USE OF BIPEDICLE FLEP FOR GIANT OMPHALOCELE RECONSTRUCTION

Aim: omphalocele is a rare congenital anomaly represented with protruding intraabdominal organs covered with a sac of peritoneum and amniotic membrane through a midline abdominal wall defect. the defect of the fascia is generally more than 4cm and diameter of the sac is more than 8cm in the giant form. the aim of this case report is to present a giant omphalocele treated with bipedicle flap due to failure of standard procedures.

 

Materials and methods: The baby girl,who was diagnosed during the antenatal period, was born with C/S at the 39th week of gestation. her weight was 2940 gr and her physical examination revealed an omphalocele sac with a diameter of 10cm. the hernial sac was partially ruptured and was containing liver, stomach and small intestine. no additional abnormality was detected.

as multiple surgical attempts with silo were failed, bipedicle flap closure was planned in order to close the midline defect. bipedicle flaps were prapered with bilateral midaxillary vertical abdominal incisions. the defect was closed with these flaps sutured at the midline. bilateral vertical incisions were left healing secondarily and followed with daily topical antibiotic applications. at the 20th postoperative day the abdomen was totally healed and the patient was discharged.

Result and conclusion: Bipedicle flap technique can be a good surgical alternative while dealing with giant omphalecele, especially when multiple attempts of surgery is failed.     

 

 

DEV OMFALOSEL OLGUSUNDA BİPEDİCLE FLEP ONARIMI

Amaç: Omfalosel umbilikal seviyede gerçekleşen karın duvarı defekti sonucu karıniçi organların periton ve amniyotik membranla çevrili bir zarla çevrili olarak herniye olması ile karekterize nadir bir konjenital anomalidir. Dev omfaloselde fascia defekti 4cm ve üzerinde ve kesenin çapı da 8cm ve üzerindedir.

 

Bu yazıda kliniğimizde ‘‘bipedicle flap’’ ile tedavi edilen  dev omfalosel olgusu sunulmaktadır.

 

Gereç ve yöntem: Prenatal tanılı 39 hafta, 2640gram kız bebek ve sezeryan ile doğdu. 10cm çaplı kısmen hasar görmüş omfalosel kesesi içerisinde karaciğer, mide, barsaklar mevcuttu. Ek sorunu yoktu.

 

Silo onarımı ile çeşitli denemelerde kapatılamayan defekt, karın sağ ve sol duvarına orta aksiler seviyede yapılan vertikal kesi ile oluşturulan ve orta kaydırılan bipedicle flapler yardımı ile kapatıldı. Yanlardaki defektlere antibiyotikli pansumanlar yapılarak sekonder iyileşmeye bırakıldı. Onarım sonrası 20.günde hastamız karın duvarı onarımı yapılmış ve lateral duvar defektleri kapanmış halde taburcu edildi.

 

Sonuç ve tartışma: Bu yöntemin dev omfalosellerde veya çoklu uygulamalara rağmen karın duvarı kapatılamayan hastalarda uygun bir tedavi alternatifi olduğuu düşünüyoruz
