New hope in the septic shock: plasmapheresis

Aim: Plasmapheresis is an increasingly common treatment option due to sepsis and septic shock in intensive care units. Plasmaferez can be defined as the removal of whole blood from the patient, the separation of the apheresis device into its components, and the return of the shaped elements to the patient. The purpose of this retrospective study was to discuss the demographic characteristics of the cases treated with plasmapheresis and the results of the follow-up in the literature. 

Material and Methods: Cases of plazmereperez for 1,5 years between January 2016 and June 2017 were reviewed retrospectively in the children's burn intensive care unit. The demographic characteristics, percentage of burns and effect, microbial agent that is cultured in pus, blood and tracheal aspirate culture and antibiogram susceptibilities, sepsis diagnosis and plasmapheresis application criteria, duration and mortality were evaluated. 

Results: A total of 19 sessions of plasmapheresis were performed in eight patients. A minimum of 1 and a maximum of 5 (mean 2.3) sessions per plasmapheresis case were performed. The ages were between 1.5 and 14.5 years (mean 6.3). ın our study, 62.5% (5) of the cases were male, 37.5% (3) were female. Burn percentage was between 23% and 60% (mean 41.5%). 50% (n = 4) of the patients were flammable and 25% (n = 2) had flame and inhalation burns. In other cases; 25% was hot water (n = 2), 12.5% ​​(n = 1) milk and 12.5% ​​(n = 1) concentrated liquid (soup) was burn effect. In 87.5% of the patients (n = 7), pseudomonas aeruginosa was detected in blood and pus culture. In only one case, candida albicans and acinetobacter baumannii were present. The duration of cases of sepsis varies between 8 and 82 days and plasmapheresis administration sepsis 1-18. days. Of the eight patients with severe sepsis who were treated with plasmapheresis, 75% were mortal and 25% were discharged with healing. 

Conclusion: Sepsis and septic shock are among the most important causes of mortality in patients receiving intensive care because of burns. Standard treatment plus plasmapheresis in catecholamine-resistant severe sepsis and septic shock treatment has begun to be included in intensive care monitoring as a promising treatment modality.

Amaç : Yoğun bakım ünitelerinde sepsis ve septik şok nedeniyle plazmaferez giderek artan sıklıkta uygulanan bir tedavi seçeneğidir. Plazmaferez, tam kanın hastadan alınarak aferez cihazı ile komponentlerine ayrılıp şekilli elemanların hastaya geri verilmesi olarak tanımlanabilir. Çalışmamızda retrospektif olarak hastanemiz yanık yoğun bakım ünitesinde tedavi gören ve plazmaferez uygulanan olguların demografik özellikleri ve izlem sonuçlarının literatür eşliğinde tartışılması amaçlandı.

Gereç ve yöntemler: Kliniğimiz çocuk yanık yoğun bakım ünitesinde ocak 2016 ile haziran 2017 tarihleri arasında 1,5 yıl süresince plazmeferez yapılan olgular retrospektif olarak incelendi. Olguların demografik özellikleri, yanık yüzdesi ve etkeni, pü, kan ve trakeal aspirat kültüründe üreyen mikrobiyal ajan ve antibiyogram duyarlılıkları, sepsis tanı ve plazmaferez uygulama kriterleri, süre ve mortalitesi değerlendirildi.

Bulgular: İzlemde sekiz olguya toplam 19 seans plazmaferez yapıldı. Plazmaferez olgu başına en az 1 en fazla 5 (ortalama 2,3) seans yapıldı. Olguların yaşları 1,5 ile 14,5 yıl (ortalama 6,3) arasındaydı. Olguların %62,5 (5) i erkek, %37,5(3) i kadındı. Yanık yüzdesi %23 ile %60 (ortalama %41.5) arasında idi. Hastaların %50(n=4) si alev yanığı idi ve %25(n=2’sinde alev ve inhalasyon yanığı birlikteydi. Diğer olgularda ; %25’i sıcak su(n=2) , %12,5’i (n=1) süt ve %12,5’i (n=1) konsantre sıvı (çorba) yanık etkeni idi. Hastaların %87,5’inde (n=7) kan ve püy kültüründe psödomonas aeruginosa üremesi saptandı. Sadece bir olguda candida albicans ve acinetobacter baumannii üremesi mevcuttu. Olguların sepsise girme süresi yanığının 8- 82. günleri arasında değişmekte olup plazmaferez uygulaması ise sepsisinin 1-18. günleri arasında yapıldı. Plazmaferez uygulanan ağır sepsiste olan sekiz olgunun %75’i mortal seyredip %25’ i şifa ile taburcu oldu.

Sonuç: Yanık nedeniyle yoğun bakımda tedavi görmekte olan hastalarda sepsis ve septik şok mortalitenin en önemli sebeplerinden biridir. Katekolaminlere dirençli ağır sepsis ve septik şok tedavisinde standart tedaviye ek plazmaferez uygulaması umut verici bir tedavi modalitesi olarak yoğun bakım izlemine dahil olmaya başlamıştır.
