Evaluation of the additional anomalies in cases with hypospadias and undescended testes

Introduction: Hypospadias is a common genitourinary system (GUS) anomaly in boys. Undescended testes is frequently detected among accompanying anomalies in cases with hypospadias. Especially in proximal hypospadias, this association is increased and may indicate sexual differentiation disorders. In this study, it was aimed to evaluate the incidence togetherness of undescended testes and additional anomalies in children with hypospadias followed in clinic.

Patients and method: Between 2007 and 2016, data of 392 children who underwent surgery for hypospadias were screened retrospectively. Urethral meatus was present at proximal level (scrotal and penoscrotal) in 65 cases (16.6%) and distal level (glanular, coronal, subcoronal, midpenile) in 327 cases (83.4%). The cases were divided into two groups as those with both testes in scrotum and those with undescended testes and anomalies is recorded.

Results: The mean age of the children with proximal hypospadias was 38.4 months (6-240 months). Of the children with proximal hypospadias, 26 (40%) had undescended testes and 39 (60%) had testes in the scrotum. Undescended testes was detected bilaterally in 17 patients (65.4%) and unilaterally in nine patients (34.6%) in the undescended testes group. In the undescended testis group, 24 patients (92.3%) had suspected external genitalia on physical examination. Sixteen of these patients had bilateral undescended testes and eight had unilateral undescended testes. Fourteen of these patients (53.8%) had associated genetic endocrinological disease (5-alpha reductase deficiency (n = 4), partial androgen receptor insensitivity (n = 4), congenital adrenal hyperplasia (n = 3), gonadal dysgenesis = 2), Klinefelter syndrome (n = 1)) were found. Ten patients (25.5%) had suspicious external genitalia in the physical examination in the proximal hypospadias group who had both testes in scrotum. Four of them (10.3%) had associated genetic endocrinological disease (partial androgen receptor insensitivity (n = 3), congenital adrenal hyperplasia (n = 1)). In the undescended testes group, three patients (11.5%) had additional urinary pathology (left horseshoe kidney (n = 1), left multicystic dysplastic kidney (n = 1), antenatal hydronephrosis (n =1)) and ten patients (38.5%) had other system anomalies (cardiac anomaly (n=3), neurological anomaly (n=2), anal atresia (n=1), Down syndrome (n=1), gynecomastia (n=1), hypothyroidism (n=1) and polydactyly (n=1)). Additional urinary pathology  in one patient (2,5%) (hydronephrosis (n = 1)) and other system anomalies in five (12.8%) patients (inguinal hernia (n=2), cardiac anomaly (n=1), total alopecia (n=1), and allergic asthma (n=1)) of the 39 patients who had both testes in the scrotum was detected.

The mean age of children with distal hypospadias was 45.6 months (6-156 months). Nine of 327 children with distal hypospadias (2.8%) had undescended testes. Of these, five were bilateral (55.6%) and 4 had unilateral undescended testes (44.4%). None of the patients with distal hypospadias (0%) had any suspicious genitalia on physical examination and these cases had no associated genetic endocrinological diseases. In the undescended testes group, one patient (11.1%) had additional urinary pathology (left horseshoe kidney (n=1)) and one patient had other system anomalies (Down syndrome (n=1)). Seven patients (2.2%) who had both testes in scrotum had additional urinary pathology (left multicystic dysplastic kidney (n=2), antenatal hydronephrosis (n=2), right vesicoureteral reflux (n=1), kidney stone (n=1) left Wilms tumor (n=1)), and 32 patients (9.8%) had other system anomalies (inguinal hernia(n=9), neurological (n=5), allergic-immunologic (n=5), cardiac (n=4), hypothyroidism (n=2), anal atresia (n=2), skeletal anomalies (n=2), Hirschsprung's disease (n=1), hemophilia (n=1), familial Mediterranean fever (n=1). Conclusion: The developmental disorder caused by proximal hypospadias associated with undescended testis is thought to occur  in the earlier stage of GUS maturation and the more serious anomalies, sexual differentiation disorders, and comorbid genetic endocrinological diseases that concern the reproductive system are common in these patients compared to other patient groups. For this reason, it should be kept in mind that further tests may be needed in cases where the undescended testis is accompanied by proximal hypospadias. However according to do results we suggest that additional anomalies should be investigated in children all hypospadias children.

Hipospadias ve inmemiş testis birlikteliğinde ek anomali sıklığının değerlendirilmesi

Giriş: Hipospadias erkek çocuklarda sık görülen bir genitoüriner sistem (GÜS) anomalisidir. Hipospadias olgularında eşlik eden anomaliler arasında inmemiş testis saptanması sıktır. Özellikle proksimal hipospadiasta bu birliktelik artar ve cinsel farklılaşma bozukluklarına işaret edebilir. Bu çalışmada klinikte takip edilen hipospadiaslı çocuklarda inmemiş testis sıklığı ile ek anomalilerinin değerlendirilmesi amaçlandı.

