The efficacy of subureteric injection in children with duplex system of vesicoureteral reflux

Aim: To evaluate the efficacy of subureteric injection in children with dublex system of vesicoureteral reflux (VUR)

Patients and Method: Patients with VUR diagnosed and duplex system, including 2008-2017, were retrospectively screened. The results were assessed for complete recovery, continued VUR and open surgery.

Results: Subureteric injection was performed in 16 patients with VUR with a double system in 13 (81%) female and 3 (19%) male. The mean age of the patients was 6,13 ± 3,46. Seven patients had complete, 5 patients had incomplete and 1 patient had bilateral duplex system (right: complete, left: incomplete) and 3 patients did not record. 11 of the patients were injected 1 time, 3 of the patients were injected 2 times, and 2 of the patients were injected 3 times. None of the patients developed complications in the early post-injection period. 12 patients were evaluated with VCUG and 1 patient with USG. 9 of 16 patients (56%) were found to have no VUR after injection. 3 (18%) patients did not regress after VUR injection, open surgery was performed. 2 patients came out of follow-up, 2 patients still continue to follow.

Conclusion: Patients with duplex system of VUR are generally treated by open surgery. In this study, complete recovery was achieved in half of the patients after subureteric injection. In patients with duplex system, subüreteric injection, which is a less invasive procedure before giving open surgery, may be considered the first.

Vezikoüreteral reflü tanılı çift sisteme sahip çocuklarda subüreterik enjeksiyonun etkinliği

Amaç: Vezikoüreteral reflüsü (VUR) olup, çift sisteme sahip çocuk hastalarda subüreterik enjeksiyonun etkinliğinin değerlendirilmesi amaçlandı.

Hastalar ve Yöntem: 2008-2017 yılları dahil, VUR tanılı ve çift sistemli hastalar geriye dönük tarandı. Sonuçlar tam iyileşme, VUR’ un devam etmesi ve açık cerrahiye gitme yönünden değerlendirildi.

Bulgular: 13’ ü (%81) kız, 3’ ü (%19) erkek olan çift sisteme sahip VUR tanılı 16 hastaya subüreterik enjeksiyon yapıldı. Hastaların yaş ortalaması 6,13 ± 3,46 yıldı. 7 hastada komplet, 5 hastada inkomplet ve 1 hastada bilateral çift sistem (sağ: komplet, sol: inkomplet) mevcuttu; 3 hastanın kayıtlarında bu bilgi yoktu. Hastaların 11’ ine 1 kez, 3’ üne 2 kez ve 2’ sine 3 kez enjeksiyon yapıldı. Hastaların hiçbirinde enjeksiyon sonrası komplikasyon gelişmedi. Takipte 12 hasta işeme sistoüretrografisi ile ve 1 hasta USG ile değerlendirildi. 16 hastanın 9’unda (%56) enjeksiyon sonrası VUR olmadığı görüldü. 3 (%18) hastada enjeksiyon sonrası VUR da gerileme olmadı, açık cerrahi yapıldı. 2 hasta takipten çıktı, 2 hastanın halen takibi devam etmektedir.

Sonuç: Çift sisteme sahip VUR tanılı hastalar genel olarak açık cerrahi ile tedavi edilir. Bu çalışmada subüreterik enjeksiyon sonrası hastaların yarısında tam iyileşme sağlanmıştır. Çift sisteme sahip hastalarda açık cerrahi kararı vermeden önce daha az invaziv bir işlem olan subüreterik enjeksiyon ilk planda düşünülebilir.
