Coexistence of Isolated Esophageal Atresia and Propionic Acidemia: The First Case in the Literature

AIM Propionic acidemia (PA) is a metabolic disease resulting from the deficiency of the propionyl Coa carboxylase (PCC) enzyme at the valine, isoleucine, leucine, methionine, threonine, and mono-chain fatty acids pathways. PA has an incidence of 1/35,000-1/75,000 and is presented with vomiting, dehydration, lethargy, and encephalopathy after starting of feedings, changing of the formula or infection. We present a newborn patient with isolated esophageal atresia (IEA) and PA, that have not reported in the English literature previously.

MATERIAL AND METHOD: The patient was born 2300 gr from a healthy 18 year old mother (G1P1Y1, 36 + 6 weeks) with cesarean section. Postnatal examinations revealed IEA and primary esophageal atresia repair was performed.

FINDINGS: In the postoperative follow-up, the genereral condition of the patient has detoriated and hypotonia occured. The labarotory test revealed metabolic acidosis (pH: 7.09) and high anion deficit, hypoglycemia (50 mg/dL), neutropenia (300/uL), erythropeni (3260000/uL), thrombocytopenia (36000/uL), Gamma Glutamyl Transferase 601 U/L , triglyceride 1085 mg/dL, and ammonia 195 g/dL.When the patient starts to feed without any surgical complication; due to the addition of tonic contractions and seizures, persistent acidosis, hypoglycemia, hyperammonemia; metabolic disease is considered. Propionic acidemia was diagnosed on detection of c3-acyl carnitine: 17,7, glycine: 1369,3 in tandem mass spectrometry; mutation in the PCCA gene was detected in the genetic study. The general condition of the patient was improved and discharged with appropriate diet.

RESULTS: In the newborn patient, in case of unexpected symptoms and findings within a few days postoperatively; conditions such as surgical complications, sepsis, pneumonia, hypoxic encephalopathy and congenital heart diseases should be investigated. However, metabolic diseases such as PA should also be investigated in the unsolved cases where no ethiology can be found.

İzole Özofagus Atrezisi ve Propiyonik Asidemi Birlikteliği: Literatürdeki İlk Olgu

AMAÇ: Propiyonik asidemi (PA); valin, izolösin, lösin, metiyonin, treonin ve tek zincirli yağ asitlerinin metabolik yollarındaki propiyonil Coa karboksilaz (PCC) enziminin eksikliği sonucu oluşan metabolik bir hastalıktır. Görülme oranı 1/35000-1/75000 olan hastalık, beslenmeye başlama veya mamanın değiştirilmesi ya da enfeksiyon nedeniyle kusma, dehidratasyon, letarji, ensefalopati ile ortaya çıkar. İngilizce literatürde daha önce yayınlanmamış izole özofagus atrezisi (İÖA) ile PA birlikteliği saptadığımız yenidoğan hastamız sunulmuştur.

GEREÇ VE YÖNTEM: Sağlıklı 18 yaşında anneden (G1P1Y1, 36+6 hafta) 2300 gr sezeryan ile doğan antenatal ÖA tanılı kız bebeğin postnatal incelemelerinde İÖA saptanmış ve primer özofagus onarımı uygulanmıştır.

BULGULAR: Postoperatif izleminde genel durumunda kötüleşme, hipotoni saptanan hastanın incelemelerinde; metabolik asidoz (pH:7,09) ve yüksek anyon açığı, hipoglisemi (50mg/dL), nötropeni(300/uL), eritropeni (3260000/uL), trombositopeni(36000/uL) ile beraber Gamma Glutamil Transferaz 601 U/L, Trigliserid 1085 mg/dL, amonyak 195 ug/dL olduğu görülmüştür. Cerrahi komplikasyon saptanmayan hasta beslenmeye başlandığında belirtilere tonik kasılmalar ve nöbetlerin eklenmesi; inatçı asidoz, hipoglisemi, hiperamonyemi olması nedeniyle metabolik hastalık düşünülmüştür. Tandem mass spektrometisinde c3-açil karnitin:17,7, glisin:1369,3 saptanması üzerine propiyonik asidemi tanısı konulmuş; genetik çalışmada ise PCCA geninde mutasyon saptanmıştır. Uygun diyet düzenlemeleri ile genel durumu düzelen hasta poliklinik izlemi ile taburcu edilmiştir.

SONUÇ: Yenidoğan hastada, postoperatif birkaç gün içinde önceden düşünülmeyen belirti ve bulguların ortaya çıkmasında; öncelikle cerrahi komplikasyonlar, sepsis, pnömoni, hipoksik ensefalopati ve doğumsal kalp hastalıkları gibi patolojiler araştırılmalıdır. Ancak çözüm bulunamayan inatçı tablolarda PA gibi metabolik hastalıklar da araştırılmalıdır.
