An unpredictable cause of acute scrotum

The undescended testis is detected in 0.8-1.2% of the boys aged 1 year and it is one of the most common operations performed almost daily in the pediatric surgery practice. Here we present a case which was misdiagnosed and treated as undescended testis and scrotalization of ovotestis was performed in a patient with disorder of sexual development.

A 17-year-1-month-old male patient was admitted to our endocrinology department with a complaint of gynecomastia for 4 years. During the follow up the patient was consulted due to the onset of pain in the left scrotum. It was learned from his past medical history that he had surgeries for bilateral undescended testis and hypospadias elsewhere. An urgent surgery for acute scrotum was performed. Hemorrhagic-necrotic areas on the gonad were detected, necrotic areas were cleared and biopsy was taken. The histopathological examination revealed as of the patient was reported as an hemorrhagic corpus luteum cyst and atrophic ovarian tissue. The chromosome analysis turned out to be 46 XX. In the suprapubic ultrasonography; uterus and a tubular structure compatible with vagina was detected. Total abdominal hysterectomy and bilateral gonadectomy were performed, after the decision to continue in male sex due to the age in the evaluation of the ethics committee for gender determination. In the histopathologic evaluation of the gonads, it was found that the right and left gonads were over, and the right ovary contained a microscopic irregular seminiferous tubule of the testis. The patient was diagnosed as “Ovotesticular Disorder of Sexual Development (OT-DSD)”.

Ovotestis can be defined as the gonad containing both ovarian follicles and testicular tubular elements. If ambiguous genitalia are detected at birth in these patients, after the required investigations the diagnosis can be made and treatment can be planned in the most appropriate manner. Disorder of Sexual Development should be kept in mind in patients with bilateral nonpalpabl testis, or, in patients with unilateral nonpalpabl testis and hypospadias together. It is very important to make an endocrinologic evaluation before planning any surgery for these patients.

 

Akut skrotumun tahmin edilemez nedeni

İnmemiş testis 1 yaşındaki erkek çocukların % 0.8-1.2’sinde saptanan, çocuk cerrahisi ameliyathanelerinde hemen her gün yapılan olmazsa olmaz ameliyatlardandır. Burada cinsiyet gelişim kusuru olan ve yanlışlıkla testis olduğu düşünülerek ovotestisin skrotuma indirildiği bir olgu sunulmuştur.

17 yaş 1 aylık erkek hasta, 4 yıldır devam eden memelerde büyüme şikayetiyle başvurduğu Endokrin bölümümüzce tetkik edilirken sol yumurtasında ağrı başlaması üzerine tarafımıza danışıldı. Hastanın öyküsünden bir dış merkezde iki taraflı inmemiş testis ve hipospadias nedeniyle ameliyatlarının yapıldığı öğrenildi. Sol akut skrotum nedeniyle yapılan eksplorasyonda, gonad içerisinde yer yer kanamalı-nekrotik alanlar saptandı, nekrotik alanlar temizlenip biyopsi alındı. Hastanın patoloji sonucu kanamalı korpus luteum kisti ve atrofik over dokusu olarak raporlandı. Kromozom analizi 46,XX gelen hastanın suprapubik ultrasonografisinde; uterus ve vajenle uyumlu tubuler yapı saptandı. Cinsiyet belirleme etik kurulunda değerlendirildikten sonra yaşı itibariyle erkek cinsiyette devam etmesine karar verilen hastaya total abdominal histerektomi ve bilateral gonadektomi yapıldı. Ganadların patolojik değerlendirimesi sonucunda, sağ ve sol gonadın over olduğu, sağda mikroskopik bir alanda testise ait düzensiz seminifer tübül içerdiği saptandı. Hastaya ovotestiküler cinsiyet gelişim kusuru (OT-DSD) tanısı koyuldu.

Ovotestis, gonadın hem ovariyan foliküller hem de testisküler tübüler elemanlar içermesi olarak tanımlanabilir. Bu hastalarda doğumda kuşkulu genitalya saptanırsa, gerekli araştırmaların ardından tanı koyularak en uygun şekilde tedavi planlanabilir. İki taraflı nonpalpabl testis veya tek taraflı nonpalpabl testis olup hipospadiasın eşlik ettiği olgularda bir cinsiyet gelişim kusuru olabileceği akılda tutulmalıdır. Bu hastalarda herhangi bir cerrahi girişim yapılmadan önce Endokrin değerlendirmenin yapılması çok önemlidir.
