This technique offers an alternative to open hernia repair in paediatric patients.

Amaç:  Bu çalışmanın amacı laproskopinin opsiyonel olarak kullanıldığı bir çocuk cerrahisi kliniğinde dört yıl süresince PIRS yöntemi ile opere edilmiş inguinal hernili olguların sonuçlarını paylaşmaktır.

OLgular ve Yöntem:  Ekim 2011ile kasım 2015 tarihleri arasında 365 (172 erkek, 193 kız) inguinal hernili olgu PIRS yöntemi ile opere edildi.  Tüm nüksler ve komplikasyonlar dökümante edildi.
Toplam olarak,  PIRS yöntemi ile 462 laparoskopik herni onarımı yapıldı. Ortalama yaş 6.67±3.04 yıl (2 ay-17 yıl), ortalama operasyon süresi 13.96±7.83 dakika olarak saptandı.  Bir erkek hastda direk diğer erkek hastada ise femoral herni saptandı. Her ikisi de PIRS yöntemi ile opere edildi. Bu iki olguda iç delik ağızları kapalıydı.  Beş hastada nüks gelişti (%1.08).  Tüm nüksler operasyondan sonraki altı ay içinde gelişti ve açık yöntemle onarıldılar. Sadece bir hastada 6 ay içinde spontan olarak kaybolan hidrosel gelişti. Post operatif dönemde iyatrojenik testis ya da testis atrofisi saptanmadı.  Polipropilen sütür kullanılan sekiz hastada sütür absesi gelişti. Polyester sütür kullanılan hastalarda böyle bir komplikasyon saptanmadı.

Sonuç:
PIRS güvenli ve estetik yönden üstün bir yöntem olup çocukluk çağındaki herni operasyonlarında açık yönteme bir alternatiftir.
