Intestinal Atresia: 20 Years of Experience of a Reference Hospital

Aim: Intestinal atresia (IA) is the most frequent cause of intestinal obstruction of newborns. The aim of this study is to reveal the factors effective on clinical outcomes of the patients who underwent surgery in a university hospital with the indication of IA.

Methods: Seventy-four neonatal cases with intestinal atresia who underwent surgical treatment between January 1997 and December 2016 were investigated. The cases with esophageal atresia were not included in the study.

Results: The study population consisted of 40 females, and 34 male newborns with a mean age of 6.4 ± 8.3 days at diagnosis. Mean values for birthweight (2.3 ± 0.6 kg), gestational age (35.6 ± 2.8 weeks), maternal age (28.9 ± 6.1 years), hospital stay (24.5 ± 25.3 days) were as indicated. Most frequently duodenal (n: 31, 42%), and least frequently colonic atresia (n: 2, 3%) were seen. The longest, and the shortest mean hospital stays were detected in cases with jejunal (32.8 ± 41.6 days), and colonic atresia (8 ± 0 days), respectively. Survival rates was at the lowest in multiple atresia (81%), and duodenal atresia (81%), while all patients with pyloric, and colonic atresia survived. In 9 of 11 (82%) patients who exited additional congenital anomalies (major cardiac anomalies, n=3; Down syndrome n= 2), congenital biliary atresia (n= 1), and esophageal atresia + tracheo-esophageal fistula + anal atresia + tracheal atresia (n= 1) were detected. First minute Apgar score in 5 newborns (46%) was ≤5 points.

Conclusion: Corrective surgery forms the basis of treatment in intestinal atresia. Although developments in neonatal care, and advancements in surgical techniques markedly decrease mortality rates, concomitant severe congenital anomalies in these patients, effect hospital stay, and mortality rates adversely.

İntestinal Atreziler: Referans Bir Hastanenin 20 Yıllık Deneyimi

Amaç: İntestinal atrezi (İA) yenidoğanlarda intestinal obstrüksiyonun en sık sebebidir. Bu çalışmanın amacı bir üniversite hastanesinde İA nedeniyle cerrahi uygulanan hastaların klinik sonuçları üzerine etkili olan faktörlerin ortaya konulmasıdır.

Yöntem: Ocak 1997 - Aralık 2016 tarihleri arasında cerrahi tedavi uygulanan 74 intestinal atrezili yenidoğan olgu incelendi. Özefagus atrezili olgular çalışmaya dahil edilmedi.

Bulgular: Hastaların 40’ ı kız 34’ ü erkek cinsiyete sahipti ve ortalama tanı yaşı 6.4 ± 8.3 gündü. Ağırlıkları 2.3 ± 0.6 kg, gebelik yaşı 35.6 ± 2.8 hafta, anne yaşı 28.9 ± 6.1 yıl, hastanede kalma süresi ise 24.5 ± 25.3 gün idi. En sık sebep duodenal atrezi (n: 31, %42) iken en az kolon atrezisi (n: 2, %3) idi. En uzun hastanede kalma süresi jejunal atrezili olgularda (32.8 ± 41.6 gün), en kısa ise kolon atrezisinde (8 ± 0 gün) tespit edildi. Hayatta kalma oranları en düşük multipl atrezi (%80) ve duodenal atrezide (%81) iken pilor ve kolon atrezilerinin tamamı yaşadı. Eksitus olan 11 hastanın 9’unda (%82) ek konjenital anomaliler (majör kardiyak anomali (n: 3), Down sendromu (n: 2), konjenital biliyer atrezi (n: 1) ve özefagus atrezisi + trakeoözefageal fistül + anal atrezi + trakeal atrezi (n: 1)) vardı ve 5’inde (%46) birinci dakika Apgar skoru ≤5 idi.

Sonuç: İntestinal atrezide cerrahi düzeltme tedavide esastır. Yenidoğan bakımındaki gelişmeler ve cerrahi tekniklerdeki ilerlemeler mortaliteyi belirgin azaltsa da, bu hastalarda eşlik eden şiddetli konjenital anomaliler hastanede yatış süresi ve mortaliteyi olumsuz etkilemektedir.
