Laparoscopic cholecysto-appendico-colonic anastomosis for the treatment of progressive familial intrahepatic cholestasis

Progressive familial intrahepatic cholestasis (PFIC) is a potentially fatal, autosomal recessive liver disease characterized by intermittent attacks of cholestasis and severe pruritus. Surgical biliary diversion methods are useful for resolving pruritus and for the delay of the progressive course. Among many diversion methods, cholecysto-appendico-colonic anastomosis is a newly developed one and its laparoscopic application is not reported yet. We present here a patient with PFIC and laparoscopic cholecysto-appendico-colonic anastomosis.

 

Two year old boy with PFIC was admitted with jaundice, severe pruritis and direct hyperbilirubinemia. A laparoscopic cholecysto-appendico-colonic anastomosis was decided. Three ports were used. The appendix was freed with protection of its vessels and cut off (isolated) from the cecum. Its position was retrocecal and it was carried to the gallbladder behind the ascending colon. The proximal tip is anastomosed to the gallbladder, the distal tip to the tenia of the transvers colon using intracorporeal suturing technics. No complication was encountered. The postoperative course was uneventful. The pruritus was obviously reduced and the direct bilirubin levels were dropped. Sixth month after surgery, pruritus recurred. The appandiceal conduit was obstructed. A cholecysto-ileo-colostomy was added. The child is currently well without any symptoms.

 

Conclusion: Laparoscopic cholecysto-appendico-colonic anastomosis is an alternative feasible, safe and effective surgical method for the resolution of pruritus in patients with PFIC. However, obstruction of the conduit should be kept in mind.

 

Progresif Familial İntrahepatik Kolestaz tedavisinde Laparoskopik Kolesisto-Appendiko-Kolonik Anastomoz

Progresif Familial İntrahepatik Kolestaz(PFIC) potansiyel olarak fatal seyirli, otozomal resesif bir karaciğer hastalığıdır. İntermitan kolestaz atakları ile seyreder ve ciddi kaşıntı yapar. Cerrahi bilier diversiyon yöntemleri bu hastalığın neden olduğu ciddi kaşıntıyı ve hastalığın progresyonunu önleme kapasitesine sahiptir. Tanımlanmış pekçok bilier diversiyon teknikleri arasında Kolesisto-Appendiko-Kolonik Anastomoz tarafımızdan yeni tanımlandı ve şimdiye dek laparoskopik olarak uygulanmamıştı. Burada bir PFIC hastasına uygulanan laparoskopik Kolesisto-Appendiko-Kolonik Anastomoz sunulmaktadır.

Olgu:PFIC ile takip edilen ve ciddi kaşıntısı ve hiperbilirubinemisi 2 yaşındaki erkeğe laparoskopik Kolesisto-Appendiko-Kolonik Anastomoz yapıldı. Üç port tekniği ile apandiks mezosu korunarak serbestleştirildi. Proksimal ucu safra kesesine, distal ucu da transvers kolona, intrakorporeal sütür teknikleri kullanılarak anastomoz yapıldı. Komplikasyon olmadı. Postoperatif dönemde kaşıntı ve hiperbilirubinemi tamamen düzeldi. Ancak 6. ayda kaşıntı nüks etti ve apandiks'in tıkalı olduğu belirlenerek kolesisto-ileo-kolostomi eklendi. Şu anda tamamen normal durumda izlenmektedir.

Sonuç: Laparoskopik Kolesisto-Appendiko-Kolonik Anastomoz, PFIC olgularında alternatif, güvenli ve etkili bir yöntemdir. Ancak tıkanma olabileceği unutulmamalıdır.
