Gradual special approach to every patient with short bowel syndrome by intestinal elongation

Aim: To present our experience on a patient with a SBS(short bowel syndrome) because of complicated gastroschisis who’s TPN (total parenteral nutrition) was stopped by the help of 3 staged  anisoperistaltic and isoperistaltic intestinal elongation.

Case: With a diagnose of complicated Gastroschisis On the 16th week of antenatal life and ileal atresia,6 month old patient who  was operated in another hospital, nearly total ileocolectomy and proximal jejenostomy was performed and directed to our clinic. Patient was 100% TPN dependent. At  examination 50 cm jejunum  and  15 cm colon was found. Firstly, narrowed jejunocolic anostomosis was performed by tapering to dilate jejunum and to decrase fluid loss. As a second stage after four weeks,  anostomosis was broke up to perform anisoperistaltic intestinal elongation (Bianchi) to distal 20 cm distal jejunum. During the post operative 5 month TPN dependence was decreased however no gain was supposed on weight, so third stage isoperistaltic Biachi(isoperistaltic, 15cm to proximal of anisoperistaltic segment)  was performed. In the per operative follow up no complication was observed and in the  post operative  3 week patient was discharged to home with a fully oral feeding as weel as weight gain.

Conclusion: It is required to approach specially to every SBS patient. Gradual approach with 3 stage (anisoperistaltic and isoperistaltic intestinal elongation) stands to be effective and safe.

Kısa barsak sendromunda hastaya özel 3 aşamalı yaklaşım ile barsak elongasyon

Amaç:

 Komplike gastroşizis nedeniyle kısa barsak sendromu (KBS) ve   aşamalı yaklaşım ile 3 seansta  total parenteral nutrisyon (TPN) ihtiyacı ortadan kalkan bir olgudaki deneyimimizin sunulması amaçlanmıştır.

 

Olgu:     Antenatal16. haftada tanılı komplike gastroşizis ve ileal atrezi nedeniyle başka bir merkezde totale yakın ileokolektomi ve proksimal jejunostomi uygulanan 6 aylık erkek olgu merkezimize yönlendirildi. Olgu %100 TPN bağımlı idi. Yapılan değerlendirme  sonunda olguda 50 cm jejunum, 15      

cm kolon olduğu saptandı. Olguya öncelikle tapering ile daraltılmış bir jejunokolik anastomoz uygulanarak jejunumda dilatasyon  sağlandı. Dört hafta sonra ikinci seansta ,

anastomoz bozularak mevcut jejunumun distal 20 cm’lik bölümüne pasajı yavaşlatıp sıvı emilimi sağlanması amacıyla  anizoperistaltik (Bianchi) barsak elongasyonu uygulandı. Postoperatif 

5 aylık dönemde TPN bağımlılığı azalan ancak kilo alımı olmayan olguya 3. Seansta izoperistaltik

 Bianchi barsak elongasyonu (anizoperistaltik segmentin proksimalindeki 15cm’ lik segmente) uygulandı. Peroperatif izleminde sorun olmayan olgu postoperatif

3. Haftada tamamen oral beslenerek taburcu edildi.

Sonuç:

 Kısa barsak sendromu tanılı hastalara yaklaşımda her hastaya özel değerlendirme yapılmalıdır. Aşamalı yaklaşım ile anizoperistaltik-izoperistaltik barsak elongasyonu etkin ve güvenli görünmektedir.
