Two different methods in pilonidal sinus treatment: Surgical excision and phenol injection

Introduction: Primary surgical excision is the treatment of pilonidal sinus, however phenol injection is getting popular nowadays, due to its minimal invasive manner.

 Material and methods: A retrospective review of medical charts of patients operated either surgically or by phenol injection for pilonidal sinus between the years of 2011 and 2016 was included in this study. Recurrence rates and the pleasure of the patients were recorded.

Results: Of the 44 patients; 28 were male (63%), 16 were female (37%) with the average age of 14.4±1.5 years. The mean age was 14.4±1.5 years. 12 patients (27%) treated with phenol injection and surgical treatment was performed in 32(73%).  Phenol injections were performed under local anesthesia by applying 2-5 ml of 80% phenol solution after a incision about 1 cm and removing debris and hair inside the sinuses. There was no complication except mild hyperemia around the sinus. The recurrence rate was 6.5% (n:2) in surgical group and 41% (n:5) in phenol group. All patients cured after second phenol injection.

Conclusion: Phenol injection is minimal invasive method for treating pilonidal sinus and is getting popular nowadays. Although the advantage of easy application and low complication rates of phenol injection that leads to patient comfort and satisfaction; the recurrence rates found to be high in first injections.  

Pilonidal Sinüs Tedavisinde İki Farklı Yöntem: Cerrahi Eksizyon Ve Fenol Tedavisi

Giriş: Pilonidal sinüs tedavisinde primer cerrahi eksizyon ile günümüzde minimal invaziv olduğu düşünülen fenol tedavisi uygulanmaktadır.

 Materyal ve Metot:  Çalışmada 2011-2016 yılları arasında kliniğimizde pilonidal sinüs nedeni ile cerrahi eksizyon veya fenol tedavisi alan çocuklarda nüks oranları, hasta memnuniyeti geriye yönelik incelendi.

Bulgular: Toplam 44 çocuğun verileri geriye dönük olarak incelendi.  Çocukların 28’i erkek (%63) 16’sı kızdı (%37). Hastaların yaş ortalaması 14.4±1.5 yıl idi. Fenol tedavisi 12 çocuğa (%27), cerrahi eksizyon 32 çocuğa (%73) uygulandı. Fenol tedavisinde işlem; lokal anestezi altında sinüs ağzı 1 cm insize edilerek içindeki kıllar ve debris temizlendikten sonra 2-5 ml, %80 fenol solüsyonu sinüs içerisine verilerek yapıldı. Sinüs çevresinde hafif hiperemi dışında komplikasyon görülmedi. Cerrahi eksizyon yapılan hastalarda nüks oranı %6.5 (2 hasta) iken, fenol tedavisinde nüks oranı %41 (5 hasta) görüldü ve ikinci uygulama sonrasında kür sağlandı.

Sonuç: Pilonidal sinüste fenol tedavisi; minimal invaziv olması nedeni ile kullanımı yaygınlaşan bir yöntemdir. Fenol tedavisinde; işlemin kolaylığı ve düşük komplikasyon oranları ile sağlanan hasta konforu ve memnuniyeti gibi üstünlükler olmakla birlikte ilk uygulama sonrası nüks oranları yüksek bulunmuştur.
