Our Results of Laparoscopy Assisted Percutaneous Repair in Patients with Inguinal Hernia

Aim: Inguinal hernia repair which is the most common surgical pathology in children can be performed with minimally invasive surgery. In this study, we aimed to evaluate results of our laparoscopy assisted percutaneous repair(LAPR) experiences in the treatment inguinal hernia.

Materials and Methods: Between september 2015- March 2016, file records of patients who were treated with LAPR method were evaluated. Under general anesthesia, a 5mm optic port was placed through the umbilicus and internal ring was closed with 2/0 prolene suture  using 16G anjiocath

Results: A total of 23 patients (17 male, 6 female) had 25 hernia repairs. The average age was 65,7±44,9 (10-168) months. Patients presented with hernia on right in 17, left in 5 and bilaterally in 1 of patients. One of the  patients was  recurrence . Three of the patients who underwent previously open surgery with a diagnosis of inguinal hernia from the other side were treated. Two patients were underwent LAPR after reduction of incarcerated hernia. One patient was treated umblical hernia simultaneous. Contralateral patent processus vaginalis was observed in 2 of patients and repaired in the same session. The average operative time was 14,9±8,5 minutes. In 2 patients peroperative complications which are minimal retroperitoneal leakage were observed. Postoperative complication was observed in one patient who developed suture reaction in fifth month and recurrance was seen in one patient.

Conclusion: LAPR is an alternative, effective procedure. Visualization of the peritoneal cavity allows for detection of other abnormalities, as well as for performing other procedures during the same session (such as closing the contralateral inguinal canal or umbilical hernia surgery).

İnguinal Herni Tanılı Hastalarda Uygulanan Laparoskopi Yardımlı Perkutan Onarım Sonuçlarımız

Amaç: Çocuklarda en sık cerrahi patoloji olan inguinal herni onarımı minimal invazif girişimlerle yapılabilmektedir. Bu çalışmada inguinal herni tanısyla laparoskopi yardımlı perkutan onarım uygulanan hastaların sonuçlarının değerlendirilmesi amaçlanmıştır.  

Gereç ve Yöntem: Eylül 2015- Mart 2016 tarihleri arasında inguinal herni tanısyla laparoskopi yardımlı ameliyat edilen hastaların dosya bilgileri incelendi. Tüm hastalara, genel anestezi altında, umbilikustan  5 mm optik port yerleştirildi. internal ring perkütan olarak 16G anjiokat kullanarak 2/0 prolen sütür ile kapatıldı.

Bulgular: Toplam 23 hastaya  (17 erkek, 6 kız) 25 herni onarımı yapıldı. Yaş ortalaması 65,7±44,9 (10-168) aydı. inguinal patolojinin 17 sağda, 5 solda ve 1 tane bilateral olduğu izlendi. Hastalardan 1 tanesi nüks, 3 tanesi daha önce diğer taraftan açık hernioplasi uygulanan hastalardı. 2 hastaya inkarsere herni redüksiyon sonrası laparoskopi yardımlı perkutan onarım uygulandı. 1 hastaya eş zamanlı umblikal herni onarımı yapıldı. Ortalama operasyon süresi: 14,9±8,5 dakikaydı. Yatış süresi 9,6 saatti. 2 hastada ameliyat sırasında minimal retroperitoneal sızıntı  oldu. Ameliyat sonrası 1 hastada 5.ay sütür reaksiyonu gelişti. Bir hastada nüks görüldü.

Sonuç: Laparoskopi yardımlı perkutan herni onarımı aternatif ve etkili bir yöntemdir. Karın içi boşluğun görüntülenmesi diğer anomalilerin saptanmasına olanak ve aynı seansta diğer cerrahi yöntemlerin uygulanması olanak sağlar ( örneğin diğer taraf inguinal kanalın kapatılması ve umblikal herni onarımı).
