Evaluation of the Relation Between Anastomosis Technique and Complications in Esophageal Atresia

Background and Aim: Numerous methods have been described for esophageal anastomosis in primary esophageal repair in cases with esophageal atresia. In this study, it was aimed to reveal the relationship between anastomosis tecnique and postoperative complications in cases of primary esophagus repair.

Materials and Methods: Records of all patients who were hospitalized with esophageal atresia between January 2004-January 2018 were reviewed retrospectively. The results were analyzed with anastomotic leakage, anastomotic stricture, stricture with requirement of esophageal dilatation and recurrent tracheoesophageal fistula in which primary esophageal repair practiced like oblique-to-oblique or oblique-to-end/end-to-end techniques.

Results: During 14 years, 172 esophageal atresia patients were hospitalized and 167 of them were operated; 121 patients (%72.4) discharged, and 46 patients (%27.5) died. Primary esophageal repair were done in 147 neonates; 42 of 147 patients (%28.5) had been oblique-to-oblique anastomosis, and 105 of 147 patients (%71.4) had been end-to-oblique/end-to-end anastomosis. %17.8 anastomotic leakage and %27.6 anastomotic stricture with required of esophageal dilatation and %4.3 recurrent tracheoesophageal fistula determined in 121 survived cases.

28 of 121 survived patients (%25) had been repaired with oblique-to-oblique tecnique, and 84 of these (%75) had been repaired with end-to-end/end-to-oblique anastomosis. Totally 69 esophageal dilatations were used in 30 patients; the mean dilatation number was 2. In the oblique-to-oblique method, anastomotic leakage was found to be 58% less frequently; anastomotic stenosis was detected 60% more, but anastomotic stricture with required esophageal dilatation was 6% less than the other tecnique. Fistula recurrence developed in 20% of cases with anastomotic leakage.

There was no statistically significant difference between anastomotic techniques in terms of anastomotic leakage and fistula recurrence, and was statistically significant difference in terms of anastomotic stricture. There was no statistically significant difference between the anastomotic leakage, anastomotic stricture, dilatation requiring anastomotic stricture and fistula recurrence rates in terms of the extrapleural or intrapleural completion of the thoracotomy.

Conclusion: The primary esophagus repair with oblique-to-oblique tecnique compared to end-to-end/end-to-oblique showed better results in terms of anastomotic leakage and anastomotic stricture required dilatation. It should not be forgotten that anastomosis technique you will prefer during primary esophageal repair of atresia will affect mortality and morbidity.

Özofagus Atrezilerinde Anastomoz Tekniğinin Komplikasyonlar ile İlişkisinin Değerlendirilmesi

Giriş/Amaç: Özofagus atrezisi olgularının primer özofagus onarımında özofagus anastomoz tekniği konusunda pek çok yöntem tariflenmiştir. Bu çalışmada, primer özofagus onarımı yapılan olgularda anastomozun tekniği ile postoperatif komplikasyonlar arasındaki ilişkinin ortaya konulması amaçlandı.

Gereç ve Yöntem: Ocak 2004-2018 yılları arasında kliniğimizde özofagus atrezisi nedeniyle yatırılan yenidoğanların tüm hastane kayıtları retrospektif olarak incelendi. Oblik-oblik veya uç-uç/uç-oblik anastomoz ile primer özofagus onarımı yapılan özofagus atrezili olgularda anastomoz kaçağı, anastomoz darlığı, dilatasyon gerektiren anastomoz darlığı ve trakeoözofageal fistül rekürrensi açısından sonuçlar analiz edildi.

Bulgular: 14 yıllık sürede interne edilen 170 özofagus atrezili yenidoğanın 167’si opere edildi. 121’i (%72.4) taburcu edildi; 46’sı (%27.5) kaybedildi. Fistül ligasyonu ve primer özofagus onarımı yapılan 147 yenidoğanın 42’sinde (%28.5) oblik-oblik, 105’inde (%71.4) uç-uç veya uç-oblik anastomoz yapıldı. Bunlardan yaşayan 112 olgunun %17.8’inde anastomoz kaçağı, %27.6’sında dilatasyon gerektiren anastomoz darlığı ve %4.3’ünde trakeoözofageal fistül rekürrensi saptandı. Yaşayan 121 hastanın 28’inde (%25) oblik-oblik, 84’ünde (%75) uç-uç/uç-oblik anastomoz yapılmıştı. Anastomoz darlıklı 30 hastada ortalama dilatasyon sayısı 2 olup toplam 69 dilatasyon uygulandı. Oblik-oblik yöntemde anastomoz kaçağı %58 oranında daha az görüldü; %60 oranında daha fazla darlık saptandı, ancak dilatasyon gerektiren darlık %6 oranında daha az görüldü. Anastomoz kaçağı olgularının %20’sinde fistül rekürrensi gelişti. Oblik-oblik ve diğer anastomoz teknikleri ile opere edilen hastalar arasında anastomoz kaçağı ve fistül rekürrensi açısından istatistiksel olarak anlamlı farklılık olmadığı ; anastomoz darlığı açısından ise istatistiksel olarak anlamlı farklılık olduğu tespit edildi. Torakotominin ekstraplevral veya intraplevral tamamlanması ile anastomoz kaçağı, anastomoz darlığı, dilatasyon gerektiren anastomoz darlığı ve fistül rekürrensi oranları arasında istatistiksel olarak anlamlı farklılık bulunmadığı tespit edildi.

Sonuç: Oblik-oblik yöntemle primer özofagus onarımı yapılması, uç-uç/uç-oblik’e göre anastomoz kaçağı ve dilatasyon gerektiren anastomoz darlığı açısından daha iyi sonuçlar verdiği saptandı. Özofagus atrezisinin primer onarımı sırasında tercih edeceğiniz anastomoz tekniğinin mortalite ve morbiditeyi etkileyeceği unutulmamalıdır.
