An omphalopagus case

INTRODUCTION: Conjugated twins are anomalies in which two monozygotic, monoamniotic and monochorionic fetuses are not fully separated. It is associated with severe mortality and morbidity. Conjugated twins are classified as thoracopagus (thorax), omphalophagus (abdomen), pigopagus (sacrum), ischiopagus (pelvis), craniopagus (head), cephalopagus (face) or archipedes (back) according to their junction regions.

 

CASE: Conjoined twins born in Syria who were not prenatal followers were brought to an emergency. The heads of the babies were separate, three arms were present, torakoomfalopagus was present. Externally was girl baby, rectovestibular fistula with anal atresia was present. Emergency operation was not considered from fistula it observed that stool output was from fistula. In Computed Tomography, it was seen that the thoracic organs and diaphragms of the infants were separated, the two liver were fused to the midline, but the other internal organs were single. It was observed that the abdominal aorta was double and connected at the iliac level. The spinal canal was seen from the cervical to sacral level in duplicate. There was only one ventricle and one atrium in the right baby in the ECO that was performed.

CONCLUSION: If conjoined twins can be diagnosed early by prenatal follow-up, options that include continuing gestation, gestational termination, or only fetocide with postnatal surgery will provide better family counseling. Despite advances in radiological imaging, anesthesia, and surgical treatment techniques, morbidity and mortality rates are high in conjoined twins. Determination of the location and extent of the adhesion region, shared organs and major congenital anomaly is important for the prognosis when treatment of live conjoined twins is planned. Only about 40-60% of conjoined twins survive, and about 35% of them die in the first 24 hours. Management options including continuation of gestation, gestational termination, or only fetocide with postnatal surgery can be better advised by the family if early detection is possible with prenatal follow-up of conjoined twins . The detailed anatomic information obtained may contribute to the informing of the family while shedding light on the surgical planning of the newborn.

GİRİŞ: Yapışık ikizler, monozigotik, monoamniyotik ve monokoryonik iki fetusun tam olarak ayrılmadığı bir anomalidir. Ağır mortalite ve morbidite ile seyreder. Yapışık ikizler birleşme bölgelerine göre, torakopagus (toraks), omfalopagus (abdomen), pigopagus (sakrum), iskiopagus (pelvis), kranyopagus (kafa), sefalopagus (yüz) veya rakipagus (sırt) olarak sınıflandırılırlar.

OLGU: Suriye'de doğmuş  prenatal takibi olmayan yapışık ikizler acile getirildi.  bebeklerin kafaları ayrı, üç kolu mevcut,  torakoomfalopagus mevcut idi.  haricen kızdı, rektovestibüler fistüllü anal atrezisi mevcut idi Acilde çekilmiş olan bt'de  bebeklerin  torakal organlarının ve diyaframlarının ayrı olduğu, iki ayrı karaciğerin orta hatta füzyone olduğu ancak diğer batın içi organların tek olduğu izlendi.  Abdominal aortanın çift olduğu ve iliak düzeyde birleştiği izlendi. Spinal kanal servikalden sakral düzeye kadar çift olarak izlendi.Yapılan eko'da sağ bebekte tek ventrikül ve tek atrium vardı.

SONUÇ: Yapışık ikizlerin  prenatal takipleri ile erken tanısı konulabilirse, postnatal cerrahisi ile gebeliğin devamı, gebeliğin terminasyonu veya yalnızca fetosit uygulamasını içeren seçenekler aileye daha iyi danışmanlık verilmesini sağlar. Radyolojik görüntüleme, anestezi ve cerrahi tedavi tekniklerindeki ilerlemelere rağmen yapışık ikizlerde morbidite ve mortalite oranları yüksektir. Canlı olarak doğan yapışık ikizlerin tedavisi planlanırken yapışıklık bölgesinin yeri ve genişliğinin, paylaşılan organlar ve major konjenital anomali varlığının belirlenmesi prognoz açısından önemlidir. Yapışık ikizlerin ancak %40-60’ı canlı doğar ve bunların yaklaşık %35’i ilk 24 saatte kaybedilir Yapışık ikizlerin  prenatal takipleri ile erken tanısı konulabilirse, posnatal cerrahisi ile gebeliğin devamı, gebeliğin terminasyonu veya yalnızca fetosit uygulamasını içeren yönetim opsiyonları aileye daha iyi danışmanlık verilmesini sağlar. Elde edilen ayrıntılı anatomik bilgi, yeni doğanın cerrahi planlamasına ışık tutarken ailenin de bilgilendirilmesine katkıda bulunabilir.
