A rare complication associated with TPN: Wernicke's encephalopathy

Introduction: Wernicke's encephalopathy (WE) is an acute, severe neurological table due to thiamine deficiency. Alcoholism and severe malnutrition are among the most common causes, but may also result from long-term parenteral nutrition. We present a case of WE due to long-term total parenteral nutrition (TPN) treatment in a patient who was operated for perforated appendicitis.

Case: 11 years old girl was admitted with fever lasting 1 week, abdominal pain, vomiting and impaired general condition. Physical examination revealed extensive rebound tenderness in the abdomen. The patient underwent emergency laparotomy due to acute abdomen. Perforated appendicitis and widespread adhesions and abscesses were present. She was followed up postoperatively in intensive care unit. On the 5th day of the follow-up, the patient was re-operated due to intestinal contents leakage from the drains. There were punctate perforations in three locations in the ileum secondarily to intra-abdominal adhesions. Primary repair was performed. The patient was followed up with broad-spectrum antibiotics. TPN treatment was given for 20 days because there was no oral intake. The patient who had enough oral intake and did not need antibiotics and intravenous fluids was examined on the complaints of sudden blurred vision, double vision, ataxic gait, and left eye shift at the discharge phase. There was a loss of consciousness in the patient. On magnetic resonance imaging of the patient, there were atrophy in cerebellum and diffusion restricted areas in bilateral posteromedial thalamus sections. Wernicke's encephalopathy is considered as diagnosis. Double vision, hallucinations, ataxic movements, complaints of blurring of consciousness improved after B vitamin complex treatment had been started.

Conclusion: In patients with prolonged parenteral nutrition, sudden onset of consciousness, visual impairment and gait disturbance, WE should be considered at the differential diagnosis and early treatment should be initiated.

Giriş: Wernicke ensefalopatisi (WE) tiamin eksikliğine bağlı gelişen akut, ağır bir nörolojik tablodur. Alkolizm ve ciddi malnutrisyon en sık nedenler arasında olmakla birlikte uzun süreli parenteral beslenme sonucu da görülebilir. Bu sunuda perfore apandisit nedeniyle ameliyat edilen hastada uzun süre total parenteral nutrisyon (TPN) tedavisine bağlı gelişen WE olgusu sunuldu.

Olgu: 11 yaş, kız, 1 haftadır olan ateş, karın ağrısı, kusma, genel durumunda bozulma ile başvurdu. Fizik muayenesinde batında yaygın rebound mevcuttu. Hasta akut batın düşünülerek acil laparatomiye alındı. Perfore apandisit ve batın içinde yaygın yapışıklık ve abse mevcuttu. Postoperatif mekanik ventilatörde yoğun bakımda izlendi. İzleminin 5. gününde drenlerinden intestinal içerik gelmesi üzerine hasta tekrar opere edildi. Batın içi yapışıklıklara sekonder ileumda üç yerde noktasal perforasyonlar olduğu görüldü. Primer olarak onarıldı. Hasta geniş spektrumlu antibiyotik ile takip edildi. Hastaya oral alımı olmadığı için 20 gün TPN tedavisi verildi. Takibinde oral alımı yeterli olan, antibiyotik ve mayi gereksinimi kalmayan hasta taburculuk aşamasında, ani gelişen bulanık görme, çift görme, ataksik yürüyüş, sol gözde içe kayma şikayetleri üzerine tetkik edildi. Hastada bilinç kaybı gelişti. Hastanın manyetik rezonans görüntülemesinde serebellar folyolarda atrofi, bilateral talamus posteromedial kesimlerinde difüzyon kısıtlayan alanlar görüldü. Klinikle bir arada düşünülünce Wernicke ensefalopatisi tanısını düşündürdü. Hastaya B vitamin kompleksi tedavisi başlandı. Çift görme, halüsinasyon, ataksik hareketler, bilinç bulanıklığı şikayetleri düzeldi.

Sonuç: Uzun süreli parenteral beslenen hastalarda ani gelişen bilinç bulanıklığı, görme bozukluğu ve yürüme bozukluğunda ayırıcı tanıda WE düşünülmeli ve erken dönemde tedaviye başlanmalıdır.
