Clinical and histopathologic features of midline cervical lesions in children

Aim: A retrospective study was performed to evaluate the clinical and histopathological features of midline cervical lesions in children.
Methods: Children who underwent surgical excision of midline cervical lesions for the last 15 years were evaluated for age, sex, clinical and diagnostic features, results of surgical treatment and histopathological findings.
Results: Seventy cases with surgical excision were included in the study. Fifty-seven of the patients had thyroglossal canal cyst (TCC), and 13 of them had dermoid cysts (DC). Male to female ratio was 33:24 in TCC and 8:5 in DC. The mean age of the patients were 4.9  2.7 and 6.4  4.8 respectively. The most common presentation of TCC was cervical midline mass in 39 patients (68.5%) and purulent drainage in 16 patients (28%). In DC, only one of the cases had erythema and all of the others presented with cervical midline mass. Nine of the patients were admitted to our department with recurrence. The history of infection was seen in 40.4% of the patients in TCC and none of DC cases had infection. Five (8.8%) of the infected cases underwent abscess drainage with antibiotherapy. The most common diagnostic method was neck ultrasonography (USG) and performed in 45.6% of patients with TCC and 46.2% of cases with DC. Five of the patients also underwent thyroid scintigraphy. Corpus of hyoid bone was excised in all cases and no operative complication was detected. The most common epithelia were ciliated epithelium (35.1%) in TCC and squamous (53.8%) in DC in histopathologic evaluations. Inflammatory cells were encountered in 77.2% of TCC and 30.8% in DC. 34% of cases showed thyroid follicle cells in TCC whereas there was no information about rest of patients. None of the cases showed malignant findings in histopathological analysis. The mean follow-up period was 9.2  5.08 years in TCC and 9.6  5.17 years in DC and three of the cases were reoperated because of recurrence.
Conclusion: Most of the midline cervical lesions are TCC in children and commonly seen in boys. TCC can be infected in contrast to DC. Since most of the lesions consists thyroid follicles, surgical excision is mandatory for differential diagnosis and for definitive treatment.

Çocuklarda boyun orta çizgi lezyonlarının klinik ve histopatolojik özellikleri

Amaç: Çocuklarda boyun orta çizgi lezyonlarının klinik ve histopatolojik özelliklerini incelemek üzere geriye dönük bir çalışma yapılmıştır.
Yöntem: Boyun orta çizgi lezyonu nedeniyle son 15 yılda cerrahi sağaltım yapılan olgular yaş, cinsiyet, klinik ve tanı özellikleri, cerrahi sağaltım sonuçları ve histopatolojik özellikleri bakımından incelenmiştir.
Bulgular: Çalışmaya cerrahi eksizyon yapılan 70 olgu alınmıştır. Olguların 57’si tiroglossal kanal kisti (TKK), 13’ü dermoid kist (DK) tanısı almıştır. Erkek-kız oranı TKK’lerinde33:24, DK’de 8:5 olup yaş ortalaması sırası ile 4,49  2,7 ve 6,4  4,8 yıldır. TKK’de en sık başvuru yakınması boyun orta çizgi üzerinde şişlik (n=39, %68,5) ve pürülan akıntıdır (n=16, %28). DK’de yalnız bir olguda boyun orta çizgide kızarıklık yakınması olup (%7,7), tüm olgular şişlik yakınması ile başvurmuştur. Dokuz olgu başka bir merkezde ameliyat edilmiş ve yineleme nedeniyle başvuran olgulardır. TKK’de infeksiyon öyküsü %40,4’ken, DK’lerde infeksiyon öyküsü yoktur. İnfekte olan olguların 5’inde (%8,8) antibiyoterapi ile birlikte apse boşaltılması yapılmıştır. En sık kullanılan tanı yöntemi boyun ultrasonografisi olup, TKKli olguların %45,6’sında, DK’li olguların %46,2’sinde yapılmıştır. TKK olan olguların 5’inde (%8,8) tiroid sintigrafisi de yapılmıştır. Tüm olgularda hiyoid korpusu çıkarılmış ve hiçbir olguda cerrahi karmaşa gelişmemiştir. Histopatolojik incelemede en sık görülen kist epiteli silyalı epiteldir (%35,1). DK’lerin %53,8’inde döşeyici epitel yassı epiteldir. İnflamatuar hücreler TKK’lerinin %77,2’sinde rastlanırken DK’lerin %30,8’inde görülmektedir. TKK’lerinin %35’inde tiroid folikül hücreleri de saptanmıştır. Olguların hiçbirinde malignite bulgusu yoktur. Diğer olgularda tiroide ait hücre varlığı bilinmemektedir. İzlem süresi TKK’de ortalama 9,2 5,08 yıl, DK’de 9,6  5,17 yıl olup 3 olgu yineleme nedeniyle izlem sırasında yeniden opere edilmiştir.

Sonuç: Çocuklarda boyun orta çizgi lezyonlarının önemli bir çoğunluğu TKK’dir ve erkek çocuklarda daha sıktır. TKK’leri DK’den farklı olarak infekte olabilirler. Önemli bir kısmı tiroid folikülü içermesi nedeniyle ayırıcı tanı ve sağaltım için cerrahi eksizyon gereklidir.