Hastalar ve yöntem: 2007-2016 yılları arasında hipospadias nedeniyle ameliyat olan 392 çocuğun verileri geriye dönük olarak tarandı. Bunlardan 65’inde (%16,6) olguda meatus proksimal seviyede (skrotal ve penoskrotal), 327’sinde (%83,4) distal seviyede idi (glanüler, koronal, subkoronal, midpenil). Olgular her iki testisi yerinde olan ve inmemiş testise sahip olanlar olarak ayrıldı ve ek anomaliler kaydedildi.

Bulgular: Proksimal hipospadiaslı çocukların ortalama yaşı 38,4 ay(6-240 ay) idi. Proksimal hipospadiaslı çocuklardan 26’sının(%40) inmemiş testisi mevcutken, 39 (%60) olguda testisler skrotumda ele geldi. İnmemiş testisi olan gruptaki hastaların 17’sinde bilateral (%65,4), dokuzunda tek taraflı (%34,6) inmemiş testis saptandı. İnmemiş testisi olan grupta 24 hastada (%92,3) fizik muayenede kuşkulu dış genitalya mevcuttu. Bu hastaların 16’sında bilateral inmemiş testis, 8’inde tek taraflı inmemiş testis mevcuttu. Bu hastaların 14’ünde (%53,8) yandaş  genetik endokrinolojik hastalık ( 5-alfa reduktaz eksikliği(n=4), parsiyel androjen reseptör duyarsızlığı(n=4), konjenital adrenal hiperplazi(n=3), gonadal disgenezi(n=2), Klinefelter sendromu(n=1)) saptandı. Her iki testisi inmiş olan proksimal hipospadiaslı grupta on hastanın (%25,5) fizik muayenesinde kuşkulu dış genitalya mevcuttu. Bunların dördünde (%10,3) yandaş genetik endokrinolojik hastalık (parsiyel androjen reseptör duyarsızlığı(n=3), konjenital adrenal hiperplazi (n=1)) vardı. İnmemiş testisi olan grupta üç hastada(%11,5) ek üriner patoloji ( sol atnalı böbrek (n=1), sol multikistik displastik böbrek (n=1), antenatal hidronefroz (n=1)), on hastada (%38,5)  ise diğer sistem anomalilerinin (kardiyak anomali(n=3), nörolojik anomali(n=2), anal atrezi(n=1), Down sendromu(n=1), jinekomasti(n=1), hipotiroidi(n=1), polidaktili(n=1)) de eşlik ettiği saptandı.  Her iki testisi skrotumda ele gelen 39 hastanın birinde (%2,5) ek üriner patoloji (hidronefroz(n=1)), beşinde (%12,8) ise diğer sistem anomalilerine ( kasık fıtığı(n=2), kardiyak anomali(n=1), total alopesi(n=1), alerjik astıma(n=1)) rastlandı.

Distal hipospadiaslı çocukların ortalama yaşı 45,6 ay(6-156 ay) idi. Distal hipospadiaslı 327 çocuğun 9’unda (%2,8) inmemiş testis mevcuttu. Bunlardan beşinde bilateral (%55,6) , 4’ünde tek taraflı (%44,4) inmemiş testis izlendi. Distal hipospadiası olan hastaların hiç birinde (%0) fizik muayenede kuşkulu genitalya saptanmadı ve bu olguların yandaş  genetik endokrinolojik hastalıkları yoktu. İnmemiş testisi olan grupta bir hastada(%11,1) ek üriner patoloji ( sol atnalı böbrek (n=1)),  bir hastada (%11,1)  ise diğer sistem anomalilerinin (Down sendromu(n=1)) de eşlik ettiği saptandı.  Her iki testisi skrotumda ele gelen yedi hastada (%2,2) ek üriner patoloji (sol multikistik displastik böbrek(n=2), antenatal hidronefroz(n=2), sağ vezikoüreteral reflü(n=1), böbrek taşı(n=1), sol Wilms tümörü(n=1)), 32 hastada (%9,8) ise diğer sistem anomalilerine ( kasık fıtığı(n=9), nörolojik (n=5), alerji-immunolojik(n=5), kardiyak(n=4), hipotiroidi(n=2), anal atrezi(n=2), iskelet anomalisi(n=2) Hirschsprung hastalığı(n=1), hemofili(n=1), Ailevi Akdeniz ateşi(n=1)) rastlandı.

Sonuç: İnmemiş testisin eşlik ettiği proksimal hipospadiaslı olgularda neden olan gelişimsel bozukluk GÜS olgunlaşmasının daha erken aşamalarında oluştuğu düşünüldüğünden, bu hastalarda üreme sistemini ilgilendiren daha ciddi anomaliler, cinsel farklılaşma bozuklukları ve yandaş genetik endokrinolojik hastalıklara diğer hasta gruplarına göre sık rastlanmaktadır. Bu nedenle inmemiş testisin proksimal hipospadiasa eşlik ettiği durumlarda daha ileri tetkikler gerekebileceği belirgindir. Bununla birlikte bulgularımız ışığında inmemiş testis olsun ya da olmasın tüm hipospadiaslı çocukların ek anomali açısından incelenmesini öneriyoruz.
