Introduction:

Possible complications of gastrostomy include the abdominal wall of the stomach decomposition of gastrostomy, peritonitis and to get out of the head mushrooms in the stomach. Herein we present a rare complication of gastrostomy patients causing acute pancreatitis.

Case:

2.5 -year-old girl with hypoxic-ischemic encephalopathy, mental motor retardation, epilepsy and gastrostomy with feeding catheters in pediatric intensive care services admitted to our department with acute pancreatitis. At abdominal MRI in type 1 bile duct cysts and a nother cyst had found in duodenum. When we have done detailed examination of the patient and MR images is analyzed by us plenty of food given to the gastrostomy feeding tube and the balloon portion of the scroll until the end of the duodenum of kateter there was found to be causing obstruction . The gastrostomy kateter was renamed and the patient showed reduction in infective table very quickly. Incoming condition for the continuation of improved patient care was transferred to the pediatric service.

Discussion:

Gastrostomy, but for various reasons it can not be fed orally gastrointestinal function is to place the feeding tube directly into the stomach with the aim of providing support to patients with normal nutrition . Wound infection , infection periostomal , periostomal leakage, bleeding , ulceration, tube obstruction, gastric outlet obstruction, is known among burried bumper or kolokütanöz fistula are the complications. But the fact of changing gastrostomy tube, as in our patient balloon accidentally given food and thus within the scroll up balloon duodenum filled with food sfinter front obtruction and cause acute pancreatitis is really a complication that can be very rare. So percutaneous endoscopic gastrostomy or gastrostomy cases, complications such as acute pancreatitis may develop in patients with cholestasis or catheter should be considered .

Giriş:

Gastrostominin olası komplikasyonlar arasında midenin karın duvarından ayrışması, peritonit, mide içindeki mantar başın dışarıya çıkması gibi durumlar yer almaktadır. Burda akut pankreatite neden olan nadir bir gastrostomi komplikasyonlu hastayı sunduk.

Olgu:

2,5 yaşında kız hasta hipoksik iskemik ensefalopati, mental motor retardasyon, epilepsi ve gastrostomi beslenme kateteri ile pediatri yoğunbakım servisinde izlenen hasta bölümümüze akut pankreatit ve çekilen abdominal MR’da tip 1 koledok kisti ve düodenumda 2. bir kistik lezyon nedeni ile danışıldı. Hastanın ayrıntılı muayenesi ve MR görüntüleri tarafımızdan incelendiğinde gastrostomi beslenme sondasının balon kısmına bol miktarda mama verilediği ve katetrin ucunun düodenuma kadar ilerleyip orada obstruksiyona neden olduğu anlaşıldı. Hastanın gastrostomi katetri değiştildi ve takiben hastanın enfektif tablosunun çok hızlı bir şekilde gerilediği görüldü. Gelen durumu düzelen hasta pediatri servisine devredildi.   

Tartışma:

Gastrostomi, çeşitli nedenlerle oral yoldan beslenemeyen ancak gastrointestinal fonksiyonları normal olan hastalara nutrisyon desteği sağlamak amacı ile mideye direk olarak beslenme tüpü yerleştirilmesidir. Yara yeri enfeksiyonu, periostomal enfeksiyon, periostomal sızıntı, kanama, ülserasyon, tüp tıkanıklığı, gastrik çıkış darlığı, burried bumper ya da kolokütanöz fistül bilinen komplikasyonları arasındadır. Fakat hastamızda olduğu gibi değişen gastrostomi tüpünün balonuna yanlışlıkla verilen mama ve böylelikle içerisi mama ile dolu olan balonun düodenuma kadar ilerleyip sfinterin önünde obtruksiyona ve akut pankreatite neden olması gerçekten çok nadir olabilecek bir komplikasyondur. Bu yüzden perkütan endoskopik gastrostomi ya da gastrostomi kateteri olan hastalarda gelişebilecek akut kolestaz ya da pankreatit gibi durumlarda gastrostomi komplikasyonu akla getirilmelidir. n hastalarda gelişebilecek akut kolestaz ya da pankreatit gibi durumlarda gastrostomi komplikasyonu akla getirilmelidir.
