Three cases of cystıc bılıary atresıa mısdıagnosed as choledochal cyst

Introduction: Cystic biliary atresia is an uncommon variant of biliary atresia. Prognosis may be favorable by early evaluation of misdiagnosis as choledochal cyst with antenatal USG. In this study, we present three cases of cystic biliary atresia misdiagnosed as choledochal cyst.

Case reports:

Case 1: 17-week-old female patient delivered vaginally, diagnosed antenatally with USG as choledochal cyst. On the postnatal 23rd day, cyst excision and Kasai portoenterostomy were performed. Liver biopsy revealed ductal cell proliferation and fibrosis. In the postoperative 22th month, liver function tests, direct bilirubin level, growth and development were normal.

Case 2: A 35-week-old female delivered vaginally was diagnosed with choledochal cyst on the 4th postnatal day. On the postnatal 21th day, Roux-en-Y portoenterostomy was performed. Congenital hepatic fibrosis was detected in liver biopsy. In the postoperative 23rd month, liver function tests, direct bilirubin level, growth and development were normal.

Case 3: A female patient delivered cesarean birth was diagnosed as antenatal choledochal cyst. On the 24th postnatal day, Roux-en-Y portoenterostomy was performed. Liver biopsy showed no finding other than hemosiderin pigment in parenchyma. In the postoperative 33rd month, liver function tests, direct bilirubin level, growth and development were normal.

Conclusion: Cystic biliary atresia cases misdiagnosed as choledochal cyst with antenatal USG have a favorable prognosis due to earlier diagnosis and earlier surgical intervention.

Koledok kisti olarak yanlış tanı almış 3 kistik biliyer atrezili olgu sunumu

Giriş: Kistik biliyer atrezi, biliyer atrezinin yaygın olmayan bir varyantıdır. Antenatal USG ile koledok kisti olarak yanlış tanımlanması, erken tanı konulmasını sağlayarak prognozu olumlu etkileyebilmektedir. Bu çalışmada, koledok kisti tanısı konulmuş 3 kistik biliyer atrezi olgusu sunulmuştur.

Olgu sunumu:

Olgu 1: Vajinal yolla doğan term kız olgu antenatal USG ile 17. haftada koledok kisti tanısı almış. Postnatal 23. günde kist eksizyonu ve Kasai portoenterostomi uygulandı. Karaciğerden alınan biyopside duktal hücre proliferasyonu ve fibrozis saptandı. Postoperatif 22. ayda karaciğer fonksiyon testleri, direkt bilirubin değeri, büyüme ve gelişme olağan saptandı.

Olgu 2: Vajinal yolla 35 haftalık doğan kız olgu postnatal 4. günde koledok kisti tanısı almış. Postnatal 21. günde Roux-en-Y portoenterostomi uygulandı. Karaciğer biyopsisinde konjenital hepatik fibrozis saptandı. Postoperatif 23. ayda karaciğer fonksiyon testleri, direkt bilirubin değeri, büyüme ve gelişme olağan saptandı.

Olgu 3: Sezaryan ile doğan term kız olgu antenatal koledok kisti tanısı almış. Postnatal 24. günde Roux-en-Y portoenterostomi uygulandı. Karaciğer biyopsisinde parankimde hemosiderin pigmenti dışında bulgu izlenmedi. Postoperatif 33. ayda karaciğer fonksiyon testleri, direkt bilirubin değeri, büyüme ve gelişme olağan saptandı.

Sonuç: Antenatal USG ile koledok kisti olarak yanlış tanı alan kistik bilier atrezi olgularında erken tanı ve erken cerrahi prognoza olumlu katkıda bulunmaktadır.
