Introduction

The most frequent emergent surgical operation performed in children is that required for acute appendicitis. With accumulation of knowledge and experience, laparoscopic appendectomy has become a standard procedure in current practice.

In this study, we compared the operation time, duration of hospital stay, and complications for laparoscopic appendectomies in children performed with intracorporeal knot ligation and those performed with LigaSureTM sealing for ligating the appendiceal stump.

Materials and methods

Data from 74 patients with acute appendicitis who underwent laparoscopic appendectomy with intracorporeal knot ligation (group A, n = 33) or LigaSureTM stump sealing (group B, n = 31) in our clinic between October 2016 and August 2017 were evaluated retrospectively. 10 patients were excluded from the study.

Results

Of 64 patients, 34 (53.1%) were female and 30 (46.9%) were male. The average operation times were 29.6 min in group A and 18.9 min in group B. No significant differences in age or duration of hospital stay were observed between groups A and B (p = 0.735 and p = 0.242, respectively), but the operation time differed significantly (p < 0.001).

Conclusion

LigaSureTM use in laparoscopic appendectomy is a safe, inexpensive, and rapid method for sealing of appendiceal stump in appendicitis treatment.

Çocuklarda en sık yapılan acil cerrahi girişim akut apandisittir. Günümüzde deneyim ve tecrübenin artmasıyla laparoskopik apendektomi standart hale gelmiştir.

Bu çalışmada ligasure (Covidien, USA) ile intrakorpeoreal düğüm ile yapılan laparoskopik apendektomilerde ameliyat sürelerini, hastanede kalış süresini ve komplikasyonları karşılaştırmayı amaçladık.

MATERYAL VE METOD

Ekim 2016 ile ağustos 2017 tarihleri arasında kliniğimizde apendiks güdüğünün intrakorporeal düğüm (grup A, n=33) ve ligasure ile mühürlendiği (grup B, n=31) 74 hastanın dosyası retrospektif olarak incelendi.

SONUÇLAR

Toplam 64 hastanın 34’u kız (%53,1), 30’U erkek(%46,9) idi. Grup A ve grup B arasında yaş ve hastanede kalış süresi açısından anlamlı fark saptanmadı (sırasıyla p=0,735 ve p=0,242) ancak operasyon süreleri arasındaki fark istatistik olarak anlamlı (p<0,001) saptandı.

Laparoskopik apendektomide apendiks güdüğünü bağlamak için ligasure kullanımı güvenli, ucuz ve hızlı bir yöntemdir.
