A rare cause of duodenal obstruction: Heterotaxy syndrome

Aim: Situs ambiguous or heterotaxy syndrome describe as an abnormal positioning of abdominal internal organs. Situs ambiguous is traditionally divided into two general categories: left isomerism (situs ambiguous with polysplenia) and right isomerism (situs ambiguous with asplenia). Herein we present a case of situs ambiguous with intestinal malrotation and a duodenal obstruction.

Case: A newborn female, born at 38 weeks gestation via cesarean section underwent fetal echocardiography in 31.week and diagnosed as left heterotoxy syndrome and persistent left superior vena cava. The liver was found to be localized in midline by abdominal ultrasonography. Echocardiography showed ASD and persistant left superior vena cava.  Patient underwent abdominal radiographs at 12 hours of life because of respiratory distress that showed a reverse "double bubble" sign, with the stomach on the right and the liver on the midline. Gastrointestinal barium graphy showed stomach settled on the right and had a malrotation. Barium meal study revealed duodenal obstruction. An exploratory laparotomy was performed to further assess the cause of the proximal gastrointestinal obstruction. Intraoperatively, the duodenum was found to be toward the left and the stomach was seen a mirror-image configuration on the right. A duodenal obstruction by preduodenal portal ven was detected. The small bowel was located within the right side of the abdomen, and the colon in the left hemi-abdomen, consistent with intestinal non-rotation. The spleen was identified in the left upper quadrant. There was no polisplenia. Duodenoduodenostomi and Ladd procedure were performed to relief the obstruction. The appendix was removed, because of its atypical location in the left lower quadrant. Upper gastrointestinal meal study was revealed normal anastomotic passage on seventh postoperative day and feeding was performed. He was discharged on postoperative fourteenth day. She is following up till.

Conclusion: A thorough imaging survey is essential for patients to have their unique anatomy classified. Multidisciplinary and meticulous approach is required to identify all abnormalities and determinate the appropriate treatment strategies.

Amaç: Situs ambigous veya heterotaksi sendromu, iç organların anormal konumlanması şeklinde tanımlanır. Situs ambigious sol (polispleni ile situs ambigious) ve sağ (aspleni ile situs ambigious) izomerizm olmak üzere iki şekilde görülür.  Duodenal obstrüksiyon ve malrotasyon tablosu ile belirti veren sol izomerizm olgusunu sunulmuştur.

Olgu: 38 haftalık, sezeryan ile 3200gr ağırlığında doğan yenidoğan hasta, antenatal dönemde 25.haftadan itibaren sol izomerism nedeniyle takip edilmekteydi. 31. haftada yapılan fetal ekokardiografide (EKO) heterotaksi sendromu ile birlikte persistan sol superior vena cava saptanmış. Postnatal dönemde yapılan ultrasonografide karaciğerin orta hat yerleşimli olduğu görüldü. EKO’da ASD ve peristan sol süperior venakava tesbit edildi. Solunum sıkıntısı nedeni ile çekilen direk grafide batında distale gaz geçişinin görülmemesi nedeni ile intestinal obstruksiyon ön tanısı ile bölümümüze danışıldı. Üst pasaj grafisi çekildi. Midenin orta hattın sağında yerleştiği ve malrotasyon olduğu görüldü. Duodenum da buna bağlı olarak yer değiştirmişti. Jejunuma opak maddenin geçişi olmadı. Hastaya laparotomi yapıldı. Eksplorasyonda midenin ayna görüntüsü halinde sağ yerleşimli, duodenumun 2. kısmında preduodenal portal venin obstrüksüyona neden olduğu görüldü. İnce bağırsaklar karın boşluğunun sağ tarafında kolon ise sol tarafta yerleşmişti. Dalak sol üst kadranda görüldü. Polisplenisi yoktu. Ladd prosedürü uygulandı. Duodenum 1. segment ile 3. segment arasında duodeno-duodenostomi yapıldı. Appendiksin sol alt kadran yerleşimli olması nedeni ile appendektomi yapıldı. Postop takibinde 7. gün mide-duodenum grafisi çekildi. Anastomozun açık olduğu görüldü. Oral beslenme başlandı. Postop 14. gün taburcu edildi. Hastanın takibi devam etmektedir.

Sonuç: Heterotaksi olgularında her hastanın kendine özgü anatomik sınıflaması olduğu için ayrıntılı bir görüntüleme araştırması esastır. Tüm anormallikleri tanımlamak ve uygun tedavi yaklaşımının belirlenmesinde multidisipliner hassas bir yaklaşım gereklidir. Sol izomerizm olgularında polispleni temel unsurlardan biri olarak kabul edilmekteyken, olgumuzda bu bulguya rastlanmaması her olguda farklı tablolar ile karşılaşılabileceğinin göstergesidir.
