Intestinal perforation caused by multiple magnet ingestion: case report

Introduction: Ingestion of foreign bodies is common in childeren those between 6 months and 3 years of age. The most amount of foreign bodies passes gastrointestinal system (GIS) spontaneously without occuring any complication. Magnetic foreign body ingestion cases are increased due to explosion in magnetic toys, desk toys and jewelleries in the lat 10 years. İngestion of multiple magnetic foreign bodies may lead perforation, fistula, volvulus in gastrointestinal system.

Herein we aimed to present a patient operated for ileo-ileal perforation and fistula caused by swallowed multiple magnets.

Case: Eight year-old male addmitted with abdominal pain and nousia. Right lower quadrant pain and defense was examined. Gas fluid levels especially on right lower quadrant and foreign body image in the central of abdomen was seen on standing X-ray graphy. Dilated bowels and moderately pelvic fluid was reported by ultrasonography. Diagnostic laparoscopy was done with suspicion of acute abdomen. GIS perforation was detected and laparoscopy was terminated and turned to laparotomy. There was two ileal perforations and two connected couples of magnets trough. Magnets were removed and perforations were primarily sutured. Operation was ended after floroscopic residual foreign body check. The patient was externed without any complication by post operative day 4.

Coclusion: Swallowed multiple magnets requires surgical removal until any complication occured. It must be ensured that no residual magnets were left by floroscopic or X-ray imaging during the operation.

Çoklu mıknatıs yutmaya bağlı intestinal perforasyon: olgu sunumu

Giriş: Yabancı cisim yutulması daha çok 6 ay-3 yaş arası çocuklarda görülmektedir. Yutulan yabancı cisimlerin çoğu sorunsuz bir şekilde gastrointestinal sistemden (GİS) spontan olarak çıkmaktadır. Son 10 yılda mıknatıslı oyuncak, kırtasiye malzemeleri ve takıların artışına bağlı olarak mıknatıslı yabancı cisim yutulmasına daha fazla rastlanmaktadır. Çoklu mıknatıs yutulması GİS’te fistül, volvulus veya perforasyona yol açabilmektedir.

Çoklu mıknatıs yutma sonrası ameliyat edilen, ileo-ileal fistül ve perforasyon tespit edilen bir hastayı sunmayı amaçladık.

Olgu: Sekiz yaşında erkek hasta karın ağrısı ve kusma şikayeti ile başvurdu. Fizik muayenesinde sağ alt kadranda hassasiyet ve defansı vardı. Ayaktan direkt karın grafisinde sağ alt kadranda daha belirgin olmak üzere hava-sıvı seviyesi ve orta hatta yabancı cisim görüntüsü var idi. Ultrasonografide dilate barsak ansları ve orta düzeyde pelvik serbest sıvı raporlandı. Akut batın ön tanısı ile tanısal laparoskopi yapıldı. Laparoskopide GİS perforasyonu tespit edilerek laparotomi yapıldı. İleumda iki ayrı yerde perforasyon ve içerisinde birbirine yapışık iki çift mıknatıs görüldü. Mıknatıslar çıkarılarak perforasyonlar primer onarıldı. Floroskopi ile başka mıknatısın kalmadığı görülerek ameliyat sonlandırıldı. Ameliyat sonrası 4. gün hasta sorunsuz bir şekilde taburcu edildi.

Sonuç: Yutulmuş çoklu mıknatıslar herhangi bir komplikasyon gelişmeden cerrahi olarak çıkarılmalıdır. Ayrıca ameliyat sırasında floroskopi veya direkt grafi çekilerek rezidü mıknatısın bırakılmadığından emin olunmalıdır.
