Duplicate appendicitis with cecal duplication: a rare anomaly

Aim:

Coexistence of duplicate appendix and cecal duplication is an extremely rare anomaly of the gastrointestinal tract. The majority of cases become symptomatic as acute abdomen, bowel obstruction or gastrointestinal bleeding and diagnosed before age of 2 years. Here we present a cecal duplication volvulus case after appendectomy.

Case:

A 6 year-old girl presented from an outside facility with abdominal discomfort and bowel obstruction, on the 2nd postoperative day after a double appendectomy for duplex appendisitis. Surgical note mentioned an inverted Y shaped duplicated appendicitis. Physical examination revealed an afebrile patient with tenderness and rebound tenderness in whole abdomen. Her white blood cell count was elevated with neutrophilia. Computed tomography scan of the abdomen showed possible cecal volvulus and ileus. Laparotomy revealed two appendix stumps that one of them  separately located on a cecal duplication at the antimesenteric side of the cecum. During exploration  a dilated, blind-ending, gangrenous bowel loop originated from duplicate cecum was extending from right iliac fossa  to left upper quadrant. A 360° torsion at the cecal opening of the mass was observed. After subtotal resection of the cystic mass completed, mucosal stripping was performed on shared muscle wall between the cyst and cecum. The patient had no postoperative complications was discharged home in stable condition after 3 days in the hospital.

Conclusion:

Appendiceal duplication is extremely rare with an incidence of 0.004% to 0.009% in appendectomy specimens. As our patient, existing of a duplicated appendix should prompt further investigation into the possibility of other congenital anomalies, including duplications of the gastrointestinal tract. Failure to recognise this condition may have serious clinical and medicolegal consequences.

Duplike apandisit ve çekal duplikasyon birlikteliği: nadir bir anomali

Amaç:

Çift apendiks ve çekal duplikasyon birlikteliği, gastrointestinal sistemin (GİS) oldukça nadir bir anomalisidir. Olguların çoğu, akut karın, barsak obstruksiyonu ya da  GİS kanamasıyla semptomatik hale gelir ve 2 yaşından önce tanı alır. Bu olguda apendektomi sonrası çekal duplikasyon volvulusu gelişen bir hastamızı sunuyoruz.

Olgu:

6 yaşında kız hasta, akut apandisit tanısıyla yapılan laparotomide apendiks duplikasyonu saptanıp yapılan apendektomi  sonrası gelişen abdominal hassasiyet ve barsak obstruksiyonu nedeniyle post op 2. günde tarafımıza yönlendirildi. Ameliyat notunda ters Y harfi şeklinde duplike apandisit tarif edilmişti. Ateşi  olmayan hastanın fizik muayenesinde yaygın hassasiyet ve rebound saptandı. Laboratuar tetkiklerinde nötrofil hakimiyeti olan beyaz küre yüksekliği mevcuttu. Bilgisayarlı tomografide olası çekal volvulus ve ileus izlendi. Laparotomi yapıldığında, biri çekumun antimezenterik tarafındaki kistik duplikasyon üzerinde yer alan iki adet apendiks güdüğü görüldü. Eksplorasyonda duplike çekumdan köken alıp sol üst kadrana uzanan dilate, kör sonlanan, gangrenöz barsak segmenti saptandı. Kitlenin çekumla birleştiği seviyede 360° torsiyon mevcuttu. Kitle subtotal rezeke edilip çekumla ortak duvarı üzerindeki mukoza soyulup çıkarıldı. Post operatif komplikasyon gelişmeyen hasta 3. Gününde hastaneden taburcu edildi.

Sonuç:

Apendiks duplikasyonu, apendektomi materyallerinde %0,004 ile %0,009 insidansla rastlanılan oldukça nadir bir anomalidir. Bizim hastamızda olduğu gibi duplike apendiks varlığı, GİS duplikasyonları gibi olası diğer konjenital  anomaliler açısından ileri değerlendirme gerektirir. Bu olguların gözden kaçırılmasının olumsuz klinik ve yasal  sonuçları olabilir.
