Definitive surgery preference in hirschsprung disease and long-term results

Introduction: Follow-up in cases with Hirschsprung's disease (HH) includes diagnosis, surgical treatment and long-term follow-up. We aimed to evaluate the long-term outcome of our case by defining the diagnostic and treatment features of the cases with definitive operations performed with the diagnosis of HH in our clinic.

Methods: A total of 39 Hirschsprung’s disease cases were diagnosed and treated in the department between january 2009 and June 2017. We reviewed the charts of these retrospectively and evaluated the age, sex, the length of bowel involved, type of pull-through performed, functional results of treatment.

Result: There were 30 boys and 9 girls. The diagnosis age of the cases was 13.5 months and 71% (n = 28) were diagnosed before one year of age. The most frequent site of aganglionosis was at the rectosigmoid (87 %). A decompressing enterestomy was performed in 21 patients. After enterostomy; Duhamel-Martin as a definitive surgical procedure was performed in 13 cases, in two patients in Swenson and in five cases in transanal endorectal pull through (TEPT). TEPT was performed on 19 of the cases. In the postoperative period; no urinary incontinence and fecal incontinence. In total there were three cases of constipation (one Duhamel-martin and two TEPT) and responded to medical treatment. Three patients with Duhamel-Martin, two patients with TEPT developed enterocolitis, two patients with anal stenosis (after TEPT) responded to dilatation.

Conclusion: Duhamel-Martin technique was applied early in the surgical treatment of HH in our clinic and more TEPT was preferred in recent years. The choice of corrective surgery is based on the age of the patient, the possible aganglionic segment level and clinic.

Amaç: Hirschsprung Hastalığı (HH) olan olgularda takip; tanı, cerrahi tedavi ve uzun dönem izlemi kapsar. Çalışmamızda, kliniğimizde HH tanısı konularak definitif operasyonları yapılan olguların tanı ve tedavi özelliklerini belirleyerek uzun dönem sonuçlarını değerlendirmeyi amaçladık.

Yöntem: Ocak 2009 – Haziran 2017 yılları arasında HH tanısı alan 39 olgu tanı yaşı, ganglionik barsak segment lokalizasyonu, tedavi metodu, uzun dönem fekal ve üriner inkontinans, kabızlık gibi fonksiyonel sonuçlar açısından geriye dönük olarak değerlendirildi.

Bulgular: Toplamda 39 olgunun 30’u erkek (%76,9), 9’ u kızdı (%23,1). Olguların tanı yaşı 13,5 aylık olup %71’ i (n=28) bir yaşından önce tanı almıştı. Aganglionik barsak segmenti %87 rekto-sigmoid bölgede tespit edildi. Olguların 21’ine ostomi açıldı. Ostomi sonrası 13 olguya definitif cerrahi olarak Duhamel-Martin, iki olguya Swenson, beş olguya transanal endorektal pull through (TEPT) yapıldı. Olguların 19’una TEPT yapıldı. Postoperatif dönemde üriner ve fekal inkontinans izlenmedi. Biri Duhamel-Martin, ikisi TEPT sonrası gelişen toplam üç olguda konstipasyon mevcuttu, medikal tedaviye yanıt verdi. Üçü Duhamel-Martin, ikisi TEPT sonrası toplam beş hastada enterokolit, iki olguda anal darlık (TEPT sonrası) gelişti, dilatasyona yanıt verdi.

Sonuç: Sonuç olarak kliniğimizde HH’nin cerrahi tedavisinde erken dönemde Duhamel-Martin tekniği uygulanmış olup son yıllarda daha çok TEPT’in tercih edildiği görülmüştür. Düzeltici cerrahi tercihinde olgunun yaşı, olası aganglionik segment seviyesi ve kliniği göz önünde bulundurularak olguya özgün tercih yapılmaktadır.
