PYLORODUODEONAL INTUSSUSEPTION: REPORT OF A CASE

Aim: Gastrointestinal polyps are common cause of intussusception in children. Pyloroduodenal intussusception (PDI) due to pyloric polyps are very rare and have not been reported. A 3 year-old boy with PDI due to pyloric polyps is presented to discuss the diagnosis and treatment of PDI.

Case Report: A 3 year-old boy with bone marrow transplantation due to severe combined immune and adenosine deaminase deficiency and hypoalbunemia is admitted with non-billous vomiting and abdominal distention. In his previous history, he had multiple gastric, duodenal and colonic polyps. His vital signs were normal and physical examination revealed epigastric distention and palpable mass. The hemoglobin level was 9.8 mg/dl and thrombocytes were 351x103μL. Blood electrolytes and renal function tests were normal except decreased albumin levels. Abdominal x-ray showed air-fluid levels at stomach and ultrasonography showed 5 cm intussusceptum at right upper quadrant (Figure 1a). Upper GI study showed gastric outlet obstruction (Figure 1b). Endoscopy showed multiple polyps at stomach and pylorus was totally obstructed with polyps that it was not possible to pass the pylorus. In surgical exploration, multiple polyps originating from pylorus passing to duodenum and causing PDI (Figure 2 a). Duodenotomy from anterior wall was performed and polyps were excised to reduce the intussusception (Figure 2b). After excision of polys, the passage is obtained and no need to perform a gastroduodenostomy. The postoperative period was uneventful and the histopathologic evaluation revealed juvenile polyps.

Conclusion: Polyps at pylorus or gastric antrum may pass through duodenum and cause PDI. PDI is very rare type of intussusception and cause gastric outlet obstruction. Upper GI studies and endoscopy is useful in the diagnosis. However, surgical excision of polyps and obtaining gastrointestinal passage is first line of treatment, gastroduodenostomy should be kept in mind in patients if the total excision of polyps is not possible.  

 

PİLORODUODENAL İNVAJİNASYON: OLGU SUNUMU

Amaç: Gastrointestinal polipler çocuklarda sık görülen invajinasyon nedenleri arasında yer almaktadır. Pilordan köken alan poliplere bağlı gelişen pilorduodenal invajinasyon (PDI) çok nadirdir ve daha önce rapor edilmemiştir. Üç yaşında erkek hasta pilor poliplerien bağlı gelişen PDI tanı ve tedavisi tartışılmak üzere sunulmuştur.

Olgu Sunumu: Ciddi combine immune yetmezlik, adenozin deaminaz eksikliği ve hipoalbunemi nedeniyle kemik iliği transplantasyonu yapılan 3 yaşında erkek hasta safrasız kusma ve karın şişiliği şikayeti ile başvurudu. Daha önceki takiplerinde mide, duodenum ve kolonda yaygın polipleri olan hastanın başvuru sırasında vital bulguları doğaldı. Fizik incelemede epigastrik bölgede distansiyon ve ele gelen kitlesi mevcut olan hastanın tam kan sayımında hemoglobin 9.8 mg/dl, trombosit sayısı 351 x103 μL idi. Albumin düşüklüğü dışında kan biyokimyası ve böbrek fonksiyon testleri normaldi. Ayakta direk karın grafisinde midede seviye veren gaz gölgesi olması sonucu yapılan USG ‘de sağ üst kadranda pilor ve duodenuma uyan bölgede 5 cm’lik invajine segment izlendi (Şekil 1a). Üst GIS grafisinde mide çıkımında tıkanıklık saptandı (Şekil 1b). Endoskopide mide de bir kaç adet polip olduğu, pilorun ise poliplerle tam tıkalığı olduğu görüldü ve pilordan geçilemedi. Cerrahi eksplorasyonada pilordan köken alan çok sayıda polipin duodenuma doğru invajine olarak PDİ neden olduğu görüldü (Şekil 2a). Duodenum ön yüzünden yapılan duodenotomide pilorun çıkışı kapatan çok sayıdakki polip eksize edilerek invajie segment redükte edildi (Şekil 2b). Polip eksizyonu sonrası pasaj sağlandığından gastroduodenostomi gereksinim duyulmaksızın işleme son verildi. Postoperatif dönemde sorunu olmayan hastanın yapılan histopatolojik incelemesinde poliplerin juvenil polipozis ile uyumlu olduğu görüldü.

Sonuç: Pilor veya mide antrumu kaynaklı polipler duodenuma doğru ilerleyerek invajinasyona neden olabilirler. PDI oldukça nadir bir invajinasyon yerkleşimi olup mide çıkım tıkanıklığı ile bulgu verebilir. Tanıda üst GIS kontrastlı grafiler ve endoskopiden faydalanılmalıdır. Cerrahi polip eksizyonu ve pasajın sağlanması öncelikli tedavi seçeneği olmakla birlikte polip eksizyonunun tam yapılamadığı olgularda gastroduodenostomi akılda tutulmalıdır.
