MANAGEMENT OF THE CONGENITAL POUCH COLON WITH TUBULARIZED COLOPLASTY METHOD

Background and Aim: Congenital pouch colon in the anorectal malformations presents with highly variable and anomalous anatomy. Where the colon is replaced by/or terminates into a large aperistaltic intestinal pouch and as same as short bowel syndromes. Herein two cases of congenital pouch colon treated with tubularized coloplasty were presented.

Case Reports: Tip I pouch colon was noticed in exploration and stoma was done 5cm proximal to pouch due to high anorectal malformation in a male newborn. In 15 months old; colovesical fistulous communication was catheterizated with cystoscopy; and a 20cm’s new colon from pouch colon considering the vascular structure was obtained with a lineer stapler after fistula ligation and new colon pulled through endoscopically to the perineum. Three months later ileostomy was closed, gastrointestinal passage was normal in 6 month of follows up.

Tip I pouch colon was noticed in exploration, and ileostomy were done 5cm proximal to pouch due to persistent cloaca in a female newborn. She suffered from short bowel syndrome, salt loosing enteropathy and severe malabsorbsion until three years old. In 3 years old; the colocloacal fistulous communication was catheterizated by cystoscopy; urethra and double vagina and rectal fistula were mobilised posterosagittaly; in same session, severe intestinal adhesions were excised. A new colon from pouch colon considering the vascular structure was tubularised with lineer stapler by abdominal incision; thus aproximately 25cm’s a new colon was obtained. Colon pull-through and vaginoplasty and urethroplasty were completed with posterosagittally. Histopathology of pouch biopsy showed normal ganglion cells in both patients.

Conclusion: Preoperative assessment included clinical and radiological studies, examination under anesthesia, genitourinary endoscopy, and fistula catheterisation is very important in the selection of the definitive treatment method for pouch colon. Tubularised coloplasty considering the vascular structure of pouch colon is safe and suitable method in selective cases with pouch colon.

TUBULARİZE KOLOPLASTİ YÖNTEMİYLE KONJENİTAL POŞ KOLONUN TEDAVİSİ

Giriş ve Amaç: Anorektal malformasyonlarda konjenital poş kolon, kolonun geniş, aperistaltik bir intestinal poş ile devam ettiği ya da sonlandığı kısa barsak sendromu benzeri bir durum olan oldukça değişken ve anormal bir anatomik varyasyondur. Burada, tübülarize koloplasti yöntemi ile tedavi edilen iki poş kolonlu olgu sunuldu.

Olgu Sunumları: Yüksek tip anal atrezili bir erkek yenidoğanda yapılan eksplorasyonda Tip I poş kolon nedeniyle poş kolonun 5 cm proksimalinden stoma oluşturuldu. 15 aylıkken sistoskopi ile kolovezikal fistül mesaneden kateterize edildi ve sonrasında yapılan abdominal insizyonla fistül ligate edildi, poş kolonun vasküler yapıları dikkate alınarak lineer stapler yardımıyla 20 cm’lik yeni kolon oluşturuldu ve oluşturulan yeni kolon perineye endoskopik olarak çekildi. 3 ay sonra ileostomi kapatıldı. 6 aylık takibinde gastrointestinal pasaj tamdır.

Persistan kloakalı kız yenidoğanda Tip I poş kolon nedeniyle poşun 5 cm proksimalinden stoma oluşturuldu. Hasta 3 yaşına kadar kısa barsak sendromu nedeniyle su tuz kaybettiren enteropati ve ciddi malabsorbsiyon probleminden muzdarip oldu. 3 yaşında iken önce sistoskopi ile kolokloakal fistül kateterize edildi; posterosagittal yaklaşım ile üretra, double vajina ve rektal fistül mobilize edildi; aynı seansta abdominal insizyon ile ince barsaklardaki bridler ayrıldı; kolokloakal fistül ligasyonu sonrası, poş kolonun vasküler yapıları dikkate alınarak poş kolondan lineer stapler yardımıyla yaklaşık 25 cm’lik yeni kolon tübülarize edildi. Kolon pull-through, vajinoplasti ve üretroplasti işlemleri posterosagittal yaklaşımla tamamlandı, batın kapatıldı. Postoperatif ikinci ay stoma kapatılması planlandı.  Her iki hastanın poş kolon biyopsileri histopatolojik değerlendirmesinde gangliyon hücresi mevcuttu.

Sonuç: Klinik değerlendirme ve radyolojik görüntüleme, anestezi altında muayene, alt genitoüriner sistem endoskopisi, üriner fistül kateterizasyonunu içeren preoperatif değerlendirme konjenital poş kolon olgularının definitif tedavi yöntemi seçiminde önemlidir.  Konjenital poş kolonlu seçilmiş vakalarda poş kolonun vasküler yapıları dikkate alınarak yapılan tübülarize koloplasti güvenli ve uygun bir yöntemdir.
